Adams County Craig A. Showvaker, certificate no. CA019228L, of Lewisburg, Union County, and formerly of Adams

County, was assessed a civil penalty in the amount of $1,000, and had his certificate of certified public

accountant and current license to practice public accounting revoked for his felony conviction of mail

fraud in federal court. (11/28/11)

Allegheny County Delbert D. Tyler, license no. CA-027923-L, of Monroeville, Allegheny County, was assessed a civil

penalty in the amount of $3,000 because he practiced public accounting while his license was lapsed and

he submitted a false verification of candidate experience.

Louis Joseph Estermyer, certificate no. CA023420R, and Estermyer & Associates, P.C., licensed

accounting firm no. AF049620, of Oakdale, Allegheny County, was assessed a civil penalty in the amount

of $500 for practicing with lapsed licenses. (01/09/12)

Milton Eisner, license no. PA000476L of Pittsburgh, Allegheny County, was ordered to pay a $2,500 civil

penalty because he engaged in dishonesty, fraud, or gross negligence in the practice of public accounting

and engaged in conduct discreditable to the profession of public accounting. (04/17/12)

Berks County Rebecca Diane Erb, certificate and license no. CA018042L of Boyertown, Berks County, was ordered to

pay a civil penalty in the amount of $3,000 and an assessment for the costs of investigation in the amount

of $184.25 because she practiced as a certified public accountant when she was not currently licensed and

registered between January 1, 2008 and January 7, 2010 and failed to submit the required peer review

Bucks County George Richard Mark, certificate and license no. CA022498L of Doylestown, Bucks County, had a public

reprimand placed on his permanent disciplinary record with the Board and agreed to the permanent

voluntary surrender of his certificate and license to practice as a certified public accountant in the

Commonwealth of Pennsylvania because he has pleaded guilty to, entered a plea of nolo contendre to, or

has been found guilty of a felony under any federal or state law or the laws of any foreign jurisdiction and

Dauphin County David B. Blain, certificate no. CA036785L, of Harrisburg, Dauphin County, was assessed a civil penalty

of $11,000 and suspended for four years with the first two years served active and the remaining two years

served on probation because he engaged in unprofessional conduct by failing to comply with a standard

promulgated by the Securities and Exchange Commission and because his privilege to practice before the

SEC was suspended.

Delaware County Segal & Brint, LLC, license no. AF048848 of Ardmore, Delaware County, was ordered to pay a $2,500

civil penalty because it practiced as an accountancy firm when it was not currently licensed and

placed on his permanent disciplinary record with the board and was ordered to pay a civil penalty in the

architecture because he practiced and offered to practice architecture without a license. (05/10/12)

Donald A. Dale, Inc. of Lancaster, Lancaster County, was ordered to pay a civil penalty in the amount of

$4,000, to cease and desist from all activities or acts that fall within the definition of the practice of

architecture, and to disgorge all fees collected from the affected client because it, as an unregistered firm,

offered to design multi-family dwellings each such designed housing units having a capacity in excess of

two family units, practiced architecture by designing four separate sets of multi-family dwellings featured

on their website each having a capacity in excess of two family units, offered to practice architecture

through a proposal letter offering to provide six distinct architectural instruments of service relating to

four separate sets of four unit townhouses, each set being a multi-family dwelling having a capacity in

excess of two family units, and practiced architecture when they delivered five separate instruments of

service, architectural design documents for the construction of four unit townhouses to a client. (05/10/12)

Cumberland County The Mechanicsburg Club, of Mechanicsburg, Cumberland County, was ordered to cease and desist from

soliciting contributions in the Commonwealth until such time as it duly registers with the Bureau of

Charitable Organizations based on findings that the Mechanicsburg Club is a charitable organization and

has solicited contributions in the Commonwealth while it was not registered. (02/27/12)

PA Board of Chiropractic

Bucks County Christopher Vaill Walcott, of Yardley, Bucks County, license no. DC007908L, and adjunctive procedure

certification no. AJ007908L, was temporarily suspended, pending a hearing, on the grounds that his

continued practice of chiropractic within the Commonwealth may be an immediate and clear danger to the

Chester County Stephen Anthony Conicello, license no. DC009298 of Downingtown, Chester County, was ordered to pay

a civil penalty in the amount of $500 and his license suspended for three years retroactive to January 10,

2010, stayed immediately in favor of probation to run concurrently to his probation with the State of

South Carolina, because his license was disciplined by the proper licensing authority of another state.

the salon license in a conspicuous place within the business premises of the salon; did not have a “no

smoking” sign prominently posted; failed to have a closed container for soiled linen; failed to have a

closed towel cabinet for clean linen; failed to have a closed container for sanitized implements; failed to

maintain the sanitary use of towels; and failed to maintain the sanitization of equipment. (05/07/12)

Berks County Creative Expressions Hair Studio, license no. CB120374 of Birdsboro, Berks County, was ordered to pay

a civil penalty in the amount of $600 and costs of investigation in the amount of $28.52 because it failed

to display a "No Smoking" sign, failed to have a complete first aid kit, and possessed a metal rasp in the

The Best Little Nail House, license no. CY106773, of Shillington, Berks County, was ordered to pay a

civil penalty in the amount of $1,150 and costs of investigation in the amount of $18.94 because it did not

keep unused cloth towels in a closed cabinet, had an uncovered waste container and practiced the

profession in a grossly incompetent or unethical manner through the use of a credo tool and multiple razor

Innovative Hair Designs, license no. CB122332, of Temple, Berks County, was ordered to pay a civil

penalty in the amount of $500 and costs of investigation in the amount of $28.52 because it practiced

cosmetology in a grossly incompetent and/or unethical manner through the possession of a metal rasp.

neither the owners nor a designated person was readily available in person to Bureau inspectors during

a civil penalty in the amount of $3,100 and costs of investigation in the amount of $71.42 because it had

neither the owner nor a designated person in charge of the salon readily available in person to Bureau

inspectors during business hours, dirty tools were found in cosmetology salon when tools that come in

contact with clients are to be kept in a sanitary condition at all times, it permitted cosmetology to be

practiced in a grossly incompetent and/or unethical manner by permitting an employee without an

esthetician license to perform esthetician services in the salon, it aided and abetted the unlicensed practice

of a profession, business or occupation by permitting an unlicensed individual to practice esthetics in the

salon, it failed to keep clean paper towels in a closed cabinet or closed towel dispenser, and dirty tools

were found in esthetician salon when tools that come in contact with clients are to be kept in a sanitary

condition at all times. (05/07/12)

Blessed Image Cuts, license no. CB117630, of Downingtown, Chester County, was ordered to pay a civil

penalty in the amount of $1,450 and costs of investigation in the amount of $53.57 because it acted with

gross incompetency by employing an unlicensed individual to perform cosmetology services, aided and

abetted the unlicensed practice of a profession, business or occupation, failed to have the salon license in

its possession in the salon, failed to have a covered waste container, failed to maintain tools, instruments,

utensils and appliances that come into contact with a client in a sanitary condition at all times, failed to

have a complete first aid kit in the salon and failed to have a reception desk in the salon. (05/07/12)

Advanced Nail Care, license no. CY098297L, of Phoenixville, Chester County, was ordered to pay a civil

penalty in the amount of $2,400 because it practiced the profession in a grossly incompetent or unethical

manner through the use of metal rasp tools, and failed to maintain tools and instruments that come into

contact with a client in a sanitary condition at all times. (05/07/12)

Delaware County Inna Melamed t/d/b/a Professional House of Hair, license no. CB089807L, of Upper Darby, Delaware

County, was ordered to pay a civil penalty of $200 because the salon failed to have a complete first aid

kit, failed to have a covered waste container, and failed to keep dirty towels in a closed container.

pay a civil penalty in the amount of $2,000 and costs of investigation in the amount of $71.42 because it

acted with gross incompetency by allowing an unlicensed individual to perform nail technology services

on a customer, aided and abetted the unlicensed practice of a profession, business or occupation, failed to

keep unused disposable paper towels in a closed cabinet or closed towel dispenser, and as both a nail

technology salon and as an esthetician salon, failed to maintain tools, instruments, utensils and appliances

that come into contact with a client in a sanitary condition at all times, and failed to place used cloth
