Paid Family Leave (PFL) Bonding Benefits Important: You may be eligible to receive up to six weeks of Paid Family Leave (PFL) benefits to bond with your newborn child after your pregnancy- related Disability Insurance claim ends. When planning your time off with your employer, you might want to consider your option to take bonding benefits. Bonding benefits are potentially payable within one year of your child’s birth, adoption, or foster care placement. A form, Claim for Paid Family Leave (PFL) Benefits - New Mother, DE 2501FP, will automatically be sent to you when your pregnancy- related Disability Insurance claim ends. This claim form will be sent in a separate envelope at the time your last Disability Insurance payment is issued. You may also submit this form using SDI Online. Do not submit the DE 2501FP until after you have recovered from your pregnancy-related disability. Note: The other parent who takes time off work to bond with a new child may also be eligible to receive up to six weeks of PFL benefits. Benefits are potentially payable within one year of the birth, adoption, or foster care placement. Paid Family Leave (PFL) Bonding Benefits

Important: You may be eligible to receive up to six weeks of Paid Family Leave (PFL) benefits to bond with your newborn child after your pregnancy-related Disability Insurance claim ends.

When planning your time off with your employer, you might want to consider your option to take bonding benefits. Bonding benefits are potentially payable within one year of your child’s birth, adoption, or foster care placement.

A form, Claim for Paid Family Leave (PFL) Benefits - New Mother, DE 2501FP, will automatically be sent to you when your pregnancy-related Disability Insurance claim ends. This claim form will be sent in a separate envelope at the time your last Disability Insurance payment is issued. You may also submit this form using SDI Online. Do not submit the DE 2501FP until after you have recovered from your pregnancy-related disability.

Pagos de Beneficios del Permiso Familiar Pagado (PFL) - Mientras Toma Tiempo Libre de Su Empleo Para Establecer Lazos Afectivos Con Un Nuevo Bebé o Niño(a) Adoptado

Si usted es una nueva madre, es posible que sea elegible para recibir hasta seis semanas de pagos de beneficios del Permiso Familiar Pagado (PFL, por sus iniciales en inglés) si toma tiempo libre de su empleo para establecer lazos afectivos con su nuevo bebé después de haber dado a luz.

El formulario en papel de la Solicitud de Beneficios del Permiso Familiar Pagado (PFL) – Nueva Madre (DE 2501FP) se le enviará automáticamente por correo postal en un sobre separado a las nuevas madres después de que se les haga el último pago de beneficios del Seguro de Discapacidad (DI, por sus iniciales en inglés) para que lo completen y lo presenten al Departamento (EDD). También se puede completar y presentar está solicitud electrónicamente a través de SDI Online. Atención: La Solicitud de Beneficios del Permiso Familiar Pagado (PFL) – Nueva Madre (DE 2501FP) no se puede presentar al Departamento (EDD) sino hasta que la nueva madre se haya recuperado de cualquier discapacidad relacionada con su embrazo y/o parto.

Si usted es el otro padre o madre (el cual no dio a luz al nuevo bebé) que también desee tomar tiempo libre de su empleo para establecer lazos afectivos con su nuevo bebe, o un niño(a) adoptado permanentemente, o un niño(a) colocado bajo el cuidado de crianza temporal (foster care), puede que también sea elegible para recibir hasta seis semanas de pagos de beneficios del Permiso Familiar Pagado (PFL). Para solicitar estos pagos de beneficios, usted debe completar el formulario en papel de la Solicitud de Beneficios del Permiso Familiar Pagado (PFL) (DE 2501F) y enviarlo al Departamento (EDD) por correo postal o completarlo electrónicamente a través de SDI Online.

Cualquier persona que desee solicitar pagos de beneficios del Permiso Familiar Pagado (PFL), tiene hasta un año después de la fecha del nacimiento de su nuevo bebé, o después de haber adoptado permanentemente a un niño(a), o haber tomado la responsabilidad de un niño(a) colocado bajo el cuidado de crianza temporal (foster care), para hacerlo.
