cancelled due to rain or extreme heat, adifferent activity will be arranged. We will

try to provide 24 hours' notice if thisoccurs. 

NEW ENROLMENTS Completed enrolment forms for 2020 must

be returned to the OSHC prior to booking. Children with medical needs must also supply the OSHC with medication and

appropriate documentation with their enrolment before booking.

HOW DO I PAY? Payments can be made by cash, EFTPOS, Direct

Deposit or Debit Success. Statements will be emailedout weekly following the children’s attendance. This

will show all adjustments to CCS & bookings. Anyoutstanding accounts must be paid before childrenattend. Care will be suspended if fees are unpaid.

Child Care Subsidy can be paid on behalf of eligible parents by contacting the Family Assistance Office on

13 61 50 & acquiring a CRN, then making a claim. 

*Please ensure you pack healthy food for your children, lollies & chocolate are discouraged.

Please initial each condition to indicate consent and that you have read & understood the terms & conditions. I give permission for my child/ren to participate in any of the activities at Holiday Club or on an excursion. I have made the OSHC educators aware of any changes to my contact details or my children’s medical history. If my child requires medication, I have provided medical action plans for the dosage of the medication. All medications are named, in the original packaging and labelled with instructions. I give permission for my child/ren’s photograph to be taken and allow his/her image & name to be published in circumstances the Director deems to be appropriate i.e. The school newsletter, website, and other promotional material. I am aware that the activities will follow the program as outlined; however the staff reserves the right to alter the activities as necessary. The staff will endeavour to choose an activity that is suitable for all children.

I am aware that cancellation must occur the Friday before care is required in order not to be charged. A half fee will apply if cancellation occurs at least 24 hours prior, or if my child is sick and a medical certificate is supplied. The full fee will still apply if I cancel a booking within 24 hours of care. I am aware that full payment of outstanding OSHC fees need to be paid a week before Holiday Club starts, otherwise my child/ren will not be booked in. I give permission for my child to watch PG movies during Holiday Club under the supervision of staff.
