Pediatric Motor Disorders Program A team of specially trained Cleveland Clinic pediatric neurologists offer advanced diagnosis and treatment for motor disorders in children. The team also works together with physical therapists and physiatrists at the Cleveland Clinic Children’s Hospital for Rehabilitation to offer day treat-ment programs and long-term rehabilitation for neurologic injury or chronic neurologic conditions.

Neuromuscular DiseasesCleveland Clinic Pediatric Neurology specialists diagnose neuromuscular diseases, including muscular dystrophy, spinal muscular atrophy, congenital myopathies and myasthenia gravis, hereditary neuropathies and other polyneuropathies, using advanced techniques including specialized DNA tests, pediatric EMG and neuropathological examination of nerve and muscle biopsies. Affected children have access to the latest therapies including drug trials, plasmapher-esis and immunoglobulin infusion when indicated. The Pediatric Neurology team collaborates with other pediatric specialists in orthopaedics, cardiology, pulmonology, and sleep medicine to provide multidisciplinary care for children with these disorders. We also provide heart transplan-tation for appropriate candidates.

Movement DisordersCleveland Clinic is a nationally and internationally renowned center for the diagnosis and treatment of Tourette syndrome and other pediatric movement disorders including ataxia. Pediatric neurology move-ment disorders specialists offer traditional medical as well as innovative therapies in special circumstances, including botulinum toxin injections guided by electromy-ography and deep brain stimulation.

Spasticity and Cerebral PalsyCleveland Clinic pediatric neurologists offer specialized treatments to children with spasticity and cerebral palsy, including oral medications, physical therapy, orthopaedic interventions, botulinum toxin injections, intrathecal baclofen infusion, and selective dorsal rhizotomy. The Pediatric Neurology team also leads a multidisciplinary group of pediatric neurosurgeons, orthopaedists, gastroenterologists, and urologists to manage the diverse challenges faced by children with spina bifida and myelomeningocele.

Pediatric Headache ProgramCleveland Clinic is nationally and internationally renowned for its expertise in the diagnosis and treat-ment of all types of headache in children. Therapeutic options include the newest medications for acute treatment and chronic prevention, as well as emergency intravenous infusions for headache crises. Cleveland Clinic pediatric neurologists also work closely with pediatric psychologists to achieve the best outcomes for children with persistent chronic headache.

In cases of chronic pain that will not subside or that interfere with children’s school or parents’ work schedules, a pain rehabilitation program is available. Through the Cleveland Clinic Children’s Hospital for Rehabilitation, this two-week program is run by an interdisciplinary team of pediatric experts specializing in helping children function normally again. The goal is to help youngsters cope with their pain, and achieve an acceptable quality of life.

Pediatric Neurometabolic and Genetic Disorders Program Genetic and metabolic disorders are the underlying basis of many pediatric neurological and developmental conditions. A team of Cleveland Clinic pediatric neurologists and medical geneticists, each with special expertise and interest in neurometabolic and neurogenetic disorders, offers diagnosis and treatment of these complex diseases. Areas of particular focus include evaluation and treatment of children with central nervous system white matter disorders and underlying genetic and metabolic disorders in the setting of mental retardation and epilepsy. Metabolic pathways studied with particular interest and expertise include disturbances of mitochondrial oxidative phosphorylation and fatty acid oxidation, disorders of amino and organic acid metabolism, and lysosomal storage diseases. The diagnostic evaluation of affected children is critically dependent on highly specialized laboratory testing, and the team utilizes the finest clinical laboratories in the world, including some at Cleveland Clinic. Pediatric neurologists and medical geneticists work together with other pediatric subspecialists to provide multidisciplinary care for children with these highly complex disorders. Clinical research programs are in progress in several areas including autism and the development of new diagnostic assays.

Pediatric Cerebrovascular Disorders ProgramThe Cleveland Clinic Pediatric Neurology team is a member of the International Pediatric Stroke Study Consortium, a group of international child neurologists, hematologists and rheumatologists from more than six countries with a special expertise and interest in childhood stroke. The pediatric stroke team is also a founding member of the Ohio Pediatric Stroke Consortium which has established a pediatric stroke registry in Ohio. State-of-the-art imaging, including brain

brachial plexopathies, neonatal hypotonia, meningomyelocele, hydrocephalus, IVH and other problems. Together, Cleveland Clinic hospitals offer specialized services for over 9,600 births per year.

Pediatric Neuro-Oncology ProgramPediatric neurologists specializing in the treatment of brain and spinal cord cancer collaborate with pediatric neurosurgeons, pediatric hematologists oncologists, and other pediatric specialists in the Cleveland Clinic Brain Tumor & Neuro-Oncology Center. The team offers sophisticated radiothera-pies, including the Gamma Knife for brain tumors; computer-assisted imaging for precise surgical planning, navigation, and tumor resection; and the most advanced chemotherapy as part of national study protocols.

Neurocutaneous Disorders ProgramCleveland Clinic pediatric neurologists are part of a multidisciplinary team of pediatric oncologists, orthopaedists, plastic surgeons, and neurosur-geons, providing multidisciplinary care for a large population of children with neurocutaneous disorders such as neurofibromatosis, tuberous sclerosis, and Sturge-Weber syndrome.

Pediatric White Matter Disorders and Multiple Sclerosis Program Cleveland Clinic pediatric neurologists offer advanced diagnosis and treatment of multiple sclerosis and white matter disease in children. Together with colleagues at the Mellen Center for Multiple Sclerosis Treatment and Research, we offer state-of-the-art care for children with MS and other white matter disorders. Distinction between the many genetic metabolic and acquired forms of pediatric white matter disease is facilitated by collaboration with Pediatric Neurology team members who are world renowned specialists in the clinical and laboratory diagnosis of this group of disorders.

Pediatric Sleep Disorders ProgramThe Cleveland Clinic Sleep Disorders Center is one of a handful of centers in the country offering state-of-the-art consultation and testing for infants, children and adolescents with sleep disorders. Children with complex disorders are managed by board-certified pediatric sleep specialists, in collaboration with other pediatric subspecialists. Some of the common conditions that are evaluated include sleep-related breathing disorders, narco-lepsy, unusual nighttime events like sleepwalking and night-terrors. In addition, children with abnormalities of sleep-wake rhythms receive an in-depth assessment and counseling for lifestyle issues. Children needing evaluation in the sleep laboratory undergo testing in our state-of- the-art nine-bed sleep laboratory. In the laboratory, children undergo overnight polysomnography and daytime MSLT examination. A parent may sleep comfortably next to the child in rooms specially designed for children and families.

MRI, MRA, cerebral angiogram, and CT angio-gram, as well as other comprehensive diagnostic and therapeutic interventions are offered for all forms of neonatal and childhood stroke. Children with acute stroke are cared for in a state-of-the-art pediatric intensive care unit staffed full-time by experienced pediatric intensivists in conjunc-tion with our pediatric neurologists. Special areas of stroke interest include children with central nervous system vasculitis, stroke associated with cardiac disease or interventions, neonatal stroke, and the vasculopathy of neurofibromatosis.

Pediatric Neurocardiology ProgramCleveland Clinic is the leading cardiac care facility in the United States and our pediatric cardiolo-gists and cardiothoracic surgeons offer state-of-the-art care for children with a wide variety of congenital and acquired heart disease. Cleveland Clinic pediatric neurologists offer special expertise in the management of associated neurological conditions and complications of these disorders. They also provide a comprehensive neuromuscular and neurometabolic evaluation for children with unexplained cardiomyopathy or children undergo-ing heart transplantation. Children who have undergone cardiac surgery and circulatory bypass are monitored in the ICU when needed with continuous video EEG monitoring to enhance detection of postsurgical seizures.

Neonatal Neurology ProgramCleveland Clinic pediatric neurologists offer prenatal consultation for a wide range of neuro-logical disorders detected in utero at our Fetal Care Center by fetal ultrasound, magnetic resonance imaging or amniocentesis. They also offer pediatric neurology consultation and treatment for a variety of newborn conditions including hypoxic-ischemic encephalopathy, neonatal seizures, stroke, CNS malformations,

Cleveland Clinic’s Pediatric Epilepsy Program, one of the leading

pediatric epilepsy centers in the world, is expanding to accommodate

an ever-increasing patient population.

Each year, our team sees more than 2,000 children with epilepsy in outpatient clinics and evaluates more than 400 children in our dedicated Pediatric Epilepsy Monitoring Unit. Our pediatric epilepsy team also includes several board-certified pediatric physicians in neurology, neurosurgery, neuroradiology and neuropsychology. Facilities include a self-contained, eight-bed Pediatric Epilepsy Monitoring Unit located in Children’s Hospital at Cleveland Clinic, featuring state-of-the-art, all-digital, video-EEG equipment. The Pediatric Epilepsy Unit operates 24 hours-a-day, seven days a week. The unit is staffed by a team of dedicated nurses and EEG technicians specializing in pediatric epilepsy. Children with complex metabolic, genetic or other medical disorders have access to pediatric subspecialists in Children’s Hospital. Access to video-EEG monitoring is also available in our pediatric intensive care and neonatal ICU units.

Other special services for children with epilepsy include sophisticated neuroimaging with advanced magnetic resonance imaging (MRI) using specialized protocols for epilepsy, magnetic resonance spectroscopy (MRS), ictal single-photon-emission computed tomography (SPECT) and positron emission tomography (PET). Advanced techniques such as functional MRI (fMRI) enable noninvasive mapping of language and motor function, and delineating white-matter tracts using diffusion tensor imaging (DTI) are also available. State-of-the-art software and analysis allows co-registration of data from multiple tests, enabling our epileptologists and neurosurgeon to better localize the seizure focus within the brain. Cleveland Clinic pioneered the use of invasive EEG electrode recordings from the brain. Subdural and depth electrode recordings are used when indicated to localize seizure onset.

Epilepsy

The Cleveland Clinic Pediatric Epilepsy Program has extensive experience in performing epilepsy surgery in children of all ages, from infancy to adolescence who have severe, medically refractory epilepsy. Each patient considered for surgery is carefully evaluated first in the epilepsy monitoring unit and then discussed individually in the multi-disciplinary Patient Management Conference. Each year, more than 80 surgical procedures, including lobectomies and hemispherectomies, are performed at our center. In the postoperative period, children receive state-of-the-art care in our Pediatric Intensive Care Unit. The collective experience and technical expertise of our epilepsy team has yielded some of the best seizure-free outcomes worldwide. Children who are not candidates for resective surgery may benefit from an implanted device, the vagus nerve stimulator. Our epileptologists have considerable clinical and research experience in the use of VNS.

Also available are several clinical antiepileptic drug trials, which offer access to investigational, new drugs. Our team understands the special needs of children with epilepsy and their families. Requests for consultation from patients and physicians are reviewed carefully by an epilepsy nurse to assess what tests and consultations will be needed. These can be scheduled in

• 80 epilepsy surgeries performed annually in children

advance, so that following an outpatient consultation with the epileptologist, the child is admitted to the Pediatric Epilepsy Monitoring Unit and undergoes all necessary diagnostic tests and consultations. Patients referred for epilepsy surgery will be discussed in Patient Management Conference and may undergo epilepsy surgery soon thereafter. Six months after epilepsy surgery, all patients return for an outpatient evaluation.

A multidisciplinary, comprehensive cognitive and behavioral program brings together epileptolo-gists, psychiatrists, psychologists, social workers and rehabilitation specialists to provide psychoso-cial assistance to patients with seizures at various stages of evaluation and treatment.

The Pediatric Epilepsy Program at Cleveland Clinic also has extensive clinical and research experience with the Ketogenic Diet. A dedicated epilepsy nurse and dietician offer this treatment for carefully selected patients, under the care of the pediatric epileptologist. Families receive education and training before, during and after initiation of the Ketogenic Diet, which requires a short hospitalization. In selected cases, the results can be very gratifying.

Pediatric and Congenital Neurosurgery is devoted to the comprehen-

sive and most advanced surgical treatment of children with neural

disorders and adults with congenital disorders. Our staff is recognized

internationally for advanced treatment in several areas of general

pediatric neurosurgery. Areas treated include epilepsy, pediatric brain

tumors, cerebral palsy, cranial anomalies, headache, hydrocephalus,

neonatal hemorrhages and spinal anomalies. Congenital disorders

such as hydrocephalus, Chiari malformation and tethered cord are

treated in all patients regardless of age.

The Cleveland Clinic Epilepsy Center has more than 24 years experience in treating surgical epilepsy and performing epilepsy surgery. Our outcomes are among the best in the country for a variety of epileptic syndromes. The focus of the pediatric epilepsy surgery program is to diagnose, investigate and treat surgical epilepsy. The program boasts an international referral base, large clinical volume, specialized expertise and superb results. In the last five years, pediatric epilepsy surgery cases have increased significantly. We perform more than 80 surgeries on pediatric epilepsy patients annually.

We work in close collaboration with other pediatric disciplines to form multidisciplinary care around children and adults with craniofacial disorders, myelomengiceles, cerebral palsy and hydrocephalus. Pioneering work in the diagnosis of chronic hydrocephalus for children and adults have led to advanced protocols identifying patients who improve with CSF shunting. Various

forms of treatment and technologies such as neuroendos-copy, intrathecal drug pump systems, and adjustable shunt systems are being employed or under development.

A multidisciplinary team including a full pediatric staff of oncologists, neurologists, neuroradiologists, nurses and social workers collaborate with our neurosurgeons in the Pediatric Brain Tumor Program. Surgery represents the first line of treatment for pediatric brain tumors: More than 80 percent can now be removed. The neurosurgery and pediatric oncology groups are supported by an excellent Pediatric Intensive Care Unit at Children’s Hospital. Sophisticated nonsurgical therapies, including the Novalis system, Gamma Knife radiosurgery, Intra-beam intraoperative radiotherapy, 3-D conformal radiation therapy and brachytherapy, are now mainstays of ongoing treatments of childhood brain tumors.

This section has a wide referral base and large experience in treating congenital anomalies involving the spinal cord and base of the skull. Pediatric and congenital neurosur-geons here recently developed an endoscopic-assisted neurosurgical procedure for the treatment of Chiari malformation, a congenital anomaly characterized by a downward displaced herniation of the cerebellar tonsils into the upper portion of the spinal canal.
