President’s Message ……………..ILSPAN Benefits Kelly Logli, BSN, RN, CPAN I hope that you all had a great summer. ILSPAN has just had an- other outstanding fall conference for our nurses in St. Louis. This confer- ence had many excellent educational topics and allowed perianesthesia nurses to network with one another. On the heels of a great conference, I wanted to take a moment to remind our members of all that ILSPAN has to offer. EDUCATION BENEFITS The benefits of ILSPAN have been said before, but then good things are always worth repeating. ILSPAN provides two educational seminars each year, one in spring and one in fall. In addition to these seminars, ILSPAN frequently hosts ASPAN seminars throughout the state. These are great opportunities to learn or refresh your perianesthe- sia knowledge. ILSPAN has three District Directors to take care of the three regions in the state. New ideas for conference topics are always wel- come. I encourage everyone to take a moment and contact the director in your area with ideas for conference topics. By doing this we can better serve your needs. 20 contact hours necessary for re-newing an Illinois nursing license. I attended the ASPAN Com-ponent Development Institute in September. It was a very informa-tive and exciting conference. Top-ics covered finances, attracting and keeping members, certifica-tion, and on-line meeting and sur-vey applications. I am ready to get to work in expanding member-ships in ILSPAN and working to-ward developing programs geared toward what our members want. Please help by e-mailing me ideas you would like to see at our confer-ences and at ASPAN seminars. With that in mind, I am again asking all members to consider becoming a member of the IL-SPAN Board of Directors. We have positions opening up next year for Vice-President/President Elect, Secretary, and District 1 and Dis-trict 3 Directors. If you are inter-ested, know someone who might be, or would like to nominate someone for a position because you think they would be good, fill out the Willingness to Serve form and e-mail/mail it to me at the address

on the form in this issue of Peri-Scope (see page 13). If you are nominating someone, e-mail me their name (contact information on page 2) and I will get in contact with them. Congratulations to every-one who passed the CPAN/CAPA exam! (see page 7)I know all of you studied hard and are more knowledgeable since taking the test. In 2009 in addition to a testing date at ASPAN National Conference these tests will be available as computer based tests with a choice of sites around the coun-try. Anyone who is interested in taking the CPAN/CAPA exam, visit the ABPANC web-site at www.cpancapa.org for more information.

Summer has gone, fall is upon us and winter is almost here. I was glad to see many of you joined us at the ILSPAN Fall Conference in St. Louis. I think it was a success. The topics pre-sented will be a definite aid in giving the excellent nursing care your patients have come to expect from all of you. If you missed this conference, there will be another in March 2009. The ILSPAN con-ferences also give you contact hours and are inexpensive com-pared to other conferences you can attend. These are some of your ILSPAN/ASPAN membership benefits. I hope to meet more of you at the spring conference and in the future. ILSPAN is scheduling AS-PAN seminars for next year in the various regions of the state. Look on the ILSPAN website for infor-mation about one coming to your district. These seminars are very informative covering topics rela-tive to perianesthesia practice. The seminars are worth 7.25 con-tact hours, so you will be more than 1/3 of the way to getting the

Governmental Affairs Beth Rieches, BSN, RN, CPAN As I reviewed the Illinois General Assembly web site in search of active legislation regard-ing nursing, again most of what I found had to do with the Nurse Practice Act and some of the verbi-age changes associated with it.

When Kelly Logli and I attended the ASPAN Representa-tive Assembly at the last National Conference we were very surprised to learn that Illinois is the only state that does not allow, by law, visitation in the Phase I PACU. When we returned home Kelly and I contacted our State Representa-

tive to bring this issue to his at-tention. We were asked to get in touch with the Illinois Hospital Association (IHA). The IHA was notified and I was told that they would begin looking into this issue for us. My contact at the IHA stated that this process would take quite some time. I had been hoping to have something to re-port for this newsletter, but as of yet I have not heard anything. As we get more information from the IHA I will keep you updated. As you know this is an elec-tion year. Please exercise your

vote, you count! The Presiden-tial elections increase the turn out at the poles. In actuality your voice has more impact on the state and local level. So, while the choice of the next President is extremely impor-tant, please take the time to learn about your state and lo-cal candidates so that you can make an informed decision. Your voice matters, use the power of your vote.

The ILSPAN Recruiter Award is given to the ILSPAN member who recruits the most members during the ASPAN Member-Get-A-Member Campaign. The campaign lasts from April 1, 2008 to March 31, 2009. A minimum of 4 members need to be recruited to be considered for the award which is one year paid dues for ASPAN & ILSPAN. “Me Time”…that is what the AS-PAN National Conference in Grapevine, Texas was all about last May. I took a week of vaca-tion time and spent it in Texas with other people who feel the same way about perianesthesia nursing that I do—INSPIRED! There were so many educational offerings presented, that it was hard to pick which ones to attend, but no matter what I attended, it was worthwhile and fulfilled all expectations. The conference was all about advocating for our pa-tients, our profession and our-selves. As you might guess, I left no stone unturned when it came to having fun and getting to know others when the day’s conference was over. There was time for com-mittee meetings, new experiences and enjoying the amazing facility; the Gaylord Texan Resort and

Convention Center. And yes—it was a Texas size facility! I only bring this up in October because taking the time to attend this conference was one of the best things I could do for “me”. I got to participate in a great send off for our new ASPAN president, Lois Schick, MBA, MN, RN, CPAN, CAPA. Lois will promote the theme of “building on ASPAN’s dreams and adding to its legacy.” She has a story to tell, and you can read about her on ASPAN’s web site. But more than that….here it is October, and I am still inspired! I can only encourage all of you to think twice about all the right rea-sons for attending the educational opportunities provided by our wonderful organization. It should not only be because you are a member, or because it gets those contact hours, but should be be-

UNFINISHED BUSINESS Finalize Fall Conference—ten registrations to date. St. John’s will update monthly. Baskets with an autumn theme from each district will be awarded along with a donated book. Ad-ditional brochures were mailed to surgical centers and hospi-tals in MO & IL. A spreadsheet listing surgicenters and hospi-tals from eastern MO and IL was developed for mailing of conference brochures. Spring Conference topics/speakers 2009— Spring confer-ence for 2009 is scheduled for March 21 or 28 in Rockford depending on room availabil-ity. Discussion was held about a possible tri-state spring con-ference: at least 12-18 months planning time would be needed. Year-end budget—Wanda pre-sented a budget for 07/01/09 thru 06/30/09. Newsletter—Fall issue deadline is 9/01/08. Education articles were assigned BOD meeting dates---dates set for future board meetings are 01/24/09 in Springfield; 03/20 or 27 in Rockford 07/11/09 in Springfield; 09/25 or 10/02/09 in Rock Island ASPAN Seminars-- District III will host an ASPAN conference in February and District II in May prior to Memorial Day.

Dates to be confirmed at Septem-ber meeting. Evaluate/Update strategic plan—ILSPAN’s strategic plan was re-viewed. The board is actively com-pleting each component. Goal A.2.B. eliminated due to lack of feasibility. Gold Leaf Award criteria plan-ning—Rocky Mountain component was the recipient for the Gold Leaf Award for 2008. Point total for ILSPAN continues to increase. Suggestions were made on how to improve. Areas of weakness were community service and research.

CPAN/CAPA scholarship win-ners—no applications received to date. Spring Conference 2008—a check mailed 04/18/08 to cover the re-maining costs from SwedishAmerican hospital was never received. A motion was made to stop payment on check 959 and issue a new check for the amount of $666.50 to SwedishAmerican by Wanda and seconded by Beth. Motioned passed. Hail, Honor & Salute outgoing officers—Ellie Carlberg, Sylvia Baker, Darlene Meyer, Vicki Par-son all officers from previous years along with outgoing 2007 board members Peggy McKillip and Jeanette McAllister were rec-ognized for their service with a Hail, Honor and Salute tribute. Editor position-- The search con-tinues for a new editor. Marigrace

will continue as editor for now. NEW BUSINESS Component Development Insti-tute—is set for September 5 -7, 2008 in Salt Lake City. ASPAN pays for two registrations from each component. Kim recommends that ILSPAN should send one per-son and pay for all or a portion of the hotel and travel costs. The CDI is a great networking oppor-tunity. The best person to repre-sent ILSPAN would probably be the new VP/President-elect. Ta-bled until after polices are re-viewed. Conference Call Meetings—due to increasing costs of gas and travel-ing some boards are using confer-ence calls for their meeting. AS-PAN would like other components to try a conference call for a meet-ing and is willing to subsidize a 30-minute conference call. ILSPAN Recruiter Award Win-ner—Marigrace Clarke is the win-ner. ASPAN Nominations—will be available at Fall Conference. Kim urges everyone to consider. Forms are on the ASPAN website. Open positions are Regional 3 Director, Treasurer, President-elect, and Nominating Committee. Review of Bylaws and Policies/Procedures—were reviewed and revisions were made. NEW in 2009 Computer-Based Testing (CBT) will begin with the 2009 CPAN/CAPA tests. During the months of April and November there will be a 6 week testing window for candi-dates to take the exam. Locations, times and dates will be available at various test sites across the country. Special test sites will not be used. For more information and online application visit the ABPANC web site. Once a year at the ASPAN National Conference a paper and pencil exam will be offered.

The idea behind this op-eration is to create a small pouch in the upper part of the stomach with a controlled and adjustable stoma, without sta-pling. A gastric band device is introduced through tiny (1cm) incisions in the abdomen and placed around the upper part of the stomach. The band has a balloon from the upper part of the stomach that is adjustable and can reduce stoma size, thus prolonging the period of fullness.

The operation is per-formed under general anesthe-sia and can take between 30 minutes and 1 hour. You may be eligible for Lap Band sur-gery if: you are at least 18 years old, BMI > 40 or 100 lbs. more than ideal weight, if your BMI is >35 and you have one or more co-morbidities, you have been overweight for more than 5 years, your serious weight loss attempts have had only short term success, and if

you are prepared to make changes in your eating habits and lifestyle. Some of the advantages of a lap band are: 10 times safer than a gastric bypass, adjustable, cus-tomized per patient, least inva-sive, no stomach stapling, cutting or intestinal rerouting, is reversi-ble, lowest operative complication rate, no leaks, low malnutrition risk, OR time is 1 hour or less, and short overnight hospital stay in most cases. The disadvantages are: slower initial weight loss than a gastric bypass, and the need for regular follow-up ap-pointments are necessary. The contraindications include medical history of ulcers or Crohn’s disease, severe heart or lung disease, chronic or long term steroid treatment, if you or some-one in your family has Lupus or scleroderma, previous Laparo-scopic Nissen surgery, and if you are unable or unwilling to follow the dietary rules that come with the procedure.

The post-op care includes: pain relief with NSAIDS mainly, requiring very little opiates, checking the anatomy with a lim-ited Barium meal on day 1 to check patency, band position and pouch size, discharged on the same day or day 1 or 2. Patients are on clear liquids for 1-2 days, full liquids to puree for 1-4 weeks, and then can start on solids in 4-6 weeks. The initial band adjust-ment is 5-8 weeks post-op. In the last edition of Peri-Scope, we discussed the beginning stages of our research project. Our project began with the PICO (Population Intervention Compari-son Outcome) question-- “Is bacte-riostatic normal saline (BNS) as effective as intradermal buffered lidocaine (IBL) in reducing the pain of intravenous cannulation?” This question occurred after our pharmacy suggested we use BNS instead of IBL, saving them time and money. It was their position that BNS was as effective as IBL, but most of the nurses in the Same Day Surgery (SDS) unit felt that IBL would be more effective. We knew that our “gut feeling” had to be confirmed with evidence, so our Journal Club conducted an extensive literature review. We found 10 relevant articles and de-termined that current research did not clearly answer our ques-tion. We then realized this would be a good opportunity to do, what we thought would be a simple re-search project. Protocol There is no “simple” in re-search, but with the help of our research specialist we completed our study by replicating a pub-lished study. A common problem with research studies is insuffi-cient sample size. Replication studies provide a good way to in-crease the sample size on a topic that increases the generalizability of the results. We contacted the original researcher to gain permis-sion to replicate her work. Mem-bers of the research team were educated on the ethics of research involving human subjects and then we submitted a detailed pro-tocol to obtain approval from our hospital’s institutional review board. It can take several months to draft a protocol that meets the

strict requirements of a review board. Method We wanted a large sample size, so we performed a power analysis to be sure we had the right number of subjects in our study. All patients in SDS have an IV started before surgery, so it was easy to obtain the required sample. One nurse inserted all the IV’s and ob-tained pain ratings from each sub-ject using a modified verbal nu-meric rating scale (VNRS). Our results clearly demonstrate that intradermal buffered lidocaine is more effective than bacteriostatic normal saline in reducing pain prior to IV cannulation in a pre-op setting. Results and Implications Evidence shows that the dis-comfort associated with IV inser-tion is a cause of patient anxiety and dissatisfaction. As a result of this study our SDS nurses feel con-fident they are using the best agent to ease the discomfort of the IV in-sertion. Our experience proved to be very positive and well worth all the effort we put forth. Nurses in our area are more willing to evalu-ate “why we do the things the way we do.” We now know that evidence based information can make a dif-ference in practice and bring forth excellent nursing practice. To share our information with colleagues outside our institution, we are sub-mitting our manuscript to a nurs-ing journal for possible publication. Charting by Exception ASPAN does not have a position state-ment regarding documentation. I would recom-mend that one

consult the 2006-2008 Stan-dards of PeriAnesthesia Nurs-ing Practice, Standards VII (Planning and Implementation) and VIII (Evaluation), pages 18 and 19. Both of these Stan-dards address that documenta-tion reflect: interventions; re-sponses to interventions; con-sultations and collaborations; revisions to the plan of care; and use of established guide-lines. A second resource one

may want to investigate is Peri-Anesthesia Nursing Core Curricu-lum: Preoperative, Phase I and Phase II PACU Nursing (2004), pages 65 and 66. While this re-source, like the Standards, does not specifically address charting by exception, there are standard charting guidelines that should be addressed and monitored. There are multiple opinions on the use of charting by exception (and charting in general) that should be followed. Among the most important facts that staff should adhere to is that all blanks should be filled in or addressed in some way, and one’s documenta-tion needs to tell the complete story of the care given. One will not be able to recall individual pa-

Over the past several months, I’ve had the privilege of attending several compo-nent conferences in Region 3. You would think that I would have heard some of the same information at some point but each program was unique and I was able to learn new infor-mation and skills. It was ob-vious that perianesthesia nurses were eager to increase their knowledge base because attendance at these confer-ences was impressive. Whether it’s the result of more facilities taking the magnet journey or a require-ment for licensure or certifica-tion, nurses are looking for opportunities to grow in their profession.

ASPAN membership offers many opportunities to enhance your practice. Mem-bers receive discounted rates at these component confer-ences and ASPAN seminars

as well as discounted rates on all ASPAN publications, such as the new standards. The 2008 – 2010 Standards of Perianesthesia Nursing Prac-tice are now available. They will become effective Decem-ber 26, 2008. Some of the changes are updated sections that were reviewed for evi-dence. These sections include the Resource on Safe Transfer of Care, A Position Statement on “On Call/Work Sched-ule” (with a newly designed fatigue checklist included as a resource) and A Position Statement on Visitation in Phase I Level of Care. Re-

source 2 (previously Re-source 3 on Patient Classifi-cation/Recommended Staff-ing Guidelines) offers addi-tional clarification related to emerging staffing and patient acuity issues. The Standards also include two new important position statements: one on Geriat-rics and one on Advocacy. Take advantage of all your ASPAN membership has to offer.
