diagnosisafewyearsbackandfourshouldersurgeriesalongtheway.WhenhescheduledanappointmentwithDaniel Canter, MD,thelastthingonhismindwasthethreatofprostatecancer. “Denniscametomeforunrelatedsymptoms,”explainsDr.Canter,whoservesasAssociateChairmanoftheUrologicInstituteofSoutheasternPennsylvaniaatEinstein.“Becausehe’sintheagerangewhenweperformprostatecancerscreenings,IcheckedhisPSA[Prostate-SpecificAntigen]andperformedadigitalrectalexam.Ifeltanabnormalareaonhisprostateandaccordingly,recommendedabiopsy.Unfortunately,itshowedprostatecancer.”

“You’re never ready for that diagnosis the first time you hear it.” “You’reneverreadyforthatdiagnosisthefirsttimeyouhearit,”saysDennis.“Ithoughttomyself,nowwhat?”HemetwithDr.Canterandtheclinicalteamtoreviewtheoptions. “Peoplehearcancer,andthey’reshockedandscared,”saysDr.Canter.“Butprostatecancer,ingeneral,tendstobelessaggressivewhencomparedtoothercancers,suchasbreastorcoloncancer. “Themostcommontreatmentforlocalizedprostatecancerissurgicalremovaloftheprostate,whichisalmostalwaysperformedasaminimallyinvasiveorroboticprocedure,”explainsDr.Canter.“Theothercommontreatmentisradiationtotheprostate,andtherearetwodifferentapproachestoradiation.InDennis’case,therewasathirdoption.Hiscancerwasataveryearlystage–smallenoughthathecouldforegotreatmentandinstead,wewouldpursueacourseofactivesurveillancewherewekeepaclosewatchforanysignsofcancerprogression.” Dr.Canterpartnerswitheachpatienttoprovidetheeducationalinformationandresourcesthatwillallowthemtomakethebestdecision.DennisdiscussedtheoptionswithhisdaughterLorie–aphysicianassistant–andhiswifeof38years,Lynne.Theactivegrandparentsoftwodecidedonthemostdefinitivetreatment–surgicalremovaloftheprostate.“Iwantedtotakealloftheworryingoutofit,”saysDennis.“Iplantobearoundalotlongerformygrandchildren.” Dr.CanterperformedDennis’surgeryonMay22,2014.Healsoremovedthelymphnodessurroundingtheprostatetoensurethecancerhadnotspread.Theprocedurerequiredanovernightstay,oneweekwithacatheterwhilerecoveringathomeandtwopost-operativevisitstothephysician’soffice.

importanceofpreventative

screeningsand,iftheydo,

manycannotaffordthe

associatedcost.

Einstein Healthcare Network – one of the most comprehensive centers for the evaluation and treatment of lung cancer in the region – is committed to diagnosing lung cancer at the earliest possible stage, and therefore will provide low-dose CT lung screening free of charge until it is accepted as a medical benefit.

“We’reonamissiontobuildawarenesssurroundingtheimportanceofpreventativelungcancerscreeningandtoremoveanyfinancialbarrierstogettingscreened,”statesDoraid Jarrar, MD,board-certifiedthoracicsurgeon,EinsteinHealthcareNetworkandco-directorofEinstein’sfreeLungCancerScreeningProgram. AccordingtotheAmericanLungAssociation,lungcancercausesmoredeathsthanbreast,colonandprostatecancercombined–160,000everyyear

intheU.S.Infact,only16ofevery100patientsdiagnosedwithlungcancerin2014willbealiveinfiveyears. “What’smostfrustratingisthatwehavethetechnologytosignificantlydecreasethesestatisticsandsavelives,”statesDr.Jarrar.“Butpatientsareoftendiagnosedtoofaralonginthediseasetobenefitfromtreatment.” Dr.Jarrarstronglyrecommendsprimaryprevention–don’tsmokecigarettes.Butforthe20percentofAmericanswhocurrentlysmokeorwhoareformersmokers,secondarypreventionintheformofearlyscreeningsisvital. “Everycancer,ifcaughtearly,istreatable,”explainsDr.Jarrar.“Tremendousstrideshavebeenmadeincreatingawarenessfortheimportanceofmammograms,colonoscopiesandprostatescreeningsand,asaresult,survivalrateshavegreatlyincreased.Butforthedeadliestformofcancer,thereislittleawareness.Despitealloftheadvances,thesurvivalrateforlungcancerisnotimproving,simplybecausepeoplearenotgettingscreened.” AmajorclinicalstudyrecentlyconductedbytheNationalCancerInstitutehasledtoarecommendationbytheU.S.PreventiveServicesTaskForceforhigh-riskindividualstoundergoregularlow-doseCT(LDCT)lungcancerscreenings.Mostprivateinsuranceproviders,aswellastheCentersforMedicare&MedicaidServices,donotyetreimburseforthesescreenings,butwillbegintodosoin2015.Thecurrentout-of-pocketcostcanrunfrom$99to$500. ThefreeLungCancerScreeningProgramalsoinvolvesasmokingcessationcomponent,includingcounseling,medicationandnicotinereplacementtherapy.The

initiativeismanagedinitsentiretybyEinsteinHealthcareNetwork.EinsteinisproudtopartnerwithFreetoBreathe,anationalcollaborationaimedatdoublingthesurvivalrateoflungcancerpatientsby2022. “It’simportanttounderstandthatmanypeoplewithlungcancerdonotexhibitanysymptomsuntilit’stoolate,”saysDr.Jarrar.“Approximatelyninemillionpeopleareconsideredhighrisk.Withpreventativescreenings,wecanreducelungcancerdeathssignificantly.”

JaspaN: We’retakinggroundbreakingstepstoreinventgynecology–addressingwomen’shealthcareoverall.onesuchexampleiscardiology.Cardiovasculardiseaseistheleadingcauseofdeathinwomen.Recentdatatellsusthereareobstetricalfactorsthatareharbingersoffuturecardiovasculardisease,suchaspre-termlabor,highbloodpressureduringpregnancyandplacentalabruption.Assuch,we’reabletoidentifyriskfactorsearlyandperformpreventativescreeningsaccordingly.

DR. JaspaN: In1999,directlyfollowingmyresidency,oneofmymentors,Dr.PhillipHorn,askedmetojoinhispracticeatGermantownHospital.GermantownHospitalbecameEinstein.Soinactuality,thiswasmyfirstjob.
