seded) A document issued by the Department ofState which permits the bearer taloave and re-enterthe United States. Basically there are three typesof passports issued: Regular, Official, and Diplo-matic. The individual assigumen\ determines thetype of passport to be issued.

(1) Diplomatic passport (no-fefy. This pass-port is issued to .persons proceedingabroad on diplomatic assignments andto their dependents, i.e., attache's, assist-ant attaches, MAAG chiefs, deputyMAAG chiefs or MAAG chiefs of staffwhen such positions are authorized inlieu of deputy MAAG chiefs, Und chiefsof services represented.

(2) Official passport (no-fee). This Wsspovtis issued to military personnel ana certaincivilian personnel ordered overseas onofficial business and to their dependents.

(3) Regular passport (no-Jee). This passportis issued to dependents when sponsors aretraveling to an assignment in a countrywhere no passport is required for\ thesponsor. See note below.

Note. No-fee passports discussed in (l),U2),and (3) above are issued to dependents whoWetraveling abroad with, or to join, their sponsorsand whose travel is authorized by militaryauthorities.

1. Purpose. This regulation tells how to obtainpassports and visas for official travel and leavetravel and other unofficial travel to foreign countries;outlines the responsibilities of commanders inconnection with the timely application for passportsand visas, and provides for their control and dispo-sition. (The Department of State controls theissuance of passports. Information in this regu-lation about the Department of State requirementsis furnished for informational purposes only.)

- 2.\ExpIanation of terms, a. Passport. A docu-ment issued by the Department of State whichpermits ahv individual to leave and reenter theUnited Statesv There are four types of passports.The individual assignment determines the type ofpassport to be issue*

b. Visa. 'An indorsement made on a passport bythe proper authorities (usually embassy or consularofficials) of a country to be visited, showing that thepassport has been examined and that the bearer mayproceed to that country.

c. Installation commander.(1) For military personnel, the home station

commander or Headquarters, Departmentof the Army Staff agency concerned.

(2) For civilian personnel traveling on tem-porary duty overseas, the commander atthe place of employment.

(3) For civilian personnel recruited for overseaduty, the personnel officer at the installa-tion which has responsibility for theaccomplishment of the initial processing.

(4) For dependents, the installation com-mander processing the dependent foroversea travel.

3. Forms for use in applying for passports.a. DD Form 1056 (Authorization to Apply for a"No-Fee" Passport). DD Form 1056 will be issuedto military personnel for official travel, militarydependents eligible for and desiring Governmenttransportation, civilian employees for official travel,and dependents of civilian employees eligible forand desiring Government transportation as theirauthority to apply for a "no-fee" passport. (Aseparate DD Form 1056 will be issued to eachmember of the family requiring a separate passport.See para 46.)

(1) Military personnel. The installation com-mander will issue DD Form 1056 approxi-mately 90 days prior to the reporting oravailability date when practicable. Eitherorders or the alert of the individual may beused as a basis for issuing DD Form 1056.(Nominees for foreign assignments undersec III, ch 4, AR 600-200, will not beissued DD Form 1056 until such time as theindividual has been definitely selected.)

(2) Civilian personnel. The installation com-mander will issue DD Form 1056 to acivilian employee at the time he is notifiedthat he is being considered for a positionin an area where a passport is required(app. I).

(3) Dependents.(a) For concurrent travel, installation com-

manders will issue DD Form 1056 to

dependents of the military member atthe time of initial processing of appli-cation for concurrent travel. DD Form1056 will be issued to dependents of acivilian employee at the time the em-ployee is notified that he is being con-sidered for an oversea position where apassport is required (app. I).

(6) For nonconcurrent travel, oversea com-manders will issue DD Form 1056 todependents authorized to travel to anoversea area. (Issuance of DD Form1056 is unnecessary if dependent appliedfor a passport for concurrent travelwithin the preceding year, since passportapplied for in connection with applica-tion for concurrent travel will be held atthe Military Traffic Management andTerminal Service Area Command for 1year.)

(c) In all other instances the agency oractivity responsible for processing thedependent for travel will furnish DDForm 1056.

Note. Treaties and agreements between theUnited States and various foreign countriesdefine dependents differently. DD Form 1050may be issued to dependents who are closeblood or affinitive relatives only when suchauthorizations do not violate these treaties andagreements. Any questionable ease should bereferred to either the Army commander in thecountry where such individual plans to travel,or The Adjutant General, ATTN: AGAO-CP,Department of the Army, Washington, D.C.,20310,

(4) Special instructions for completing DD Form1056.

(a) Item 2. Enter country or countries onitinerary where duty will be performed.In the case of civilians traveling toemployment in Korea, enter Japan andKorea, (Code letters, APO numbers, orother general language will not be used.Nicknames will be used only whenauthorization for such use has beenassigned by Headquarters, Departmentof the Army (para 5).)

(6) Item 3. If assignment is to MAAG (toinclude off-shore procurement duty whenindividual enters country under MAAGagreement); Mission; or other special-type duty, e.g., CENTO, enter this

information. If the position to beoccupied is Chief, MAAG; DeputyChief, MAAG; Chief of Staff, MAAG,when such position is authorized in lieuof Deputy Chief, MAAG; or Chief,Army Section, MAAG; also enter thisinformation. For dependents of suchpersonnel, enter information on sponsor'sassignment as above. For all othertravel enter the words "NOT APPLI-CABLE."

(c) Item 7. Check block for PCS or TDY,as appropriate. If TDY enter length ofTDY. (The abbreviation "TAD" isused by Navy under the same circum-stances as "TDY" is used by Army andAir Force.)

(d) Item 10. Enter required information foreach address at which applicant will bestaying while awaiting departure foroverseas.

(e) Item 11. For dependents—enter appro-priate Military Traffic Managementand Terminal Service Area Command orother address, as appropriate. Formilitary and civilian personnel—PCStravel, enter appropriate Military TrafficManagement and Terminal Service AreaCommand; TDY travel, enter address ofindividual or other appropriate location.

(/) Item IS. For dependents travelingconcurrently, date to be entered will be35 days prior to sponsor's reporting oravailability date; for nonconcurrenttravel, date to be entered will be 35days prior to first day of the period inwhich the dependent is expected totravel. For military personnel, date tobe entered will be 35 days prior toreporting date or availability date.

(g) Other items. All other items are self-explanatory.

(h) Distribution. Copies 1 and 2 will befurnished to the applicant or sponsor forattachment to Passport* Application(Form DSP-11). Copy 3 will beretained by the installation commander.Copy 4 MUST BE FORWARDED tothe Commanding Officer, U.S. ArmyService Center for the Armed Forces,ATTN: ANRDP-P, The Pentagon,

Washington, D.C., 20310, immediatelyafter issuance. When required by appen-dix I, visa photographs will be forwardedwith copy 4 for use in obtaining visas.

b. Department of State Form DSP-11 (PassportApplication). This form is used to apply for pass-ports (para 6) and one copy will be filled out byeach applicant for a passport when passport is re-quired as indicated in appendix I. The applicantwill complete Form DSP-11, except for his signatureand the certification by the passport agent or clerkof court, prior to taking it to the passport agent,deputy clerk of court or clerk of court. In com-pleting applications, military personnel will entergrade and branch of service opposite the word"Occupation." Civilian personnel will enter theircivil service grade and title in that space. On FormDSP-11 given to military and civilian personneland their dependents for official travel, the spaceentitled "Mail Passport To" will be left blank.

Note. FS-176 (Application For Passport) is used in lieu ofDSP-11 in oversea areas.

c. Department of State Form DSP-Wa (BirthAffidavit). Form DSP~10a is used when it isnecessary to prepare a birth affidavit (para 8).

d. Department of State Form DSP-17 (PassportRenewal Application). Applications for renewal ofpassports must be submitted on Form DSP-17(para 10).

Note. FS-299 (Application For Renewal, Amendment andExtension) is used in lieu of DSP-17 in oversea areas.

e. Department of State Form DSP-19 (Applicationfor Amendment of Passport). Applications for 'amendment of passports must be submitted on FormDSP-19 (para 9).

Department of State Form DSP-64 (Affidavit ofInability To Present Previously Issued Passpoit).Form DSP-64 is used when a valid or potentiallyvalid passport cannot be presented with the appli-cation for a new passport. It is necessary to submita sworn statement with the new application settingforth the reasons why the previously issued passportcannot be presented (para 8).

g. Supply of forms. All forms prescribed in thisregulation will be requisitioned through normalpublications supply channels except:

(1) FS forms referred to in 6, d, and e abovewhich are obtainable at consular officesoverseas, and

(2) Those forms specifically indicated in appen-dix II as obtainable elsewhere.

4. Responsibilities of installation commanders.The installation commander will insure that militaryand civilian personnel and their dependents whosetravel-is authorized at Government expense to anoversea area where passports are required, applyfor passports. Such application will be completedprior to departure from home station on leave,delay en route, or travel status. When notified thata military member (except Defense attache per-sonnel) is alerted, or that a civilian employee isbeing considered, for an oversea position where apassport is required, the installation commanderwill—

a. Complete DD Form 1056 in accordance withparagraph 3a(l) and furnish copies 1 and 2 to theindividual, with one copy of DSP-11 (para 36).

6. Complete DD Form 1056 (Authorization ToApplySFor a "No-Fee" Passport) in accordance withparagraph 3a (3) and furnish copies 1 and 2 todependentsVwith one copy of Form DSP-11 (para36). In casesNvhere individual members of a familycan justify a requirement for separate passports forofficial travel durihg their stay overseas, and desireto apply for separate passports, complete separateDD Forms 1056, and furnish copies 1 and 2 to eachdependent concerned, \ith one copy of FormDSP-11.

c. Instruct individual about applying for apassport and complying with this regulation. In—

d. Insure that individual possesses or takesimmediate action to obtain for himself and hisdependents all necessary documents required forpassport application (para 8).

e. Furnish the information in appendix II toindividual with alien dependents. For furtherinformation on naturalization and citizenship, seeAR 608-3.

/. Arrange to have photographs taken.g. Inform individual where to apply for a passport

and advise him to request the passport agent, deputyclerk of court, or clerk of court to furnish him areceipt for his passport application fee and that ofhis dependents, where applicable. Furnish per-

sonnel transportation to place of application, ifnecessary, using available transportation.

h. Immediately after issuance of DD Form 1056,forward copy 4, thereof, to the Commanding Officer,U.S. Army Service Center for the Armed Forces,ATTN: ANRDP-P, The Pentagon, Washington,D.C., 20310. When required by appendix I, visaphotographs will be inclosed for use in obtainingvisas.

i. If an individual or his dependent has a passportwith at least 2 years' validity, in lieu of takingactions indicated in a, b, and c above, forward thepassport to the U.S. Army Service Center for theArmed Forces, inclosing copy 1, copy 2, and copy4, DD Form 1056, with any necessary visa photo-graphs as indicated in appendix I.

j. Advise personnel that inquiries regardingpassport applications, passports, and visas forofficial travel for themselves or their dependents willbe addressed to—

k. Advise personnel that they must sign theirpassports immediately upon receipt since a passportis not valid unless it is signed by the bearer.

I. Not permit personnel to depart the installation(or leave, TDY en route, or otherwise) until theyhave applied for their passports and the necessaryvisa photographs and information as shown inappendix I have been obtained and forwarded to theUnited States Army Service Center for the ArmedForces.

m. Not permit personnel to depart the installation(on leave, TDY en route, or otherwise) until depend-ents who will travel concurrently with them haveapplied for passports and necessary visa materialshave been forwarded to the U.S. Army Service Centerfor the Armed Forces.

n. Have the individual present the receipt for hispassport application fee and that of his dependents,where applicable, to insure that application hasbeen made. Indicate the date, place, and court orofficial where application f'-.r passport was filed onthe reverse side of copy u t.installation Copy) of DDForm 1056.

5. Expeditious passport processing for unit move-ments. When deemed necessary. Headquarters,

Department. of the Army will direct installationcommanders to expedite passport processing forindividuals assigned to units under their jurisdictionwhich have been issued oversea movement orders.In such.instances, the following procedures will beused. Any supplemental instructions necessary as aresult of the specific movement involved will beincluded in the Department of the Army messagedirecting expeditious passport processing.

a. A project officer will be appointed who will beresponsible for processing the passport applicationsand forwarding them to the Commanding Officer,U.S. Army Service Center for the Armed Forces,ATTN: ANRDP-P. The officer should not be amember of the unit concerned since followup actionsmay be necessary after the unit and/or individualshave departed.

6. Appropriate application forms will be com-pleted, except that in Item 2 of DD Form 1056(Authorization to Apply for a "No-Fee" Passport)the nickname furnished in the Headquarters,Department of the Army message directing ex-peditious passport processing will be inserted.

c. Verification of place of birth will be accom-plished by copy of birth or baptismal certificate;statement that applicant has had a previous pass-port or, if naturalized, a copy of naturalizationpapers. In cases where birth certificates are notreadily available and extract copies cited in d beloware used, they will be authenticated by an adjutantor personnel officer. Extract copies must be com-pletely legible; thermofax copies are not acceptable.The same officer will authenticate the DD Form1056.

d. If time is short and copies of birth or baptismalcertificates are not readily available, extracts fromDD Form 4 or DD Form 47, as appropriate, willbe used as temporary proof of place and date ofbirth. Copies of birth or baptismal certificates arerequired to support the temporary evidence, andapplicants will note on the extracts that suchcertificates will be forwarded as soon as possible.These certificates will be sent to the project officerwho will follow-up and complete the action afterthe applicants have departed.

e. The project officer will personally contactofficials where the passport applications will beprocessed and explain that the requirement for thepassports is to support a classified military move ofpersonnel in which time is critical. He will arrangea procedure between himself and the officials which

will include an agreement to enter only the nick-name specified in Department of the Army message,in Item 2 of DD Form 1056 for destination andcountries to be visited, and that the passport appli-cations will be returned to the project officer by theofficial instead of the usual procedure of forwardingthe applications to the Department of State. Inaddition, he will present a letter addressed to theofficial and signed by an officer other than himselfrequesting that due to the urgency involved, thepassport applications be returned to the projectofficer rather than forwarded to the Department ofState.

/. The project officer will inform all individualsto request a receipt from the passport agent, deputyclerk of court or clerk of court for the passportapplication fee. In order to insure that applicationsare made, he will require the individuals to presentthe receipts for their passport application fees (para4n).

g. Upon receipt of the applications from theofficials, the project officer will forward the individualpassport applications to the U.S. Army ServiceCenter for the Armed Forces as expeditiously aspossible with a covering letter of transmittal. Theapplications will be forwarded in groups; however,completed applications will not be delayed in orderto send complete unit packages. When feasible,applications should be hand-carried. All applica-tions will be transmitted so as to reach U.S. ArmyService Center for the Armed Forces as soon aspossible but not later than 7 days prior to personnelreadiness date.

h. The letter of transmittal will state that thepassport applications are forwarded for processingin furtherance of Operation. _ _ _ _ . . _ . _ . _ _ _ _ _ _ _ _ . _ .

and will cite the Department of the Army messagedirecting expeditious passport processing. In addi-tion to the passport applications, it will inclosethe following:

(1) A legible copy of a roster listing the namesof the individuals on the passport appli-cations being forwarded and the organi-zations to which they are assigned.

(2) Copies 1, 2, and 4 of DD Form 1056 for. each individual.

i. If the unit movement has an advance element,applications for individuals belonging to the advanceelement will be given priority action over thosescheduled for later departure. In such cases, the

roster discussed above will specify those individualswho are assigned to the advance element.

j. If the unit being moved obtains filler replace-ment personnel from units stationed at nthcrinstallations, DD Form 1056 will he prepared inaccordance with 6 above and issued to such replace-ments by their home station commanders. Inaddition, the home station commander concernedwill assure that the U.S. Army Service Center forthe Armed Forces copy of the DD Form 105G isforwarded to that office without delay, and that thereplacement personnel have applied for passportsprior to their departure to join the unit being moved.The passport applications for filler replacementpersonnel will be forwarded by the passport agent,deputy clerk of court or clerk of court to Departmentof State in the usual manner. However, uponreceipt of the passports for such replacement per-sonnel in the U.S. Army Service Center for theArmed Forces, they will be forwarded by that officeto the appropriate project officer for the unitmovement concerned.

6. When and where to apply. When possible, theapplicant should apply for a passport as soon as hehas been issued DD Form 1056 (para 3) and at least60 days or more before availability date.

a. Persons in United States. Within the UnitedStates, applications for passports must be executedbefore a clerk of a Federal or State court authorizedby law to naturalize aliens, a deputy clerk of courtor a passport agent of the Department of State.Passport agents have been appointed at variousmilitary installations by the Department of State.In addition, passport agents are located at—

(1) Department of State, Washington, D.C.,20524.

United States, the chief executive of Puerto Rico,the Virgin Islands, Guam, and American Samoa areauthorized to accept passport applications. Inaddition, diplomatic and consular officers and certainauthorized military officials may accept passportapplications.

Note. Military and civilian personnel, and their depend-ents, who are assigned to the Defense Attache System, willexecute application for passports only upon receipt of instruc-tions from Defense Intelligence Agency.

7. Photographs, a. Passport photographs. Appli-cants must furnish two identical photographs takenwithin 2 years of the date they apply. The photo-graph must be a readily recognizable likeness of theapplicant. A pleasant natural pose is recommended.Group pictures should be furnished when a wifeand child or children are included in one application;however, children who apply separately for passportsmust submit separate photographs. (See 6 below.)Military personnel may submit photographs takenin either military or civilian attire regardless ofwhether the purpose of the trip is for official businessor for pleasure.

(1) Photographs must be on thin photographicpaper with a light background, must showfull front view of the applicants' features,and will be taken without headgear.Photographs will not be smaller than 2J^by 2% inches nor larger than 3 by 3 inches.

(2) Black and white photographs to be usedon passports for official travel may beobtained from the nearest Armed Forcesinstallation at which facilities are available.

(3) Both photographs will be signed by theapplicant in such manner as not to obscurethe features of any person included in thephotograph.

(4) Acetate or film base, snapshot, polaroid, ors, vending machine prints are not acceptable.\J Tinted photographs are not acceptable;

neither are photographs printed on paperthe back of which has been glazed, sincethey will not adhere to passports.

(5) Color photographs may be used, but mustbe procured at the individual's expense.Color photographs must be printed on apaper base and must K- rapable of with-standing a mounting temperature from180° to 200° Fahrenheit. "No-fee" pass-ports will not be replaced solely for sub-stitution of a color photograph.

6. Exceptions to passport photograph requirements.Individuals submitting applications for passports,or applications for amendment of passports, whileabroad, which will include children born abroad whoare under 5 years of age, are not required to furnishphotographs of such children for inclusion in thepassport. In lieu of the photograph, the officialissuing or amending the passport will write thebirth dates of the infants opposite their names in thepassports. Photographs may be submitted for suchchildren, for inclusion in the passports, whenspecifically desired by the parents or where there is aspecial reason such as local immigration policies.

c. Visa photographs. Travel to certain countriesrequires additional photographs for visa purposes(app. I). Additional photographs, when required,will be forwarded as provided in paragraphs 4h and4.i. Photographs for visas will conform to the speci-fications prescribed in a above. In addition, thename grade, and service number will be placed onthe reverse of each photograph required for visas bymilitary personnel. The name only will be placed onthe reverse of each photograph required for visasby civilian personnel and dependents. Care must betaken to insure that this information does not showthrough to the front side of the picture. Appendix Ispecifies the number of visa photographs requiredfor each passport issued to the family; add an equalnumber for each additional passport issued to thefamily. For example, if a principal dependentapplies for one passport for herself and children fortravel to Afghanistan, three visa photographs ofthe group are required. For each child traveling on aseparate passport, three additional photographs ofthe child are required for obtaining a visa for travelto Afghanistan.

8. Proof of citizenship. An applicant for a pass-port and each person included in the passport mustsubmit documentary evidence of United Statescitizenship (a below) with the application, unless hehas been issued a passport previously or has beenincluded in a passport. The birth certificate

described in a(2) below is the document required bythe Department of State as birth evidence to supportthe application for a passport of natural bornfit iae i is of the United States. The Departmentof State has provided that persons who are not ableto obtain a birth certificate because there is norecord of birth may submit a baptismal certificatedescribed in a(3) below. If none of the primarydocuments can be submitted, then secondarydocuments listed in b below, may be submitted forconsideration. Any evidence of citizenship sub-mitted, other than a passport previously issued tothe applicant or a certificate of birth, must beaccompanied by a statement from the officialcustodian of birth records in the State, territory, orother area where the birth of the applicant occurredshowing that a search of the records did not disclosea record of birth (app. III). The Department ofState will include all birth, baptismal, and naturali-zation certificates with the passport for delivery tothe applicant.

Note. A commissioned officer who ia born in the UnitedStates and whose parents are United States citizens need notsubmit evidence of nationality. He need only enter his officialgrade on the passport application and show his militaryidentification card to the Passport Agent or Clerk of Court.However, a commissioned officer who is a naturalized citizenmust present his naturalization certificate. A commissionedofficer who was born abroad of native born United Statescitizen parent(s) must submit one of the following: a certifi-cate of citizenship issued by the Immigration and Naturali-zation Service, the passport on which he entered the UnitedStates, a Consular Report of Birth, or a Certificate of Birthissued by the Department of State. If none of these areavailable, he should submit his foreign birth certificate,evidence of the United States citizenship of his parent(s),and an affidavit from his parent(s) showing the periods andplaces of residence in the United States and abroad (specifyingany periods in the Armed Forces) before the applicant'sbirth.

a. Primary documents.(1) Passport. Normally, a passport issued to

the applicant or one in which he wasincluded is sufficient evidence of citizen-ship. If the passport is not available, theapplicant should identify the passport inhis application by stating the name of thebearer, the number, if known, and thedate or approximate date of issue. If thepassport is valid or can be re validated, hemust include in his application or attachthereto a statement explaining in detail whyhe cannot submit the passport and whatefforts he made to locate it (para 3).

(2) Birth certificate.(a) A birth certificate must bear the written

signature of the registrar or othercustodian of the original record and theseal of his office unless the seal and signa-ture are impressed on the certificate by aProtectograph device. A certified copyof the original report of birth is prefer-able ; however, a transcript from therecord will be accepted if it shows thename of the child, the date and place ofbirth, and the date the report was filed.

(6) A certificate which does not show thedate of filing or which shows that it wasissued upon a delayed report of birth maybe accepted if it shows that the report ofbirth was filed by a person who hadpersonal knowledge of the birth or wassupported by other acceptable evidenceof birth in the United States. A birthcertificate issued upon a delayed birthrecord which was created pursuant to acourt order will be accepted if thecertificate or an accompanying letterfrom the registrar of births indicates thatthe court received evidence which, ofitself, would be acceptable for passportpurposes.

(c) A hospital birth certificate will beaccepted if it shows the name of thechild, the date and place of birth, thenames of the parents, the name andlocation of the hospital, bears the sealof the hospital and written signature ofthe superintendent and was issued with-in a few days after birth.

(d) A photostat of the original report ofbirth which forms a part of a legally-certified copy of the record is acceptable.The signature of the official custodian ofthe records will be affixed and his raisedor stamp seal impressed on such a copy.A photostat of a copy of a transcript of arecord, such as an issued certificate ofbirth, is not acceptable.

(3) Baptismal certificates. If a birth certificateis not obtainable, a baptismal certificate orchurch ceremonial certificate may be sub-mitted if it gives the name of the child, thedate and place of birth, the date of baptismor other ceremony which is the basis of

the record, the name and location of thechurch, and the date the record was made.To be acceptable, a baptismal certificate orother church certificate must show that thebaptism or other ceremony took placeshortly after birth and that the record wasmade at or shortly after the date of theceremony. It should bear the writtensignature of the pastor or other authorizedchurch official and the seal of the church(if any).

b. Secondary documents.(1) Affidavit. An affidavit must show the

name of the person concerned and the dateand place of birth, and must state brieflyhow the affiant's knowledge concerningthe birth was acquired. All affidavits mustbear the impression seal of the officer beforewhom they were executed (para 3c).

(a) A single affidavit may be submitted if itis made by a parent or by a physician,nurse, or midwife who attended thebirth, or by a person who did not attendthe birth but who became aware of thebirth at the time or shortly thereafter.

(b) If the applicant is unable to submit anaffidavit under (a) above, he may submitthe affidavits of two or more persons whohave known him for 20 years or more andwho can testify on information andbelief concerning the date and place ofhis birth. These persons should state intheir affidavits the facts and circum-stances which cause them to believe thatthe applicant was born on that date andin the place set forth and how, when, andfrom whom they received the infor-mation. These affidavits must be sup-ported by other secondary or indirectevidence of birth in the United States asthe applicant may be able to obtain,such as school, early census, confir-mation, military service selective service,voting, and family records. Pertinentfamily records might consist of evidencethat the applicant's parents were marriedin the United States and that otherbrothers and sisters were born in thiscountry.

(2) Newspaper files, ships' logs, family Bibles.If it is impossible to obtain any of the

foregoing documents, the Department ofState will consider copies of pages of afamily Bible, diary, ships' logs, newspaperfile, etc., containing a record of thebirth, which are properly verified by anaffidavit of the custodian of the record.The notice or entry must show the name ofthe child, the names of the parents, andthe date and place of the birth. It must beunmistakably clear that the notice waspublished or the entry made at the timeof birth or shortly thereafter.

c. Naturalization certificate. Persons who claimcitizenship through naturalization must furnishtheir naturalization certificates. Any person whoclaims citizenship through naturalization of a parentmust submit either his own certification of derivativecitizenship or his parent's or parents' naturalizationcertificate.

Note. The unauthorized printing, photographing, orotherwise making a copy of a certificate of naturalization orcitizenship may subject a person to a fine of 55,000 or im-prisonment of not more than 5 years, orboth (18 USC 1426(h)).

d. Extract from service record. In extreme emer-gencies, the Department of State may accept asproof of citizenship an extract from the service recordof a member of the Armed Forces who is not acommissioned officer and who was born in theUnited States of native American parents when themember is required to leave the United States onsuch short notice that he cannot obtain the usualevidence of citizenship by birth in this country.When an extract from service record stating theapplicant's place and date of birth is furnished bythe applicant, it will be certified by an adjutant orpersonnel officer. The serviceman must note onthe extract that he will obtain as soon as possiblethe usual proof, such as a birth or baptismal certifi-cate or affidavit. When the birth evidence isobtained it should be sent to the Passport Office,Department of State, Washington, D.C., 20524.If abroad, he may transmit the evidence to thePassport Office through an American Embassy,Legation, or Consulate. The Department of Justicewill not accept extracts from service records as legalevidence of citizenship and children born to alienspouses of service personnel who furnish suchextracts as evidence of citizenship may be denied acertificate of citizenship normally provided childrenin this category.

e. Special provisions for furnishing evidence ofcitizenship.

(1) Elderly applicants. When, because of theapplicant's age, the nonexistence of recordsand the demise of friends, relatives, andacquaintances, the applicant is unable topresent any of the foregoing acceptableevidence, the Passport Office will considerissuing a passport if the applicant submitsan affidavit explaining why he is unable topresent such evidence and any indirector secondary evidence of birth in the UnitedStates as he may be able to obtain. Theapplicant should, if possible, obtain au-thenticated copies of records which placehim in the United States since an early ageor furnish statements from the custodianof such records certifying to their existence.

(2) Applicant of unknown parentage. A personwho claims that his parents are unknownbut that he was found in the UnitedStates while under the age of five mustsubmit evidence establishing that hispresence in the United States was actuallyknown at that time. Such evidence mayinclude—

(a) Affidavits by persons who know thefacts; for example, persons who caredfor him in early infancy or who knew ofhis presence in a family while under theage of five, or

(b) Certified records of a court or charitableor other organizations.

(3) Adopted child. Although the Departmentof State appreciates the desire of manyparents that information regarding adop-tion be withheld from their child, it isnecessary that true record of identity andcitizenship be submitted with the passportapplication. If the adopted child is under21 years of age, one of the parents mayexecute the application in his behalf. If,however, he is over 21 years of age, theparents may arrange with the clerk ofcourt or passport agent to submit evidenceof his identity and citizenship after theapplication has been executed. Appli-cations of this type should be accompaniedwith a request that information regardingthe adoption be returned directly to theparents rather than be included in the

envelope with the passport, which is theusual practice. Below are the require-ments of identity and citizenship of adoptedchildren who were born in the UnitedUnited States.

(a) A certified copy of the adoption recordis acceptable in lieu of birth evidenceprovided—

1. The record shows that the child wasplaced in custody of the foster parentsin the United States at a very earlyage, or .

2. That documentary evidence or testi-mony establishing the parentage, thedate and place of birth, or child'spresence in the United States shortlyafter birth was presented to the court.

(b) If a certified copy of the adoption recordor normal birth evidence cannot beobtained, the sworn statement of anofficial of a charitable or other organi-zation is acceptable, provided it includesthe following:

1. The date when the custody of the childwas obtained initially.

2. The date and place of child's birth.3. The names and place of residence of

the adoptive parents.4- The date and manner of adoption, whether

formal, informal, or mere custody.5. The basis for the affiant's knowledge.6. If the child has not been formally

adopted, the fact shall be set forth inthe affidavit, together with a state-ment indicating that the child hashad a permanent or established placefor a definite period of time with thefamily in whose custody the child hasbeen placed.

(4) Children born abroad.(a) If the child was born abroad of an alien

mother and an American father andtraveled to the United States on apassport, the United States passportshould be furnished the passport agentor clerk of court.

(6) If the child described above did nottravel to the United States on a UnitedStates passport, the child's foreign birthcertificate, evidence of his father's UnitedStates citizenship, an affidavit by his

father stating the periods and places ofhis residence in the United States andabroad, periods of service in the ArmedForces, and the marriage certificate ofparents of the child should be furnishedto the passport agent or clerk of court.

(c) If the child was born abroad of an alienfather and an alien mother, evidence ofhis naturalization as a United Statescitizen will be furnished to the passportagent or clerk of court. This may be acertificate of citizenship or naturalizationcertificate of his parents. If derivingcitizenship through a parent's naturali-zation, he must submit his parent'snaturalization certificate, his foreignbirth certificate, and evidence of ad-mittance to the United States for perma-nent residence. If one parent is deceased,submit death certificate. If parents aredivorced, submit court order showingparental sole custody.

(d) If child was born abroad out of wedlockof one American parent or of twoAmerican parents, submit evidence ofcitizenship of the parent or parents, astatement of parent's or parents' periodsand places of residence (physical pres-ence) in the United States or its outlyingpossessions prior to the birth of the child,and the marriage certificate of theparents. A sworn statement from thefather stating that he is the naturalfather should also be submitted in anycase. A mere statement "acknowledgingpaternity" is not sufficient.

9. Amendment of passports. Passports may beamended to include or exclude the bearer's citizenspouse and any unmarried minor children, step-children, or adopted children, if citizens. Appli-cations for amendment of passports must be sub-mitted on Department of State Form DSP-19(Application for Amendment of Passport). (Seepara 3e.) Applications for amendment of passportsto include individuals must be sworn to (or affirmed)before an agent of the Department of State ordeputy clerk or clerk of a Federal or State Court.Applications for amendment of passports to excludeindividuals or change other information thereonneed not be executed before a passport agent,

(Do not include anyone inthe new photograph who is already in the passport.)Amendments to passports may be made by theDepartment of State's passport agents at thelocations listed in paragraph 6a(l) through (10)and the other officials listed in paragraph Gb. Pass-ports forwarded to the U.S. Army Service Centerfor the Armed Forces will be processed for amend-ment through the Department of State. No fee iscollected for amendment of passports.

10. Renewal of passports, a. Limit of validity.Passports, which have been issued to military andcivilian personnel and their dependents can berenewed as fol

6. Applications. Applications for renewal ofpassports must be submitted on Department ofState Form DSP-17 (Passport Renewal Appli-cation) . Applications for renewal of passports neednot be executed before a passport agent, deputyclerk or clerk of court. See paragraph 3d.

(1) Within the United States, applications forrenewal of passports issued for officialtravel will be forwarded to the Com-manding Officer, U.S. Army Service Centerfor the Armed Forces, ATTN: ANRDP-P,The Pentagon, Washington, D.C., 20310,and be accompanied by the passport andcopies 1, 2 and 4 of DD Form 1056.

(2) In Puerto Rico, Guam, American Samoa,and the Virgin Islands, application forrenewal of passports will be made to thechief executive. The application will beaccompanied by the passport and one copyof the travel orders directing travel abroad.

(3) In oversea areas, the application will bemade to the appropriate consular official ofthe United States in the foreign country in

which stationed or residing, and be ac-companied by the passport and one copyof the travel orders directing travel abroad,Oversea commanders may designate mili-tary officers or enlisted personnel to actas couriers to process applications forrenewal in bulk on a weekly basis wherethe personal appearance of the individualis impractical or would result in hardship.

c. Validity for returning overseas from leave in theUnited States. Oversea commanders will insure thatpersonnel, who will return to an oversea area follow-ing leave in the United States, arc informed of therequirements for a passport which is valid for thereturn overseas. They will issue properly authen-ticated DD Form 1056, have visa photographs taken,if required, and advise personnel to—

(1) Request renewal of passport before theirdeparture from the oversea area, and

(2) Forward their passport; visa photographs,where required; copies 1, 2, and 4 of DDForm 1056; and one copy of travel ordersdirecting return travel abroad to the U.S.Army Service Center for the Armed Forces,immediately upon arrival in the continentalUnited States. Also specify whether theirpassport is to be forwarded to the MilitaryTraffic Management and Terminal ServiceArea Command, the aerial port of embar-kation, or their home address. The U.S.Army Service Center for the Armed Forceswill insure that the passport is valid forthe return overseas.

11. Passport and visa fees. Passport and visafees, including the cost of photographs and birthcertificates, or other acceptable evidence of birth,are reimbursable when they are an expense ofofficial travel.

a. Passport application. A fee is collected fromall persons for the execution of passport applicationsand retained by the clerk of court or passport agent.This fee is SI if the application is executed before aclerk or deputy clerk of a Federal court or a passportagent of the Department of State and S2 if theapplication is executed before a clerk of a Statecourt.

to a person who is going abroad on Govern-ment business or to members of his im-mediate family when such travel is author-ized as evidenced by a DD Form 1056executed by appropriate official.

(2) A fee of 89 for the issuance of a passport toa person traveling on leave or in a touriststatus must be in the form of a bank draft,cashier's check, money order, or personalcheck, made payable to the PassportOffice, Department of State, Washington,D.C.

c. Passport renewal.(1) No fee is collected for renewal of a passport

for official Government travel.(2) A fee of $5 is collected for renewal of a

passport for tourist travel.

12. Visas. The countries where a visa is requiredare shown in appendix I. The Commanding Officer,U.S. Army Service Center for the Armed Forces willobtain visas for official travel. He will base hisdecision on items 2 and 3, DD Form 1056, copy 4of which is forwarded to the U.S. Army ServiceCenter for the Armed Forces in accordance withparagraph 3a(4)(&).

13. Receipt and distribution of passports andvisas for official travel. The Department of Statedelivers all passports for official travel to the Com-manding Officer, U.S. Army Service Center for theArmed Forces, Military District of Washington,U.S. Army, Washington, D.C., 20310. Upon receiptof passports from the Department of State, the U.S.Army Service Center for the Armed Forces is re-sponsible for obtaining visas from foreign embassiesand legations. He then forwards the passport andvisas to the appropriate Military Traffic Manage-ment and Terminal Service Area Command fordelivery to the traveler except the following:

a. If a traveler has valid Government travelorders, but he elects to travel by commercial meansat his own expense, his passport will be sent direct tohim.

b. Passports for military or civilian personnelwho travel overseas in a temporary duty status willbe mailed to them unless otherwise directed.

Note. The Department of State releases passports forDefense attache personnel and dependents to the DefenseIntelligence Agency. The necessary visas are then obtainedand the passports delivered to the personnel.

14. Inquiries about status of passports and visas.Inquiries concerning passports for official travel willnot be made direct to the Department of State atany time.

a. When inquiry is necessary concerning thestatus of passports and visas, installation com-manders will communicate with the CommandingOfficer, U.S. Army Service Center for the ArmedForces, ATTN: ANRDP-P, The Pentagon, Washing-ton, D.C., 20310.

b. When inquiry concerns policy matters onissuance of passports and visas, installation com-manders will communicate with The AdjutantGeneral, ATTN: AGAO-CP, Department of theArmy, Washington, D.C.r 20310.

15. Authentication of passport applications forlarge groups. When difficulty is encountered inarranging for authentication of passport applicationsby local clerks of court for large groups, e.g., unitmovements, commanders will address a letter toThe Adjutant General, ATTN: AGAO-CP, Depart-ment of the Army, Washington, D.C., 20310, out-lining the problem and requesting assistance withthe resolution thereof.

16. Change of destination or reporting date, orcancellation of travel. Whenever the destinationor reporting date of military personnel, civilianpersonnel, or their dependents is changed or thetravel is canceled following issuance of DD Form1056, a copy of amendatory orders will be forwardedto the Commanding Officer, U.S. Army ServiceCenter for the Armed Forces, ATTN: ANRDP-P,The Pentagon, Washington, D.C., 20310, in orderthat appropriate action may be taken with respect tothe passport.

17. Passports and visas for other than officialtravel. Persons who travel on leave or in a touriststatus will procure regular (tourist) passports; thisincludes military personnel who will reside in aforeign country on release from active duty, dis-charge, or retirement from the Army; dependentswho do not have approved Government travel; andcontractor personnel, when the contract is with thecompany by which they are employed. Militaryi'acilities will not be used when applying for passportsor visas for use while on leave or in a tourist status,nor will notification of such application be'made tothe U.S. Army Service Center for the Armed Forces.The application for a passport will specify that the

passport is to be delivered to the applicant at hishome address. Applicants will address inquiriesconcerning visas for such travel to the appropriateconsular officials of each country to be visited.Persons applying for tourist passports will submitphotographs taken in civilian attire. Applicants forsuch passports must pay the usual fees for executionof the passport application, issuance of the passport,and any special postal services requested.

18. How to avoid delays in receipt of passports.Delays in applying for passports cost the Govern-ment and the traveler both time and money. It isimperative, therefore, that applications for passportsbe made as quickly as possible and that they areaccurate and complete. Submission of an accurateand complete application avoids later inquiries bythe Department of State. To insure that delays areavoided, check to insure that the following has beencomplied with:

a. Properly complete DD Form 1056, with specialattention to item 3.

6. Attach copies 1 and 2 of DD Form 1056 toForm DSP-11 and give these forms to the indi-vidual applying for a passport.

c. Immediately after issuance of DD Form 1056,forward copy 4, thereof, to the U.S. Army ServiceCenter for the Armed Forces, with required visaphotographs, if any, attached.

d. Inclose satisfactory birth evidence with thepassport application, as outlined in paragraph 8.

e. On passport application—(1) Give name in full in the space provided

on the first page of the application form.Differences in the name of the applicantas it appears on the application and on theDD Form 1056 submitted with the appli-cation may delay the issuance of thepassport. If there is any variation, theapplication must be accompanied by acopy of the court order authorizing thechange of name, or other satisfactory expla-nation of the discrepancy.

(2) For naturalized citizens, state periods andplaces of residence outside the UnitedStates subsequent to naturalization.

(3) Give ful l information concerning appli-cant's parents if they were not born in theUnited States, especially the names of theparents, whether they were ever naturalizedand, if so, when and where.

(4) Give dates of previous marriages.(o) Fill in all blank spaces on the application.

When an applicant claims that he has neverbeen abroad, the fact should be clearlystated in the application instead of merelydrawing a line through the appropriatespace.

19. Maintaining a control system. Commandersat all echelons will insure that a system of control willbe established for all passports issued military orcivilian personnel for official travel. This controlwill include the following:

a. Personnel in oversea areas and those whoseduties require them to be in possession of validpassport at all times will be checked periodically toinsure that their passports are in order.

6. When military and civilian personnel completetravel abroad and return to the continental UnitedStates, their passports will be forwarded to thePassport Office, Department of State, Washington,D.C., 20524, for retention unless their duties willrequire frequent travel to areas where passports arerequired. Those passports which are retained willbe safeguarded by the installation commander andwill not be used for personal travel. Upon request,a passport which has been forwarded to the Depart-ment of State will be canceled and returned to themember if he has retired from active military serviceor if he will not be traveling abroad on officialbusiness during the remainder of the validity periodof the passport.

c. Except for enlisted personnel who reenlistwithout a break in service, passports issued in con-nection with official travel will be surrendered uponrelease from active duty, discharge, or retirementand will be returned by the commander of the trans-fer activity or unit personnel section performingtransfer processing to the Passport Office, Depart-ment of State for cancellation. If requested, thatagency will return the canceled passport to theperson at his or her home address.

d. Dependent passports will be forwarded to thePassport Office, Department of State for cancella-tion. Dependents may request that canceledpassports be returned.

e. When a student whose sponsor's tour of dutyextends through the following summer vacationarrives in the United States, he may give a letterto the Immigration Inspector which describes thestatus of the sponsor's tour and states that the

student expects to rejoin the sponsor. The passport /. I. REQUIREMENTS FOR IMMEDIATE NATURALIZATION ANDACTION NECESSARY TO PETITION FOR NATURALIZATION.1. If you,entered the United States for permanent residence, the Immigration

and Nationality Act of June 27, 1952, as amended (section 1101 et seq., title 8,USCA), provides that you may be naturalized immediately if—

When yourtravel has been approved, the commander who processes your travel will issue DDForm 1278 (Certificate of Oversea Assignment to Support Application to FilePetition for Naturalization) which is your authority to file your petition for natura-lization. As soon as you receive this form forward it immediately to the appropri-ate Immigration and Naturalization Service officer who will notify you when andwhere to file your petition for naturalization. If your petition is granted, youshould follow the instructions on DD Form 1056 (Authorization to Apply for a"No-Fee" Passport).

3. You are urged to apply for citizenship, since you may experience delay ordifficulty in reentering the United States if you travel overseas as an alien. Thisis especially true of an alien spouse or an alien adopted child of a civilian employee.An alien spouse or an alien adopted child does not require a reentry permit or animmigrant visa to reentcr the United States, provided the individual is returningto an unrelinquished lawful permanent residence after a temporary absence abroadand is the spouse or child of, and has been residing abroad with, a member of theArmed Forces of the United States stationed abroad pursuant to official orders.

II. WHAT TO DO IF YOU CANNOT BE NATURALIZED1. If you have no foreign passport, you must apply for a passport before the

consul or diplomatic representative of the country in which you claim citizenship.2. If you are a stateless alien, or if your country is without consular or diplo-

matic representation in the United States, you must obtain an Affidavit of Identityand Nationality from the Commanding Officer, U.S. Army Service Center for theArmed Forces, ATTN: ANRDP-P, The Pentagon, Washington, D.C., 20310, andhave it drawn up before a notary public. This affidavit will serve as a passport.

3. If you are the alien spouse or alien adopted child of a Department of theArmy civilian employee you should apply for a reentry permit on Form 1-131,which may be obtained from any office of the Immigration and NaturalizationService.

a. Authorization to Apply for a "No-Fee" Passport (DD Form 1056).b. Foreign passport or certificate of identity and nationality in lieu of foreign

passport.c. Reentry Permit (required by alien spouse or alien adopted child of Depart-

ment of the Army civilian employee).5. For alien spouses or alien adopted children of Department of the Army

civilian employees. Since the reentry permit is valid for only 1 year and may beextended for 1 more year, you will, after remaining overseas for 2 years, be com-pelled to secure a "returning resident visa," which is valid for only 4 months andcannot be renewed. Each time you apply for a "returning resident visa" you mustappear before a U.S. consul, undergo a physical examination and pay fees for thevisa application, visa, and physical examination for which reimbursement is notauthorized. There is also the possibility that the consul might find some reasonwhich has developed since you were first admitted to the United States which wouldprevent him from issuing you a visa, in which case you would be prevented fromreturning to the United States.

APPENDIX III

PROCEDURES TO ORTAIN BIRTHCERTIFICATE FOR PASSPORT APPLICATION

1. To obtain a certified copy of a record of birth, write or go to the vitalstatistics office in the State, territory, or other area where the birth occurred. Inwriting for a certified copy, include the following:

a. Full name (at time of birth) of person for whom birth certificate is required.b. Date of birth.c. Place of birth (city or town, county and State, and name of hospital, if any).d. Sex and race.e. Parents names (mother's maiden name)./. Reason for applying for birth certificate. For example. Birth certificate is

required to obtain passport for official Government travel.2. If a birth certificate cannot be obtained, obtain the next most desirable

documentary proof of U.S. citizenship as prescribed in paragraph 8 of this regula-tion. Be sure to inclose with the birth evidence which you do submit with thepassport application, the reply from the vital statistics office of the particularState indicating that the birth is not recorded.
