WASHINGTON (AP) — Before COVID-19 killed thousands of nursing home residents, about 4 in 10 homes inspected were cited for infection control prob-lems, according to a gov-ernment watchdog report Wednesday that finds a “persistent” pattern of lapses.
In light of the pan-demic, seemingly minor cutting of corners such as an employee caring for residents while battling a cold has taken on new significance.
“Warning signs were ignored and nursing homes were unprepared to face a pandemic,” said Sen. Ron Wyden of Oregon, the top Demo-crat on a committee that oversees Medicare and Medicaid. “There need to be big changes in the way nursing homes care for seniors.”
The report from the Government Account-ability Office found that state inspectors who help enforce federal nursing home standards classified the overwhelming major-ity of violations as not severe, generally meaning there was no actual harm to residents. The federal Centers for Medicare and Medicare Services carried out enforcement actions for 1% of viola-tions classified as not severe from 2013-2017, the report said.
Nursing homes ended up bearing the brunt of the coronavirus outbreak. About 1.4 million people live in some 15,500 facili-ties in the United States. Most of those people were already at higher risk due to age and medi-
cal history, and they also shared dining rooms, rec-reation areas, bathrooms and sleeping quarters.
An ongoing tally by The Associated Press has found over 34,000 coro-navirus-related deaths at nursing homes and other long-term care facilities, more than one-third of all COVID-19 deaths in the country.
The GAO report found that about 40% of the nursing homes inspected in each of the past two years were cited for prob-lems with infection con-trol and prevention.
Looking deeper into federal data for 2013-2017, investigators found a recurring pat-tern of problems. Data for that five-year period showed that 82% of nurs-ing homes inspected, or 13,299, had at least one deficiency related to infection control and prevention. About half of the facilities had an infec-tion-related deficiency in multiple consecutive years.
“This is an indicator of persistent problems,” the GAO’s nonpartisan inves-tigators said. The agency carries out oversight for Congress.
The types of problems involved such issues as failing to properly wash hands and not isolating sick residents during out-breaks. “Many of these practices can be critical to preventing the spread of infectious diseases, including COVID-19,” investigators wrote.
The nursing homes cited in the report were not identified.
Seema Verma, head of the federal Medicare and Medicaid agency, said this week that gov-ernors should exercise “extreme caution” before allowing nursing homes to reopen to visitors. Verma has announced that CMS plans to name a commission to exam-ine the nursing home response to coronavirus and make recommenda-tions. The GAO said it plans other reports to independently examine how CMS responded to the outbreak as well as the agency’s oversight of infection control in nurs-ing homes.
The head of the nurs-ing home trade group, the American Health Care Association, has flagged infection con-trol as a priority for the industry. “We are absolutely committed to a renewed emphasis on infection control,” Mark Parkinson said in a recent interview.

SEOUL, South Korea — South Korean students began returning to school on Wednesday, but not without some hitches, in a pos-sible template for other countries struggling to reopen educational facilities.
Hundreds of thousands of high school seniors across South Korea entered their schools after having their temperatures checked and rubbing their hands with sanitizer — familiar measures amid the coronavirus pandemic. Students and teachers are required to wear masks, and some schools have installed plastic partitions at each student’s desk, according to the Education Ministry.
Only high school seniors returned on Wednesday. Younger students are scheduled to return to school in phased steps by June 8.
In a reminder that getting back to normalcy won’t be easy, students in some schools near Seoul were asked to return home in the morning after two students were found to have contracted the coronavirus. Earlier Wednesday, health authorities also reported 32 new cases in South Korea for a total of 11,110, the first time the daily jump has been above 30 in more than a week.
South Korean officials say virus infections are likely to be reported eventually at some schools but express confidence they can sup-press those outbreaks. “We can-not avoid initial confusion and anxiety but I believe we can over-come those as we’ve been calmly and swiftly handling and contain-ing COVID-19,” senior quaran-tine official Kwon Jun-wook told reporters Tuesday.

and widespread public support for government-led social distanc-ing rules. But recently, a sudden spike in new infections linked to nightclubs in Seoul’s Itaewon entertainment district has posed a threat to the quarantine cam-paign.
There are also worries that schools could be new sources of infections. The French govern-ment said Monday that about 70 virus cases had been linked to schools, one week after a third of French schoolchildren went back to school in an easing of the coro-navirus lockdown.
In South Korea, if any school has a new virus case, its students, teachers and other staff will imme-diately be asked to return home and authorities will disinfect school facilities, Education Ministry offi-cials said.
South Korea’s new school year was supposed to start in early March, but was delayed several times due to worries about the spread of the coronavirus. About 5.4 million students in South Korea have been taking classes online.
At Seoul’s Kyungbock High
School, Cho Hee-yeon, the educa-tion superintendent in the capital, used a digital ear thermometer to check students’ temperatures at the main gate as another official placed liquid sanitizer on the hands of the students. Signs reading “No outsid-ers are allowed to enter the school premises” had been set up.
“I am here to see students returning to school while praying that there won’t be any coronavirus patients among our students in 2,200 schools” in Seoul, Cho told reporters.
Students at 66 schools in Incheon, just west of Seoul, had to leave after two students at one of the schools tested positive for the virus Wednesday. The two students didn’t attend classes on Wednesday, but authorities decided to tempo-rarily close all schools in their area, according to the Incheon Metro-politan City Office of Education.
The two students had been in a karaoke room visited by another student who tested positive for the virus after taking private classes from a person who visited an Itae-won club, according to Incheon city officials.
