The wrist is placed supine over a towel roll and extended in order to assist in translation of the trapezium dorsal to the path of the guide-wire.

After the initial guide-wire is satisfactorily placed under fluoroscopic guidance, a second guide-wire is placed across the fracture site to prevent rotation of the fracture fragments during
tapping and screw placement.
