• Dana Kematian Diberikan kepada :

• 1.Karyawan sebesar Rp 200.000,-

• 2.Suami/Istri sebesar Rp 150.000,-

• 3.Anak kandung sebesar Rp 150.000,-

• 4.OrangTua/Mertua sebesar Rp 150.000,-

Surat Keterangan dari RT / RW dan Kelurahan setempat yang dapat dipertanggung jawabkan.

• Surat Kematian dari Kelurahan Setempat yang dapat dipertanggung jawabkan.

• Surat keterangan dari Rumah Sakit apabila dirawat dirumah sakit.


Sumbangan dana Musibah/sakit

Surat keterangan Sakit dari Dokter yg ditunjuk Perusahaan. 1.Dr Lukas H. 2.Dr Dewi Syarif. 3. Dokter RS MALMinimal 3 hari.

• Kondisi sakit dan dirawat di Rumah Sakit

minimal 3 hari.

• Mengalami kecelakaan yang mengakibatkan sakit/cedera serta membutuhkan biaya pengobatan minimal Rp 400.000,- ( dengan kwitansi resmi )

● Dana Musibah diberikan kepada :1.Karyawan sebesar Rp 100.000,-2.Suami/Istri sebesar Rp 100.000,-3.Anak kandung Rp 100.000,-

4. Tenggang waktu menerima dana minimal 4 bulan.

5. Komunikasi internal Pengurus,penyiapan dana,serah terima,penandatanganan Kwitansi.

1. Aktivitas Pengumpulan Dana Lancar

2. Informasi formal dari yg

bersangkutan ke Pengurus

3. Penyampaian kelengkapan data /dokumen ke Penguraus.

Apabila terjadi defisit ketersediaan dana,pengurus mengadakan Rapat Darurat serta menginformasikan hasil rapat pada seluruh Anggota.
