• Must pass 5 credits the previous QUARTER and must be enrolled in 5 credit courses the current quarter. Summer school does not count.
•Cannot turn 19 before August 1st

•Sophomores must have a 1.25 g.p.a. in the previous quarter

All of the above signed forms MUST be on file in the A.D.’s office BEFORE the athlete is allowed to practice.
PAY-TO-PARTICIPATE FEEPAY-TO-PARTICIPATE FEE
1. Check made payable to Parma City Schools for the amount determined for your sport.
2. Football, Volleyball, Basketball, and Wrestling $300, all other sports $150.
3. Fee must be paid no later than the first contest in the sport. Once the deadline has passed, the athlete is not allowed to participate in any scrimmage or game until the fee is paid.
4. Fee is non-refundable after the date of the first scrimmage or game.
5. Paying the fee does not guarantee any playing time. Playing time is determined at the discretion of the coach.

1. All O.H.S.A.A., P.C.S.D., and team rules apply as stated in the Student-Parent Handbook, Student-Parent Handbook, OHSAA Bylaws, and team rules for your sport.OHSAA Bylaws, and team rules for your sport.
2. All O.H.S.A.A., P.C.S.D., and team rules are in effect 24 hours a day, 7 days a week during athletic season.
3. Good citizenship and sportsmanship expected at all times.
4. Do not engage in criminal activity in or outside of school.
5. Drug/Alcohol/Tobacco possession or use will result in an immediate suspension from a minimum of one contest to the possibility of expulsion from the team.

7. Student-Athlete and Parents are financially responsible to return all district owned equipment. If
damaged, lost, or stolen, a replacement fee will be issued.

Important Rules to RememberImportant Rules to Remember::
•Athletes are expected to attend all practices, meetings, and games. The coach should be notified of the absence by a parent prior to the absence. The coach has the authority to determine whether or not the reason for absence is considered excused.
• To be eligible to participate in practice or games, an athlete must be in school at least 4 full class periods.
• If suspended, the student is ineligible to participate that day.
• Weekend participation is determined by attendance on Friday.
•Athletes are required to travel on the team bus unless prior permission is granted by the Athletic Director.

What if my child is injured?What if my child is injured?
•If an athlete is injured during a practice or game, they should report it to their coach immediately.
•The coach will determine whether the athlete should be evaluated by our Athletic Trainer.
•The trainer will evaluate the injury and will recommend treatment or further evaluation by a doctor. The trainer will send home written notification if she feels that a doctor should examine your child. She will also call home for serious injuries.
•If the trainer feels that a doctor should examine your child, a signed, written release by a doctor is required to allow the athlete to resume activity.
•If an athlete sees a doctor for any injury…they must be medically released by a doctor. A signed release by a doctor must be given to the trainer before returning to play.

Health Concerns
•Maintain a healthy, balanced diet and make sure to stay hydrated to avoid heat exhaustion and heat stroke.
•Examine your body regularly. Report any rashes or skin changes to trainer immediately. (MSRA, impetigo, ringworm etc.)

PSH FITNESS CENTERPSH FITNESS CENTER•Open for use by all PSH students after school.
•Facility used by our sports teams and our Physical Education classes.
•All students using facility must do so with adult staff member supervision. No student should enter the facility without an adult present to supervise them.
•All facility users must have a signed waiver form on file in AD office and are responsible to follow all fitness center rules.
•If the athlete does not know how to perform a lift, they should ask the supervisor for instruction.
•Possession and/or use of steroids and supplements such as protein and creatine is not allowed.
