Once tempted to drop out of university, an Adelaide dentist has not only become a specialist in periodontics—he’s also improved his business skills to purchase, expand and renovate his practices.

28 BEST PRACTICE

Growing

For Dr Steven Soukoulis, the phrase, ‘The stars and moons aligned’, has pretty much summed up his business strategies for nearly a decade now. He has been running one of Adelaide’s leading periodontic practices since 2006 and its success is partly due to the dentist’s relationship with two professional colleagues.

The first is Dr Bryon Kardachi, an Australian pioneer in the field who opened his practice on Greenhill Road, just south of the Adelaide CBD, in the late 1970s. The second is South Australian-based Jeremy Stagg, a medical finance expert who is part of BOQ Specialist.

Dr Kardachi employed Dr Soukoulis in 2001 and a few years later encouraged the 34-year-old dentist to take over the reins of the business. BOQ Specialist's Jeremy Stagg made that possible in 2006 along with an immediate move to new premises. And before too long, Dr Soukoulis purchased the premises.

“We couldn’t have done it without Jeremy’s assistance,” Dr Soukoulis says. “He’s always been there at the right time with the right advice and a good understanding of what we want to do and why. And he’s a really nice guy. He knows that everyone’s busy and he gets everything done quickly and efficiently.”

Now aged 41, married to dental nurse Alana and raising daughters Adela, aged four, and two-year-old Eliza, Dr Soukoulis has seen Greenhill Periodontics & Implants grow into one of Adelaide’s two largest

7.30am Registration & Exhibition Open

8.00am Welcome – Scientific Chairman

Professor Laurence Walsh BDSc, PhD, DDSc, Gced, FRAcDS, FFOP(RcPA)

On behalf of Henry Schein Halas and Quintessence, it is a great honour for me to welcome you to the 2016 International Quintessence Symposium. As in past events, the 2016 program has been designed to benefit general dental practitioners as well as dental specialists, bringing you into contact with the most up-to-date and clinically relevant evidence-based information in restorative dentistry and periodontics through lectures and themed workshops. the program has broad appeal and will enliven all who attend with practical information and with a passion for dental excellence.

8.30am The Four Pillars of Aesthetics: Form, Function, Colour & Imagination

Dr Douglas A. terry DDS

this one-hour presentation will encompass many facets of aesthetic and restorative dentistry including biomaterials, laboratory technology, operative techniques and principles, functional composite prototyping, periodontal plastic surgical procedures and prosthetic guidelines for treatment of interdisciplinary cases, drawing on the new textbook, Esthetic and Restorative Dentistry: Material Selection and Technique. the presentation will discuss the basic building blocks that are so important for achieving restorative and perio-aesthetic excellence. the significance of form, function, colour and imagination applies to all aspects of restorative, prosthetic, and perio-aesthetic dentistry in developing natural aesthetics. Form, function and colour are the pillars for consistently and predictably developing an optimal biological balance between the restoration and the soft tissue. Although knowledge is essential, imagination is more important because it is limitless and has no boundaries to where we can go as a restorative team. utilizing the imagination of the interdisciplinary team allows us to move to new limits. this thinking process will be illustrated through clinical case presentations of utilizing diagnosis and treatment planning with different restorative materials and techniques including ceramic and composite materials.

9.30am Think Pink: A Periodontist’s Approach to Restorative Treatment

Dr Brian e. James BDSc, MDSc, FRAcDS, FPFA

there is an increasing awareness of the importance of a sound foundation for effective and aesthetic long term restorative treatment. the position, quality and volume of gingival tissue combined with tooth contour, restorative margin position and quality, and cleansability can influence restoration survival. this lecture will look at aesthetic recontouring, gingival grafting, and tooth modification to reduce/eliminate local predisposing factors.

10.30am Morning Refreshments Break

11.00am So You Think You Can Cure a Restoration in 5 Seconds?

Dr Richard Price BDS, DDS, MS, FRcD(c), PhD

Almost every dental office will have at least one curing light, and using this curing light probably helps to generate around half of the income for a restorative-focussed dental office. Recent developments in curing lights have produced some lights that claim to adequately cure composite resin restorations in as little as 1 second. However, the evidence that today’s composite resins can be adequately light cured in even 5 seconds is lacking. the presentation will review light curing technologies, and discuss if you can adequately and safely cure today’s composites in 5 seconds.

12.00pm Posterior Composite Restorations: Strategies for Improving Success

Professor Ian Meyers BDSc, FIcD, FRAcDS, FADI, FPFA

In current dental practice, many patients request the use of direct tooth coloured restorations to restore their posterior teeth. In many instances this provides a challenge to the operator when the restorations are large, if access and isolation is difficult, or if the patient has high aesthetic demands. this presentation will look at techniques for restoring posterior teeth with composite resin, the selection and use of various materials, and how to manage restorations from small interproximal restorations to larger more complex restorations to achieve predicable aesthetic and functional outcomes.

1.00pm Q & A Panel with the Morning Presenters

1.30pm Lunch

2.30pm Bright Ideas, Materials & Innovations: A Critical Look at Present Day Procedural Changes that have altered the Way in Which We Care for our Patients

Dr Sonia Leziy, DDS, Dipl Perio, FcDS(Bc), FRcD(c) and

Dr Brahm Miller, DDS, Dipl Pros, FcDS(Bc), FRcD(c)

clinicians constantly strive to improve the quality of the dentistry that we provide for our patients. this presentation will explore how being an ‘early adapter’ can be positive in shaping skills and treatment planning solutions. It will also emphasize the importance of critically assessing and deciphering what is trendy versus what will positively change the treatment outcome.

this presentation will look at less invasive, simpler and more predictable care. We will present our views on immediate implant placement strategies, working in variable gingival biotypes, dermal grafting pros/cons, customized healing abutments to guide soft tissues and protect bone, features of the ideal implant provisional restoration, technical issues such as anatomic abutment design and different scenarios for the posterior hybrid (screw-retained/cemented) crown. An emphasis will be placed on implementing a digital work flow for the entire interdisciplinary team, including digital scanning from a surgical team members’ perspective, recognizing the benefits of implant software programs coupled with cBct technology to plan, design and perform more accurate surgical treatments for bone augmentation and implant dentistry. Our discussion will look at how the digital work-flow profoundly influences the communication between the surgical, restorative and technical team members, yet challenges them in the learning process.

Objectives: upon completion of this session attendees should be able to:

• Acquire a number of surgical and restorative implant ‘pearls’ that can be incorporated into everyday practice.

• understand the role of the tissue biotype to determine if grafting is essential in every circumstance.

• Feel more comfortable with immediate implant placement even in molar areas.

• Develop different styles for implant provisionalization.

• Be involved in the design of implant abutments and evaluate good, better and best designs.

• Reduce the number of treatment steps and feel more confident with a digital workflow from treatment planning, surgery through to the final restoration.

• experience throughout the discussion where things have gone wrong.

4.30pm Afternoon Refreshments Break

4.45pm Fundamentals of Implant Prosthodontics: Current Status of Immediate Loading, Early Loading and Delayed Loading

Professor John Beumer DDS, MS

Since the 1970’s, osseointegrated dental implants have offered an alternative to conventional fixed or removable prostheses. However, the original treatment protocols utilizing machine surface implants required the patient to wait for several months after implant placement before a prosthesis could be fabricated while the implants became osseointegrated. In most instances the patients were required to use removable prostheses during this period. During the last twenty-five years, various immediate and early loading protocols have been proposed as implant macro shapes and as implant surface textures have evolved. Recent advances in cAD-cAM technologies have applied an additional stimulus to this trend. the purpose of this presentation is to offer guidelines regarding the various loading protocols currently in use, namely immediate, early and delayed (conventional) loading. the immediate load prosthesis is a complex, technically demanding treatment, and should be attempted only after the implant team has acquired the necessary experience. Mistakes in clinical judgment and execution can lead to implant failure and loss of the prosthesis.

7.30am Exhibition Open

8.20am Introduction

Professor Laurence Walsh BDSc, PhD, DDSc, Gced, FRAcDS, FFOP(RcPA)

the program today follows two strands, one focussed on the challenges of restoration and rehabilitation in patients of increasing age, and the other on adhesive dentistry.

8.30am Cervical Lesions: Guidelines for When to Graft and When to Restore

Dr edward P. Allen DDS, PhD

Determining proper treatment of cervical lesions can be perplexing, especially when the lesion is associated with gingival recession. When do you restore, when do you graft, when should you do both? If you plan to restore and graft, how do you know how far to extend the restoration? this lecture will present guidelines for determining how to treat three types of cervical lesions: 1) non-carious, 2) carious, and 3) previously restored lesions. A minimally invasive surgical technique for correction of gingival recession associated with cervical lesions will be shown.

9.30am The All-on-4: Expanded Applications with Complicated & Compromised Patients

Dr Louie Al-Faraje DDS

the concept of tilting implants to create an improved spread and enhance base stability for full arch implant restorations has allowed treatment opportunities for a broader segment of patients than previously thought possible. enhanced capabilities for planning implant placement using cBct technology along with implants now designed with greater initial stability make this approach in immediate-load situations even more viable. this is especially important for complicated patient presentations that may have been previously deemed highly compromised.

10.30am Morning Refreshments Break

11.00am How to Avoid and Manage Peri-implant Diseases

Professor Frank Schwarz Dr. med. dent. habil.

the management of infectious diseases affecting osseointegrated implants in function has become a demanding issue in implant dentistry. While nonsurgical therapy was effective in the treatment of mucositis lesions, for peri-implantitis, mechanical debridement alone has shown limited efficacy. Adjunctive measures (e.g. local antibiotics/ antiseptics, laser application) may be effective in arresting disease progression at initial sites, however, moderate to advanced peri-implantitis lesions commonly require a more demanding surgical intervention or even removal of the implant. Accordingly, the treatment of early clinical signs of peri-implant mucositis may be considered a key strategy for the primary prevention of peri-implantitis. this presentation will identify the major risk indicators for peri-implant diseases, explore the limitations of nonsurgical treatment procedures, and discuss the selection of appropriate surgical procedures for the management of peri-implantitis.

12.00pm Titanium Welding: Laboratory and Intra-Oral Applications

Dr Scott Davis BDS, MDSc, FRAcDS, MRAcDS (Pros), FPFA, FIcD

the use of immediate loading of dental implants has increased in recent years, however, the process is not without complications. Polymer based provisional prosthetics may fracture, which can result in catastrophic failure of the implants without early intervention. titanium reinforcing bars can be welded not only in the laboratory but intra-orally to titanium cylinders to reinforce the provisional prosthesis and protect the implants. Other applications include confirmation indexes and long term low cost provisional bridges. the theory and practice of intra-oral welding will be presented with clinical examples.

1.00pm Q & A Panel with the Morning Presenters

1.30pm Lunch

2.30pm Digital Dentistry: Future or Reality in Restorative Dentistry?

Professor Petra Guess DDS

Novel techniques based on computer aided design-manufacturing cAD cAM introduce an innovative concept for teeth and implants with a more cost and time effective and predictable treatment outcome. cAD/cAM technologies and digital impressions are increasingly used in prosthetic dentistry. Digital workflows result in precisely fitting highly aesthetic restorations and can be successfully implemented in daily practice. examples of clinical cases using chairside and labside workflows will be presented. comprehensive applications including face scans and virtural articulators will be demonstrated.

3.30pm Dental Implant Surgery, Setting the Patient Up for Success or Failure?

Dr Steven Soukoulis BDS, MDS, Dip clin Dent

Dental implants are an integral treatment modality in the current dental practice. Many practitioners now could not imagine their practice without the use of one of the most amazing and life changing biomedical developments (both for dentists and patients) the world has seen in the last 30 - 40 years.

Along with the success of dental implants has come a realisation that complications do occur and this does not always relate to patient and implant based factors.

ADHESION MEETS PROSTHODONTICS

9.00am Bonding Capacity of Luting Agents to Alloys and Ceramics in Adhesive Prosthodontics

Professor Matthias Kern Dr. med. dent. habil., FADM

Adhesive bonding methods changed the world of restorative dentistry in allowing minimal and highly aesthetic invasive restorations. this lecture will summarize the bonding methods available today when bonding metal or all-ceramic restorations. every bonding method consists of at least three basic steps: 1. cleaning of the bonding surface, 2. Surface roughening and increasing the wettability, and 3. creating a chemical bond via adhesive monomers. the important aspects of each of these steps will be explored. In addition, clinical applications and promising results with resin-bonded minimal invasive restorations will be presented.

ADHESION MEETS IMPLANTOLOGY

2.00pm Materials and CAD/CAM for Implant Restorations: Functional and Biologic Interfaces

Professor Markus B. Blatz DMD, PhD

Adhesion in dentistry goes far beyond “bonding to teeth”. It plays a significant role in implant prosthodontics, where functional and biologic interfaces directly influence the clinical outcomes and long-term success. Along with the increasing use of dental implants, a plethora of new materials and laboratory-based cAD/cAM systems are entering the market to enhance aesthetic results, functional longevity, and precision in a digital workflow. Selecting the proper materials and techniques based on patients’ individual needs has become increasingly challenging, especially for implant-supported restorations because greater aesthetics are often associated with poorer physical properties. An understanding of fundamental material properties and their behaviour in the oral cavity is key for success and, in combination with adhesive technologies, offers new treatment options and possibilities to serve our patients in the best possible manner. Based on the current scientific evidence, this presentation will discuss a variety of adhesive interfaces in implant dentistry from functional, aesthetic, and biologic aspects and provide guidelines for material selection and new technologies for ultimate clinical success.

ADHESION MEETS PRACTICE

3.15pm Bio-Emulated Bonded Posterior Indirect Restorations: Simple Questions, Clinical Answers

Dr David Gerdolle DDS, MS

Adhesive dentistry has fundamentally altered and literally transformed dentistry as we move towards restorations that are significantly less invasive, longer-lasting and more aesthetic. Nature is the ideal model to emulate. the basic science for dentally emulating nature is found in the histo-anatomic structure of teeth. For bonded indirect posterior restorations, fundamental concepts such as Immediate Dentin Sealing and the Deep Margin elevation have become well established. Nevertheless, the clinical handling of materials remains demanding and full of traps that can lead to frustration in everyday practice. this presentation focusses on bonded indirect posterior restorations, for restoring carious and broken down posterior teeth. the clinical aspects of preparation and cementation procedures will be presented and discussed, with the emphasis on making them safe and reliable.

Minimal Invasive Anterior (and Posterior) Single Tooth Replacement

Professor Matthias Kern Dr. med. dent. habil., FADM

Resin-bonded fixed dental prostheses (RBFDPs, so-called Maryland bridges) with two metal retainer wings have been introduced over 30 years ago for a minimal invasive replacement of missing teeth. However unfortunately due to various reasons metal-ceramic RBFDPs are not considered a comparable treatment modality to implants by most dentists.

cantilevered RBFDPs with a single ceramic retainer wing were introduced 20 years ago. they provide better aesthetics and are less invasive than two-retainer RBFDPs. In the meantime, long-term data of this minimally invasive treatment option is available. Similar concepts are now also implemented for the replacement of posterior teeth: premolars often can be replaced with resin-bonded cantilevers while of molar replacement with modified inlay-retainers show a promising medium-term outcome.

However, it is a prerequisite that the dentist has a good understanding about indications, materials properties and the clinical procedures including specific bonding techniques. this workshop covers in depth the essential knowledge on the successful clinical application of minimally invasive all-ceramic RBFDPs which in many cases presents an excellent and less costly treatment alternative to single implants or conventional FDPs.
