What is Patient Centered

Medical Home?

Patient Centered Medical Home (PCMH)

allows your child to have a direct relationship

with their Primary Care Manager (PCM).

PCMH also ensures that Patient Centered

Care is comprehensive, coordinated, all

inclusive and integrated with all other care

provided within our medical center and

healthcare system.

Naval Medical Center Pediatrics is the

central resource for all of your child's primary

healthcare needs. Your child's PCM, Team

Nurses, and staff are your inter-professional

healthcare team, committed to partnering

with you to provide optimal individualized

care. We will assist you with information and

resources in all facets of your healthcare

including specialists, acute care, chronic

care, and health screenings.

TRICARE Online Patient Portal is the

current tool used to communicate with you

through secure messaging.
