Positively impacting the lives of our patientsEmpowering patients to take a more active role in their healthcare decisionsChoose to Know is an education and prevention program created by DLO specifically for Oklahomans. DLO provides both patients and healthcare providers with information on diagnostic test recommendations from leading disease prevention organizations such as the Centers for Disease Control and Prevention (CDC), World Health Organization (WHO), and American Heart Association (AHA). Rather than focus on a specific specialty such as women’s health, Choose to Know will concentrate on the importance of patient and provider education, promoting healthy lifestyles and having a proactive approach to healthcare. Patient messaging demographically targets specific Oklahomans to educate them on health conditions, suggested testing guidelines, and laboratory tests available. The provider material, designed to complement the patient messaging, consists of information such as Oklahoma-specific statistical health data, guidelines on certain disease states, demographical targeting, and DLO’s available diagnostic tests.

Education and ServicesDLO offers comprehensive patient services to simplify patient management and communication. But only DLO goes above and beyond by providing a range of education and wellness services for patients:

• ChooseToKnow.com provides information for patients and providers from leading disease prevention organizations on current disease and/or health crisis statistics, often comparing Oklahoma with National rankings.

• Direct patient access testing to order select lab tests without a health care provider. DLO DirectTM allows patients to have the most accurate and up-to-date information needed to proactively manage their personal health and cost of health care.

• Complete on-line patient health library, as well as printed brochures in English and Spanish

• On-line, secure access to test results, medical information and bill pay

• Secure, fast, patient-friendly access to individual results to empower better choices

Accessing health information has never been easier. MyQuest delivers critical information directly to a computer, tablet and smart phone giving patients the tools needed to view, access and securely share health information everywhere.

Receive and understand lab results MyQuest provides easy to understand results directly from the lab.

MyQuest Advanced Access DLO offers expanded, electronic access back to January 1, 2010 to your health data via the new service MyQuest Advanced Access.

Track medical information Update MyQuest with details about medical conditions and food and medication allergies.

Medication tracking and reminders Store medication information with reminders with smart phones and tablets only.

Track healthcare provider information Manage physician names, specialties and contact information, hospital and pharmacy numbers, even insurance plan, group and policy numbers in MyQuest.

ICE: In Case of Emergency Save all your emergency information for instant access when needed. For the patient’s protection, MyQuest requires a password to access health information.

Share health information Email or fax to medical providers, family or emergency contacts directly from the MyQuest mobile app.

Access child’s results Must provide legal documentation that verifies authority as child’s representative.

Patient Payment OptionsInvest in answers for better healthPast Due BalanceA past due balance must be settled before new services can be provided at a Patient Service Center.If you have a past due balance from a previous service, you will be required to make a payment before you can receive future services.

• For your convenience, we accept all 4 major credit cards, debit cards, health savings cards, checks, or money orders.

Easy PayA simple way for you to pay for your portion of lab testing during your visit.Don’t worry about writing a check and mailing payments. We make it easy and convenient so you don’t receive a bill later.

• Simply present your credit card or health savings card during your visit. We accept all 4 major credit cards, debit cards, and health savings cards.

• You will approve a maximum charge to your card, which will only be made if a balance remains after your insurance provider processes your claim.

• If the balance is more than you authorized at the time of service, we will send you a bill for the remaining lab fees.

• DLO will notify you by email when your credit card has been charged if your email address has been provided.

Uninsured Patient PricingFull payment is due at time of service.Payment is required in full at time of service for uninsured patients at Patient Service Centers.

• If you are uninsured, you may receive a discount of 40%-50% off standard prices on most testing through the Quest Diagnostics Uninsured Patient Pricing (UPP) program.

• The UPP program is only available through participating doctors. To learn more about UPP, discuss with your doctor.

• You cannot sign up for UPP at a Patient Service Center.

• We accept all 4 major credit cards, debit cards, checks, or money orders.

Managing Expenses through DLO DirectTM, patient direct access testingDLO believes patients should have the most accurate and up-to-date information needed to pro-actively manage personal health. DLO Direct™, launched in July 2016, is a new program that gives patients the ability to order select lab tests without orders from a health care provider.

• DLO Direct offers a variety of confidential tests, including general wellness, drug screening , infectious disease, and many more.

• Cost of testing through DLO Direct can be less expensive than insurance co-pays and/or coinsurance.

• Payment is due at time of testing.

• We accept all 4 major credit cards, debit cards, checks, or money orders.

• Patient’s can share test results, delivered through MyQuest.com, with their health care provider for interpretation and consultation

Personal health management is most effective when it involves a partnership with a health care provider. DLO encourages patients to share test results with a personal health care provider. Only a health care provider can interpret laboratory testing results and provide an accurate picture of a patient’s health status.
