We expect it to be submitted complete and on time, but more importantly, if your student does not get a concept and cannot complete a problem or assignment, it is very important to communicate with the subject area teacher, send an email, write in the assignment notebook, ask for a phone call...We can only help fix what we know about. Recently we have made changes in lightening the volume of homework and giving students more options. We can best improve when we learn what you're witnessing at home and put that together with what we see here at school. We value the connec- tion with home and are responsive when we learn there are problems. If we don't know, we can't fix it! Thanks for your help. PTO Please join us for the next PTO meeting on Mon., May 18 at 7:00 PM in the Faculty Lunch Room. We are making plans now for the coming year and looking for more ideas and involvement. We know how busy families are, but PTO is one of the best ways to enjoy student-centered events and stay connected with what your children are doing. Principal for the Day! Child Care is open from 7:15-3:00 PM five days per week following the school calendar. Aftercare kicks in from 3:00-6:00 PM daily. In the Child Care program, children will enjoy enrichment activities, special area classes, cooking, science experiments, in-school presentations and field trips plus much more. We will have a rest time available for little ones who still nap and quiet activities such as books and puzzles for those who do not need this rest time. Please call or email with your questions or to schedule a tour. We will be rejuvenating the Aftercare rooms as we plan for this all day program. Exciting plans are in the works at Our Lady! In Prayer As a school family, we are always looking out for someone! Please join me in lifting up our eighth graders in prayer as they complete their final weeks as Crusaders and look forward to the transition to high school. They have enjoyed many special activities and Christ-centered events this year. They have even more highlights and fun times to come in these culminating days at Our Lady. It is a bittersweet time, and we wish them much joy and grace as they leave their legacy here and move on to even greater challenges and happy times. Outstanding Debt Obligation With the end of the 2014-15 school year quickly approaching, it is essential that all out-standing debts (i.e. tuition, afterschool care, cafe, etc.) be paid in full before the last day of school. Report cards will not be provided to families with outstanding debt. If you have questions or need a balance due, please call Connie Andres at the school office.

Pre-registration for Next School Year Please send in your pre-registration form and check to ensure your child’s enrollment for next year. Pre-registration fee is $75 per child, and the pre-registration form is attached.

Parish Stewardship Intention Cards Due To qualify for parishioner tuition rates next school year, please send your Parish Stewardship Intention form back to the Church Office as soon as possible. Thank you!

Applications for School Choice Vouchers and

SGO Scholarships for the 2015-16 School Year

Our school is now accepting applications for School Choice Vouchers and SGO Scholarships for next year. To apply for state vouchers, email Mrs. Horton to set up an appointment. You will need to bring income verification (2014 tax return) and child support amounts, if applicable, to the appointment. There are appointment times available beginning on May 11, on Mondays, Wednesdays and Fridays. A Parent Information Session will also be held Thursday, May 14 at St. Mary’s of the Knobs School at 6:30 pm. To apply for an SGO Scholarship, please email Mrs. Horton or contact the school office for an application. Guidelines for these programs and the meeting are attached to the end of this newsletter.

TIME TO ORDER YOUR CHILD’S SCHOOL SUPPLIES FOR NEXT YEAR!

Dread the thought of last-minute back-to-school shopping in July? Wish there was a convenient, easy, and less expensive option? EduKit is the solution! Order forms have been sent out if you prefer to pay by check OR order online now for children ENTERING grades K-4 in the fall at www.edukitinc.com. If you need a paper order form, get one from the school office. Complete the form and send it to school

with your check made payable to OLPH PTO. All orders are due before May 29 to get the best pricing. The last date to order online is July 8.

Kits will be available for pickup at the July 23rd registration. It's that easy!!

Beginning Guitar Lessons

For students entering 3rd- 8th grade

Provided By Tom Yost, Parish Staff Member

7 Week Summer Session beginning the week of June 1 - 5 and concluding the week of July 20 – 24.

There will be no lessons the week of June 29 – July 3.

Lessons will be once a week for 30 minutes. Lessons will be given on Tuesday & Thursday mornings between 10:00 am – Noon. The one exception will be Tuesday, June 9. This will be switched to June 10 if possible. Lessons will be given in the parish office building. Registrations and preferred time slots will be taken on a “first come” basis. The cost is $15 per half hour lesson. Payment is expected at the time of the lesson.

Tuning a guitar

Introduction to Keys and Chords

Strumming and Picking Patterns

5K FRIAR RUN at Mt. St. Francis

New Albany High School WNAS Radio 88.1 FM

2015 Summer Camp

Hey Kids! Want to take over the airwaves? WNAS Radio will be hosting a camp for students

in grades 3-8 where students will learn how to broadcast over the air live, tape segments,

record promos, engineering and more. The camp ends on Friday, June 19 with a taping of a

two-hour broadcast of the radio camp kids displaying all of their acquired talents as radio

broadcasters. This show will feature interviews, campers’ music selections, advertisements,

news, weather and much, much more! All of this can be heard over the airwaves of 88.1 FM

on TV channel 25 on Time Warner Cable and around the world at wnas.org! The camp runs

from June 15-19 and will be held from 9:00 am—11:30 am or 12:30 pm-3:00 pm. The cost is

$75.00, and checks should be made payable to NAFC Schools. If you have questions, contact

form in the Our Lady School office.

School Choice Scholarships (vouchers) and SGO Scholarships

Our Lady participates in the Indiana School Choice Scholarship (voucher) Program which helps fam-ilies pay for tuition and fees using state tax dollars.

To be eligible, a student must satisfy each of the following initial requirements: 1. Have legal settlement in Indiana.

2. Be between the ages of 5 and 22 no later than August 1 of the school year.

3. Be accepted for enrollment into a participating Choice Scholarship school. 4. Meet income guidelines established by the state. After satisfying the initial requirements listed above, there are seven different pathways that a student can satisfy to become eligible to participate in the Choice Scholarship Program. A student must satisfy only one of the seven pathways to be eligible.

A. Continuing Choice Scholarship Student Pathway

The student received a Choice Scholarship in the school year that immediately precedes the school year for which the student is applying for a Choice Scholarship.

B. Previous Choice Scholarship Student Pathway

The student received a Choice Scholarship in a school year that does not immediately precede the school year for which the student is applying for a Choice Scholarship, or the student received a Choice Scholarship in the immediately preceding school year, but the student exited the Choice School prior to the end of the school year.

C. Previous SGO Award Pathway

The student received an SGO Scholarship in a preceding school year, including a school year that does not immediately precede the school year for which the student is applying for a Choice Scholarship.

D. Special Education Pathway

The student has a disability that requires special education and an individualized education program (IEP) .

E. “F” Public School Pathway

The student would be required to attend a specific public school based on their residence that has been assigned an “F” grade.

F. Two-Semesters in Public School Pathway

The student was enrolled in kindergarten through grade 12 in a public school, including a charter school, in Indiana for at least two semesters immediately preceding the first semester for which the indi-vidual receives a Choice Scholarship.

G. Sibling Pathway

The sibling of the newly applying student received either a Choice Scholarship or an SGO scholarship in a preceding school year, including a school year that does not immediately precede the school year for which the student is applying for a Choice Scholarship.

Our Lady provides private funding for the SGO Scholarship Program. The school granting SGO

Scholarships determines the eligibility for the SGO. At Our Lady, families who fall below the 175%

Free/Reduced lunch income level but do not qualify for a voucher may qualify for a one-time $500

scholarship for each child in kindergarten-grade 8. After one year, families can qualify for vouchers if

the annual household income is at or below the 150% Free/Reduced lunch rate. SGO income guide-

lines are:
