STEPS TAKEN TO ELIMINATE DANGER TO PERSONNEL INVOLVED AND OTHERS, INCLUDING BARRIERS WHERE APPROPRIATE:

IF, AT ANY POINT, THERE IS A CHANGE TO THE WORKSCOPE INVOLVING THE LIFTING OPERATION THE JOB MUST STOP AND BE RE-ASSESSED WITH A NEW OR REVISED LIFTING PLAN IMPLEMENTED.

The following must be adhered to:

Lifting plan and the risk assessment is to be discussed at the Toolbox Talk.

Area to be barriered off prior to start of lifting operation commencing.

Pre–use and after use inspection of the lifting equipment and accessories must be carried out by the user.

In-use function checks to be carried out at the start of each shift for pre-installed equipment.

The manufacturers’ ‘safe use of’ instructions for lifting equipment and accessories

The use of certified and colour coded portable lifting equipment & accessories.

Scaffold/ladder access to be provided for the installation and removal of the lifting equipment.

Safety harness and reel to be worn when working at height.

Only competent personnel to operate the rigging equipment.

If for any reason eg (GPA) there is a requirement to temporary suspend loads, then it must be ensured that the load is made safe by attaching a wire sling (dead leg) to the load.

Should it become apparent that the lifting operation has changed for any reason from the original plan, the lifting operation must STOP. The lifting operation must be re-assessed and the lifting plan reviewed and amended to facilitate the changes. The risk assessment must also be reviewed and any additional risks identified are to be incorporated into the original risk assessment.

Remember no job is that important that you cannot take the time to do it safely.

Safety is our priority not SPEED if in doubt STOP and take time out for safety.

10 Riggers to pre rig lift point 2 using a 3t usc3a beam clamp attached to deck head steel work as per drawing then attach a 1t swl 6mtr fall chain block using a 4.75t screw pin bow shackle

Any amendment must be endorsed by the Platform LOLER Focal Point prior to the task re-commencing.
