BASE: SOUTH SECTOR AND ARBD TEAM

Do you want to come and work with the biggest Addiction Service in Europe, whilst continuing your CPD with protected time, and with the opportunity to enhance your teaching and research CV?

NHS Greater Glasgow and Clyde addiction services are responsible for the delivery of addiction treatment and care services for currently over 12,000 clients. The services range from local Community Addiction Teams through to specialist day and inpatient care, together with liaison provision for both general psychiatry and acute hospital services and an Alcohol Related Brain Disorder (ARBD) Team. Our multidisciplinary structure includes professionals from the fields of medicine, nursing, social care, psychology, occupational therapy, dietetics and pharmacy; Glasgow Addiction Services is also a major commissioner of the voluntary and independent sector.

We are seeking an experienced doctor to work in South West Glasgow (6 sessions) and the ARBD Team (4 sessions).

The post is currently based at Leverndale Hospital and relates to 2 community addiction teams (CATs) and the ARBD Team. You would participate in all aspects of patient care, and multidisciplinary meetings providing consultant leadership and expertise. You would contribute to the general psychiatric on call rota of south Glasgow.

The posts combine the satisfaction of participating in an innovative and developing clinical service to a client group with high levels of morbidity for alcohol, drugs and mental health problems, together with the opportunity to become involved in further developing our recovery orientated system of care as well as a number of research projects and teaching initiatives.

Applicants must have full GMC registration, a licence to practise and MRCPsych or FRCPsych or equivalent. Those trained in the UK should have evidence of higher specialist training leading to CCT in General Psychiatry (addictions) or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from date of Interview. You must be eligible for approval under the Scottish Mental Health Act as an AMP.

Job Description

General introduction

Greater Glasgow and Clyde (GG&C) Addiction Services are recruiting an enthusiastic and committed specialist to a post of Consultant Addiction Psychiatrist to work in South West Glasgow (6 sessions) and the Alcohol Related Brain Disorder (ARBD) Team (4 sessions).

The addiction services in GG&C are organised within 6 HSCPs. The Glasgow HSCP is further split into 3 sectors: NorthEast, NorthWest and South. Community services in all areas include integrated community addictions teams/drug and alcohol teams with contracted services providing community rehabilitation and GP shared care scheme. The community services share access to 2 dedicated NHS addiction in-patient units- Kershaw Unit (Gartnavel Royal Hospital) and Eriskay House (Stobhill Hospital). In addition, day services for all areas are provided from these units, with the exception of South Clyde (Inverclyde and Renfrewshire) where more local day services exist. A network of local community mental health teams and an ARBD service are examples of other relevant services organised on a similar geographical basis.

The medical workforce within GG&C addiction services is currently over 50 doctors from diverse backgrounds working as medical officers, senior medical officers, consultant addiction psychiatrists and SAS doctors in addiction psychiatry. In Glasgow, the addiction psychiatrists at present work from secondary service settings reaching into community teams, with geographical areas of responsibility. This post holder with be based in Leverndale with the south east Consultant and will have a community role working with 2 community addiction teams (CATs) in southwest Glasgow, and a secondary service role within the ARBD team at Homeless Health services, Hunter Street, Glasgow.

Current Addiction services for South GlasgowMultidisciplinary and multiagency community addiction services are currently provided from3 CATs (1 large in South East and 2 smaller CATs in South West). These provide a full range of services for people with drug and alcohol problems (currently about 2,900 open cases), with a focus on reducing harms and providing recovery opportunities including alcohol detoxification relapse prevention d drugs of protection; opiate replacement therapies, BBV testing and harm reduction interventions. The Glasgow CATs have recently undergone a review and are in the process of implementing changes to deliver an updated eligibility criteria, access service and an improved recovery orientated system of care.

Office accommodation for this post is currently based in the Leverndale Hospital Department of psychiatry at present. The administrative support is currently based at Old Possilpark Health Centre. It is hoped that the consultants and supports can be co-located within the south sector in due course.

4 part time medical officers work in the 2 CATs in SW Glasgow. Their job is primarily around substitute prescribing for Tier 3 ORT patients and community alcohol services. They are managed by a local senior medical officer. Local GPs also carry shared care substitute prescribing caseloads. The consultant psychiatrist post-holder would be expected to establish close working relationships with these other doctors and agree collaborative approaches to service provision.

The secondary services support include- Use of partial hospitalisation service currently in the Kershaw unit at Gartnavel Royal

Hospital but possibly moving in the future.- Admission to psychiatric beds in the Kershaw addictions unit at Gartnavel Royal Hospital. - In-patient RMO responsibilities are transferred on admission to a Kershaw in-patient

consultant- Psychology sessions - OT input. These provide a functional assessment service and a range of group work

programmes.

Community and residential rehabilitation placement is available through a variety of contracted providers. The consultant would be expected to take a role in the decisions about placement of patients in residential rehabilitation facilities in coordination with the local head of service

The Alcohol Related Brain Damage ServiceThe ARBD service currently is based in Homeless Health Services, Hunter Street, Glasgow. The ARBD team was established in 2006 to provide enhanced services for alcohol damaged individuals from the old greater Glasgow health board geography. It consists of dedicated medical, nursing, social care, psychology, occupational therapy and dietetics input in line with the “Fuller Life “ report (2004).

The ARBD team work in conjunction with service providers (SAMH, Penumbra, Loretto) who have been commissioned to provide a range of residential placements ranging from very highly supported accommodation to supported living in tenancies. Whilst there are no dedicated addictions beds there is a developing and dedicated rehabilitation pathway.

Equality of access to service for all is an aspiration of the Board. ARBD services outwith the old GGC boundary are configured differently and agreement re appropriate pathways and access to assessment and treatment is being addressed via the NHS GG&C Clinical Services Review.

The Duties of the Post

South west addiction psychiatry- 5.5 Pas DCC- Addiction psychiatry support to Southwest and Greater Pollok CAT teams- Psychiatric

outpatient assessment and treatment for referred patients with complex substance misuse problems often in the context of co-morbid physical and mental illness. Referrals will generally come from community addiction teams and other consultants. This aspect of clinical work will have a particular focus on psychiatric co-morbidity and complexity of addiction issues especially where there is a need for in-patient and day service provision.

- CAT based clinical work (possibly ORT with a small community caseload) and MDT- Direction of the work done by speciality doctors relating to this post- Cross cover the SE consultant with regards to addiction psychiatry for south Glasgow- Would work with SE Consultant and South Senior Medical Officer to provide between

them senior medical input to south community clinical governance systems and service developments

- Emergencies- The post holder may be required to perform duties in occasional emergencies and unforeseen circumstances and at times at the request of the Associate Medical Director (Addictions). It has been agreed that such duties will be exceptional and that this kind of arrangement would not be for prolonged periods of time or on a regular basis in keeping with contractual arrangements.

- Psychiatric emergencies are generally dealt with by crisis and community mental health teams but the post-holder would be expected to be flexible in this regard.

- Mental health Legislation use- Approved medical practitioner use of the Mental Health Act, Adults with Incapacity Act and Adult Support and protection Act would be part of the job. The post holder would therefore require to be an AMP approved by the Board.

- On call- Contribution to the south Glasgow general psychiatric rota with a frequency around 1 in 25 which attracts a level 1 3% availability supplement at present.

- Clinical cover of colleagues- The 10 pa job plan would include prospective cover for a colleague’s leave. Any leave requested would require cover to be arranged in advance from another consultant. Cover for short term sudden absences e.g. sickness in a colleague consultant would also be included in the job plan

ARBD- 3.5 PA DCC

The consultant addiction psychiatrist would bring medical expertise to the existing team.- As with other members of the ARBD team the consultant would generally be expected to

provide an expert consultative and advisory service to patients and services caring for these patients

- A part of the work is in relation to application of mental health legislation in particular guardianship and capacity assessments. The consultant would sometimes act as RMO in this regard for appropriate patients. However patients on the caseload of other services such as general adult psychiatry would continue to have their RMO usually from that service. Negotiation of this on a case by case basis would be important. The RMO function is currently under review.

SPA-- There would be 1 session of supporting professional activity to include time for appraisal,

job planning, continuing professional development.

On-call-- The post-holder would be expected to participate in the South General Adult Psychiatry

Consultant on call rota.

Job plan

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director.  NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings.  The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

The post is for a standard 10 programmed activities (pas) made up of 9 pas of direct clinical care and 1 pas of supporting professional activity. The re-designation of pas from direct clinical care to supporting professional activity may be possible subject to negotiation and job planning.

The post-holder will be provided with an office in the Leverndale Hospital with access to a computer and printer. Dedicated administrative support will be provided but is currently housed at Old Possil Park Health Centre as new premises are sought in south Glasgow.

An indicative job plan is attached.

TeachingMedical students may be attached to the Consultant Psychiatrist with Medical Students receiving their psychiatry block training at various hospital sites.

Clinical Governance, Research and AuditThe post-holder will be encouraged to undertake research work. The service is particularly keen on clinical audit that establishes the effectiveness of interventions. The post holder will be expected to play a full part in this, and develop ideas for audit projects on an inter-professional basis with colleagues across NHS GG&C Addiction Services. In addition to audit and research the post holder would be encouraged to contribute more widely to clinical governance for the Addiction services. Examples of this activity might include leading the investigation of allocated critical incident reviews. This work could be done in enhanced spa time in substitution for dcc programmed activities.

Management Committee Work and Professional Representation.The post-holder will be a member of the South Glasgow Division of Psychiatry and the Division of Addiction Psychiatry. Under current management structures, he/she will be a part of the medical staff of the Greater Glasgow and Clyde addiction services and be expected to contribute as appropriate to the advisory structure within this. Expert advisory and committee work within and out with the organisation would be included in SPA time.

Information technology useThe applicant would be expected to be able to use IT in their daily work – communicating electronically and contributing to the information gathering and performance management systems of the organisation. The post holder would through annual appraisal and annual job planning have a performance management. The post holder would be expected to positively contribute to the regular IT reporting of caseload, activity, diagnostic and outcome indicators.

Section 22 Training

Completion of day 1 and day 2 Section 22 training is required prior to taking up post. The Medical Staffing Department will require sight of your certificates to this effect.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

£ 76,761 to £ 103,490 per annum (pro rata)

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

HOURS OF DUTY Full Time 40.00

SUPERANNUATION New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme.  Should you choose to "opt out" arrangements can be made to do this via: www.sppa.gov.uk

REMOVAL EXPENSES Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.

EXPENSES OF CANDIDATES FOR APPOINTMENT
