Winter 2017 Heart Update Pediatric Memphis, Tennessee Heart Institute earns national honors T welve-year-old Maggie Golden was always cold. She spent much of her day in a hoodie, sitting under a blanket. And then, the other symptoms: she grew so lethargic that she often needed to take a nap in the middle of the day, and her thigh and calf muscles frequently hurt. Standard blood tests looked normal, but as symptoms grew worse, her parents persisted. On her next visit to the pediatrician, her heart rate measured 50 beats per minute. A subsequent EKG showed a blockage and an echocardiogram gave them a rare and serious diagnosis: Ebstein’s anomaly, a condition where the heart’s tricuspid valve isn’t formed properly. That’s when Maggie’s mother, Heather Corporon, updated her Facebook status, sharing that Maggie was having heart problems and asking for prayers. Hours later, a friend messaged her about a talented surgeon in Memphis, Christopher Knott-Craig, MD, — a pioneer in Ebstein’s treatment with outcomes that surpassed the national average. A couple of days later, the Corporons sent Maggie’s records to Knott-Craig, who immediately called. “He called me and basically said: ‘We need to do surgery,’” Will Corporon said. “We have no doubt that if we wouldn’t have been led to him, very sadly, one of these mornings we would have gone up to wake her up and found her dead.” Instead, Maggie is gaining energy and is now back in school. Heart Institute earns national honors

T welve-year-old Maggie Golden was always cold.

She spent much of her day in a hoodie, sitting

under a blanket. And then, the other symptoms:

she grew so lethargic that she often needed to take a nap

in the middle of the day, and her thigh and calf muscles

frequently hurt.

Standard blood tests looked normal, but as

symptoms grew worse, her parents persisted. On her

next visit to the pediatrician, her heart rate measured

50 beats per minute.

A subsequent EKG showed a blockage and an

echocardiogram gave them a rare and serious diagnosis:

Ebstein’s anomaly, a condition where the heart’s tricuspid

valve isn’t formed properly.

That’s when Maggie’s mother, Heather Corporon,

updated her Facebook status, sharing that Maggie was

having heart problems and asking for prayers. Hours

later, a friend messaged her about a talented surgeon

in Memphis, Christopher Knott-Craig, MD, — a pioneer

in Ebstein’s treatment with outcomes that

surpassed the national average. A couple of

days later, the Corporons sent Maggie’s records

to Knott-Craig, who immediately called.

“He called me and basically said: ‘We need

to do surgery,’” Will Corporon said. “We have

no doubt that if we wouldn’t have been led

to him, very sadly, one of these mornings we

would have gone up to wake her up and found

her dead.”

Instead, Maggie is gaining energy

and is now back in school. She starts swim

practice later this month. Golden is one of hundreds of

patients to benefit from the Le Bonheur Heart Institute’s

surgical program – one of eight that earned the Society

of Thoracic Surgeons (STS) three-star rating the past

summer.

Two years after launching an ambitious plan

to expand Le Bonheur’s Heart Institute, Knott-Craig

and his Heart Institute Executive Co-Director Jeffrey

A. Towbin, MD, have expanded nine new programs

within the Institute, recruited 10 new faculty

members and launched a cardiac genetics research

program.

This month, the Heart Institute joined a

national clinical trial for a new device to close patent

ductus arteriosus (PDAs), in the catheterization lab

for neonates with extremely low birth weight. The

cardiac cath lab has been performing minimally

invasive procedures to close PDAs in premature

neonates as

“We know that we are building a program that is ready for

the changing needs of children and adults with congenital and

acquired heart conditions – and we’re focused on offering the

best possible care,” said Towbin, who also serves as Le Bonheur’s

chief of Cardiology.

The Heart Institute at Le Bonheur Children’s Hospital uses the combined expertise of an advanced pediatric cardiac team to provide specialized care for children with congenital heart disease, heart muscle disease (cardiomyopathies) and electrical disorders (arrhythmias). Pediatric cardiologists, pediatric cardiothoracic surgeons, cardiac and pediatric intensivists and anesthesiologists make up the Heart Institute. Advanced practice nurses, perfusionists, cardiac nurses, respiratory therapists and lab and imaging technicians are specially trained in pediatric cardiology care. Leaders of the Heart Institute include:

christopher Knott-craig, mD, executive co-director of the Heart Institute, medical director of cardiovascular surgery. Christopher Knott-Craig, MD, graduated from the University of Cape Town in South Africa and completed training in cardiac surgery at the Groote Schuur Hospital in South Africa. He is board certified by the South African Medical & Dental Council in cardiothoracic surgery. Knott-Craig also serves as

professor and chief of pediatric cardiac surgery at the University of Tennessee Health Science Center (UTHSC) School of Medicine. His areas of special focus include neonatal/infant cardiac surgery, Ebstein’s anomaly, Ross Procedure, minimally invasive valve surgery, cardiopulmonary bypass, ambulatory thoracic surgery, hyperhidrosis and pediatric congenital heart disease.

Jeffrey a. towbin, mD, executive co-director of the Heart Institute, medical director of Pediatric cardiology. Jeffrey Towbin, MD, earned his medical degree from the University of Cincinnati College of Medicine and completed his fellowship in pediatric cardiology at Texas Children’s Hospital/Baylor College of Medicine in Houston. He is board certified by the American Board of Pediatrics with a pediatric cardiology

subspecialty. Towbin serves as professor and chief of Pediatric Cardiology at UTHSC School of Medicine and vice chair of Strategic Advancement for Le Bonheur. He serves as chief of Cardiology for St. Jude Children’s Research Hospital and holds the St. Jude Chair of Pediatric Cardiology at Le Bonheur. He is also medical director of

the Cardiomyopathy/heart Failure/Heart Transplant Program at Le Bonheur. His areas of special focus include diagnostic and therapeutic advances for cardiomy-opathies, heart failure, heart transplantation and cardiovascular genetics.

alejandro arevalo, mD, medical director of Pediatric sports medicine. Alejandro Arevalo, MD, completed his pediatric residency at the University of Florida College of Medicine at Jacksonville and his pediatric cardiology fellowship at UTHSC. He is an assistant professor of pediatrics at UTHSC. His special interests include pediatric echocardiography and sports medicine.

umar Boston, mD, surgical director of Heart transplant and mechanical circulatory support, surgical director of adult congenital Heart Disease Program. Umar Boston, MD, received his medical training at Howard University School of Medicine and general surgery and cardiothoracic/thoracic surgical training at Mayo Clinic. He earned his pediatric cardiac surgical residency at Stollery Children’s Hospital at the

University of Alberta. His research focus includes advancing VAD support systems in children with end-stage heart failure, preservation techniques for increasing the pediatric donor heart pool and application of regenerative tissues to congenital heart surgery. At UTHSC, Boston has the academic rank of associate professor.

Thomas Fagan, MD, received his medical degree at St. Louis University School of Medicine and did his pediatric residence and chief residency at Cardinal Glennon Children’s Hospital and the St. Louis University School of Medicine before completing his pediatric cardiology and advanced catheterization and intervention fellowship training at Texas Children’s

director of Cardiac Interventional Catheterization, director of Catheterization Innovation and Research, director of Advanced Cardiac Interventional Fellowship and director of Pediatric Cardiac Catheterization Laboratory at the University of Colorado Denver School of Medicine, Denver – Children’s Hospital Colorado. He also serves as a professor at UTHSC.

Jason Johnson, mD, medical director of Pediatric cardiac magnetic resonance Imaging. Jason Johnson, MD, completed his pediatric residency, pediatric cardiology fellowship and advanced imaging fellowship with a focus on cardiac MRI at Duke University Medical Center. He is an assistant professor of pediatric for UTHSC. His special interests include pediatric echocar-diography and cardiac MRI, congenital heart disease,

cardiomyopathy and myocarditis.

Vijay Joshi, mD, medical director of Outpatient cardiology and non-Invasive cardiology. Vijay Joshi, MD, attended medical school at the University of Vermont and completed a family medicine and pediatrics residency at the University of California at San Diego and a pediatric cardiology fellowship at Children’s Hospital of Philadelphia. Board certified by the American Board of Pediatrics with a cardiology subspecialty, Joshi is also an

associate professor at UTHSC. His patient care emphasis is on general cardiology with focus on fetal cardiology, advanced echocardiography, and exercise testing.

michael rebolledo, mD, mPH, mBa, medical director of cardiovascular Quality Improvement. Michael Rebolledo, MD, MPH, MBA, completed fellowship training at the University of California Los Angeles and received master’s degrees in Business Administration and Public Health at the University of California at Irvine and Johns Hopkins University, respectively.

B. rush Waller, mD, medical director of Inpatient cardiology and adult congenital Heart Disease Program. B. Rush Waller, MD, received his medical degree from UTHSC and completed pediatric residency at Vanderbilt University Medical Center. He completed his fellowships in pediatric cardiology and apediatric interventional cardiology at the Medical University of South Carolina. Waller is an associate professor at UTHSC

and is board certified by the American Board of Pediatrics with a cardiology subspecialty and is also board certified in Adult Congenital Heart Disease medicine. His areas of focus include interventional pediatric cardiology, including therapeutic catheterizations for critically ill neonates, critically ill preoperative patients and complex cases of adults with congenital heart disease and transcatheter closure of intracardiac shunts.

glenn Wetzel, mD, PhD, medical director of Pediatric electrophysiology, director of Fellowship Program. Glenn Wetzel, MD, PhD, completed his pediatric residency and fellowship training in pediatric cardiology at the University of California at Los Angeles. He is board certified by the American Board of Pediatrics and is trained as an electrophysiologist. Wetzel is also a full professor at UTHSC. His special interests include pediatric electrophysi-

ology (arrhythmias), radiofrequency ablation and cryoablation, cardiomyopathy, pediatric pacemakers and ICDs (implantable cardioverter defibrillators).

In 2016, Le Bonheur added a Cardiology Genetic Research Laboratory aimed at discovering and investigating novel genes and gene mutations that contribute to cardiac disease such

as cardiomyopathy, heart failure and arrhythmia disorders. The lab is working to find the molecular basis for cardiac diseases and use these discoveries to develop novel, targeted therapies.

The Heart Institute, in conjunction with Le Bonheur’s Children’s Foundation Research Institute, is also conducting

trials aimed at advancing pediatric and adult congenital heart medicine. They include: • We are one of 10 clinical trial sites for a device used to close patent ductus arteriosus (PDA) in neonates in the catheterization lab. There, interventionists are closing PDA defects on babies smaller than two kilograms. • A global study for pediatric patients with heart defects resulting in a single ventricle, using a blood thinner approved by the FDA for adults. If the medication works, patients would benefit by not having to be treated by Coumadin/Warfarin, resulting in less blood monitoring and fewer

food restrictions, thereby improving their quality of life. • Studying transplant patients to learn more about the genetic and acquired aspects of heart defects. • Participating in a national study that uses Fitbit technology to record physical activity in patients with a condition that can potentially cause sudden cardiac death.

• Participating in two clinical drug trials for children with cardiomyopathies.• Studying biomarkers in children with dilated cardiomyopathy and hypertrophic cardiomyopathy.• Examining the effectiveness of lifestyle changes in obese children on the stiffness of their blood vessels.
