Person Centred Planning

Using PATH. MAP. and Essential lifestyle Planning

My part of the story

Tradtional Planning starts with the question

what’s wrong, what’s the problem

All the assessments showed that Nicola should only live in an institution. she could never go out in a car, or be part of all the main stream things, school, hobbies etc. that her brothers and sisters enjoyed.

For all of her life we were encouraged to ‘put her away’ into ‘special care’ it could cost £60.000 - £100.000 How ever much it took to work on her becoming a person who walks, talks and moved her arms.

Person Centred

1. The person is at the centre.

Person Centred Planning 5 Key Features

2. Family members and friends are partners in planning.

3. The plan shows what is important to the person – now and for the future. It shows strengths and what support they need.

4. The plan helps the person to be part of the community and helps the community to welcome them.

5. The plan is ongoing. Everyone keeps on listening and learning to make things happen. The plan puts into action. The

things the person wants to get out of life.

A Person Centred Approach

• Learn what is important to the person

and what support they require.

• Address issues of health and safety within this context.

• Put what has been learned into practice.

• Keep learning and look for opportunity to build community connections.

So far

When I started Sharing Knowledge in September 2009, which is a Partners In Policymaking Course, I was in a very different place than I find myself now!!!

I was just about to become homeless, I had no contact with Social Services and very little with the real world. I didn’t exist!!!

I WAS INVISABLE!

As A Youngster

y I felt restricted, my brothers were allowed to do more things than me, my mum used to say, “you have a disability” .

Year 11 y So when I was in year 11 my teacher said we are going

to do some independent travelling with you. We are going to put you on a bus with some teachers first then when you feel confident to be able to do it on your own we will look at timetables and number of buses. When I told my mum she was climbing the walls with fear.

Life after college Through college I joined Bury E.S.T. Employment

Support Training. My First Placement was at Matalan.

My second Placement was at Ethel Austin's. I felt my life was going nowhere

My work y I am Self Advocate / independent consultant and I work to

make changes to people’s lives who live with disability’s. y I work with Bury parents forum, where I facilitate disability

awareness training to parents. y I work with My Life CIC and other services around the country,

raising awareness of personalisation. y I also worked for Mencap as the Learning Disability Consultant

for Manchester for the Inspire Me project. y I am one of ten Ambassadors in the UK, who represent young

People for the Council For Disabled Children. y ALLFIE and Disability Rights UK for the project ‘I can make it’

America Research Washington DC week 1 Quality trust - advocates

for people with disabilities and helps them if they have any legal problems.

TASH – Which supports parents with children with disabilities to get the best outcomes for themselves and for there child's future.

Bob Williams – Head of disability benefits for the US social security.
