You may complete the form on line using either the online Passport Application Wizard or the electronic version of the DS-11 (PDF, 1004kb). The completed form should be downloaded and printed on separate sheets of white A4 or letter-size paper.

If you would rather complete the form by hand, please download and complete with a black ink pen, using block letters.

Item 5 - Social Security Number: You must complete this item. Failure to provide a Social Security Number, if you have been issued one, can result in a $500 fine enforced by the IRS.

If you have never had a Social Security Number (SSN), write zeros in the boxes.

If you have a number, but do not remember it, call the Embassy's Federal Benefits Unit for assistance. You may call the Federal Benefits Unit any workday, between 8:30 a.m. and 1:00 p.m. at:  020-7894-0926 or 020-7894-0927.

Proof of ParentageParents must submit their valid U.S. or foreign passport; and one of the following:

U.S. state birth certificate (either vital statistics or Department of Health) with parents‘ names;

Certified foreign birth certificate (with parents‘ names and translation, if necessary);

Consular Report of Birth Abroad (Form-240) (with parents names);

Certification of Birth Abroad (Form DS-1350) (with parents names);

Adoption Decree (with adopting parents names) as well as amended State birth certificate listing adoptive parents names;

Court Order establishing custody; or Court Order establishing guardianship.

Note:  If your name is not the same as your child's last name you must submit documentation that will link your names or link you to the child (marriage/divorce certificate, court order).

Parental Consent Form DS-3053All children under the age of 16 are required to appear at the Embassy or Consulate with both parents for all passport services. However, if only one parent is able to attend , they are required to bring:

Form DS-3053 (Statement of Consent) completed by the absent parent and notarized before a U.S. or U.K. Notary Public, or U.K. Solicitor. A U.S. consular officer may also notarize the Form DS-3053 . Contact us for further information . (The DS-3053 must be presented within 3 months of the date on which it was notarized): and

ID for the absent parent in the form of one of the following: • Valid U.S. or foreign passport or a good notarized

photocopy of the identity and signature pages'; • Valid driver's license; • Valid military ID; • Valid U.S. government ID; • Certification of Naturalization or Citizenship from the

USCIS with photograph; or • Alien Resident Card from USCIS (green card).

Parents with sole custody of their child will be required to furnish evidence of their sole authority to apply for the passport in the form of one of the following:

Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing you as the only parent;

Child's birth certificate listing you as the only parent; Adoption decree (if you are the sole adopting parent); Court Order granting sole custody to you (i.e. the absent

parent has no access to the child.  Child's travel must not be restricted by that order;

Judicial declaration of incompetence of other parent; or Death certificate of other parent.

Vending machine photographs and photographs printed on a home computer are unacceptable.

Your head should be between 1 inch and 1 3/8 inches (22mm and 35mm) or between 50% and 69% of the image’s total height

Applicants must provide one passport photograph. Photographs that do not meet the specification outlined will be rejected and cause delays in processing.

You can lay the child on a white /off-white sheet , or place the baby in a car seat covered with a white/off-white sheet

SCHEDULE THE APPOINTMENT

Appointments can only be scheduled on the Embassy’s website!

Children under the age of 16 must appear in person accompanied by at least one parent through a pre-arranged appointment.   The appointment is scheduled for the child. Accompanying parent(s) should list their names in the box provided.

Each child applying to renew their passport requires an individual appointment. Don’t worry if you can’t book the same time slot. You will be seen all together on the day of your appointment.

On scheduling the appointment, print the confirmation sheet and bring it with you to the appointment.

Do not send the completed application form to the United States! You must bring it to the appointment.

FEES

The fee is $105.00 USD.  Paid on the day of the appointment.The fee is a non-refundable processing fee and is retained by the Department of State whether or not the passport is issued.

We accept:

• Credit card - Visa, MasterCard, American Express, Discover, or Diners Club; • Debit card - Visa or MasterCard only; • Banker’s Draft drawn in U.S. dollars only on a bank in the United States made payable to the “United States Disbursing Officer” issued with the last 5 months; • International Money Order to the exact U.S. dollar amount made payable to the “United States Disbursing Officer” issued within the last 5 months; or• Cash in U.S. dollars or pounds Sterling ( London or Belfast only).

We are not authorized to accept personal checks.

The passport(s) will be returned to you by the Embassy’s contracted courier service, DX Secure, or Royal Mail. You may choose the method of delivery. If using Royal Mail, please furnish a self-addressed, prepared Royal Mail Special Delivery Envelope with the application that shows postage paid up to 500 grams. The services of DX Secure can be purchased in the lobby of American Citizen Services on the day of your appointment.

Processing takes, on average, 15 working days (3 weeks) from the day the Embassy receives the application.
