Oslers Newest Hospital Site Peel Memorial Centre for Integrated Health and Wellness

ANNWe are only months away from substantial completion, which is the date Osler gets the keys to the building from the buildersThe building is nearly finished and its spectacularWeve all acknowledge was an awesome and rare opportunity we have before us and all of you have embraced this energy throughout this project The purpose of this part of the presentation is to recap the process and work that has been done during Phase 1 & 2 of this project

Auditorium for education exercise classes, etc.Seniors wellness and rehabilitation clinics. - Community Capacity planning within the CW LHIN shows more services will be needed. Linkages to CCAC and other community partners.

Phase 1 (current project) focuses on the technological hub and ambulatory care services.

Phase 2 (future) primarily will comprise of inpatient Complex Continuing Care, Rehab beds. Phase 2 is currently scheduled to be at least two years after Phase 1.

2024/25 (future) Growth allowed for an additional inpatient component along with assorted programs and services.5

The growing need for mental health and addictions services will be supported using an interdisciplinary team with a focus on children and youth, adults and seniors.

Services for seniors will be delivered through collaboration with a network of health providers.

These include: Specialized geriatric and preventative clinics (e.g. Falls) Rehab services for seniors with complex medical, psychosocial and functional issues Support for prompt access to outreach and outpatient services to help seniors maintain independence

Facility has been designed to be easily accessible, and includes a bright courtyard that offers a supportive rehab environment for seniors

At Peel Memorial, patients will have access to their specialists and have their day surgery all under one roof.

The challenge that we originally posed to ourselves was to design an integrated chronic disease management model of care at PMC that will help to improve the health of patients through effective secondary prevention

New positions will first be posted internally and priority will be given to qualified internal candidates. Planning is also currently underway for staff and physician orientation so folks that are moving are ready in their new surroundings at Peel.20
