Counselling and Preference Form filling Procedure Information and Instructions for Candidates

1. Admissions to Dental Postgraduate (MDS) courses affiliated to Maharashtra University of Health Sciences, Nashik (MUHS) for the academic year 2016-2017 will be done by counselling & Preference form filling system. This admission process is for admission to Government / Corporation and Private Unaided / Private unaided minority Dental Colleges.

2. Candidates from PGD-CET-2016 State Merit List are called according to State Merit for counselling and Preference form filling as per schedule given below (Proxy will not be permitted)

Instructions to Physically Disabled Candidates : All Qualified Lower Loco-motor Disabled / PWD (PH) Category Candidates of PGD-CET 2016 are advised to get themselves examined before preference Form Filling process at Medical Board constituted by District Civil Surgeon in the State of Maharashtra to obtain disability (PH) certificate in advance. Candidates who have attended this Medical Board, should remain present alongwith the disability certificate issued by aforesaid Medical Board on respective days of counseling, at respective Centre, where he/she have appeared for PGD-CET-2016 examination, for Preference Form filling process. Those who have obtained disability certificate for All India Quota PG admission 2016 from either : i) All India Institute of Physical Medicine and Rehabilitation, Mumbai OR ii) Vardhaman Mahavir Medical College & Safadarjang Hospital, Ansari Nagar, New Delhi OR iii) Institute of Post Graduate Medical & Research, Kolkata OR iv) Madras Medical College, Chennai, institution, can produce the same disability certificate at the time of Preference Form filling process. It is mandatory to bring disability certificate issued by anyone institution. Candidate without disability certificate will be considered as Non PWD candidate.

3. SELECTION PROCESS - DOCUMENT VERIFICATION & PREFERENCE FORM SYSTEM :

3.1 The candidates will give their preferences through a process "Preference form filling process" which has been notified by the competent authority on DMER web site www.dmer.org. The schedule, seat distribution and procedure regarding preference form filling process and details of further admission process is being made available through Preference form brochure on the website. Candidates should download copy of preference form brochure and read the same carefully. A candidate should be present personally for filling preference form (Proxy will not be permitted). The selection to a course/college will be made considering his/her Final State Merit List Number and preferences given by the candidate. Candidates as per State Merit List (SML) Number will be called for counselling and asked to fill the preference form. Counselling sessions are not meant for instant seat allocation at this centre. During this session, candidates are assisted in exercising their preferences for various courses and the institutions. Seat allocation shall be made centrally at a later date on the basis of PGD-CET-2016 Final State Merit List (SML) and preferences exercised by the candidate. (Not applicable to In-Service Candidate).

3.2 Selection Process shall be as follows: 3.2.1 The candidate should present personally to fill the preference form

(Proxy will not be permitted). The candidate should submit only one preference form. Submission of more than one preference form shall lead to disqualification.

3.2.2 At the time of filling the Preference form the candidate should bring all original documents and a set of attested copies of the same in support of claim made in application form i.e. Caste Certificate, Caste Validity Certificate, and non-creamy layer certificate. If the matter is still pending with Hon'ble Court, candidate should submit a fresh Court order for PGD-CET 2016, failing which claim will not be granted and the candidate will be considered as General Category candidate.

3.3 List of Documents Is Given Below:

If applicable… xv. Caste Certificate xvi. Caste Validity Certificate. xvii. Non-Creamy Layer Certificate (for (VJ) DT -A, (NT-1) NT-B, (NT-2) NT-C, (NT-3) NT-D,

OBC and SBC Candidates) issued by appropriate authority as specified in the Government Resolution No. CBC-10/2008/CR-697 VJNT-1, dated 24th June, 2013 and should be valid upto 31/3/2016.

xviii. (a) AIPMT selection letter / letter from Dean/Principal stating that the candidate was admitted for BDS under 15% quota from All India Entrance Examination.

(b) Domicile Certificate of Maharashtra State by virtue of residence, in case of Candidates who were Domicile of Maharashtra State at the time of BDS admission and were admitted under 15% AIEE quota in a dental college/institution situated outside Maharashtra State and who have obtained their degree from a University situated outside the State of Maharashtra.

xix. For Persons with disability (PWD) Candidates - Medical Fitness Certificate. As per Annexure - D of Information Brochure.

xx. Appropriate proof/ affidavit for Minority status for Muslim. Note : 1) Category candidates who do not have Caste Validity Certificate/fresh court order for PG admission regarding Caste Validity, at the time of preference form filling, will be considered as Open merit candidates subject to fulfilling eligibility.

2) DT (VJ), NT-1, NT-2, NT-3, OBC category candidates who do not have Non Creamy Layer (valid up to 31/03/2016) at the time of preference form filling will be considered as Open Merit candidates subject to fulfilling eligibility. 3) Such category candidates without validity and / or NCL, who are considered as Open Merit candidate will be issued preference form, if his/her SML Number is within the SML Numbers of Open candidates called for Preference Form Filling. In case his/her SML number is below the number of Open candidates called for counseling, then he / she will not be eligible for issue of preference form. 4) Applicants belonging to SBC : The person belonging to Special Backward Class (SBC) are to be considered as OBC candidates and such candidates will be required to produce Non- Creamy Layer Certificate.

3.4 For Constitutional reservation category candidates: Ear-Marking - Reserved category candidate entitled to admission on the basis of merit in an

open category seat will have the option of taking admission either against

his/her respective category seat or on an open category seat. If he/she opts

for the reserved category he/she will be deemed to have been admitted as an

open category candidate and not as reserved category candidate. One seat in

the college where he/she was eligible for admission against open seat shall be

earmarked for a candidate belonging to the respective reserved category.

Such earmarked seat will be made available immediately to the successive

reserved category candidate from the same category merit.

3.5 Based on preferences given, the seats will be allotted on merit and the

allotment will be displayed on website of DMER i.e. ‘www.dmer.org’. All the

candidates to whom a seat has been allotted in the 1st round on the basis of

Merit-cum-Preference will have to compulsorily join the college and course so

allotted, with in prescribed period by completing the requisite formalities, i.e

submission of original documents as per the list given in 3.4 and payment of

requisite fees.

3.6 It is made clear that even if a candidate to whom a seat has been allotted in the 1st round wants a betterment of his/her choice, such a candidate must initially accept the allotment and join the college by completing the necessary formalities as set out by these Rules, failing which such a candidate will be thrown out of the entire admission process of the current academic year 2016-2017.

If any candidate to whom a seat has been allotted in the 1st round does not join the college and course seat so allotted, it will be presumed that such a candidate is not interested in the admission process and for all further round(s) of admission, such candidate shall not be considered for allotment of any seat whatsoever.

The seat so allotted to such a candidate who does not join, will be presumed to be a vacant seat as on and from the next date after the date set out as the last date for joining the college in terms of such allotment. Such a vacant seat will be allotted in the next round(s) of admission process within prescribed period.

3.7 All the candidates who have filled the preference form and to whom a seat

could not be allotted in the round will be held eligible for allotment of a seat

in the subsequent round(s).

3.8 After the first Preference Form round, Competent Authority may conduct

subsequent preference round(s), based on the same preference form.

3.9 Cut-off date for the eligibility: The eligibility of the candidate shall be

determined and/or decided for all purposes including for applicability of all

these Rules and Regulations by considering the last date of document

verification as the cut off date. It is made abundantly clear that if a candidate

is found ineligible as on the last date of submission of preference form, in

terms of these Rules and Regulation, and if such an ineligible candidate

acquires the requisite eligibility after the aforesaid cut off date (i.e. the last

date of document verification), such subsequent acquisition of eligibility will

not make an ineligible candidate eligible for any purpose whatsoever, much

less for the purpose of admission.

4. Procedure followed at Counselling Center 1. Orientation: Candidates will assemble at the respective center on the given

day & time. They will be given orientation about the process. the candidates must present in person. Proxy will not be allowed Candidates reporting to wrong center will not be issued preference form.

2. Preference Form information brochure: This Preference form information brochure is made available to candidates on DMER web site i.e www.dmer.org. It

contains relevant information about the process, State Merit List No. of last admitted candidate for MDS courses for the academic year 2015-2016(Table1), college codes(Annexure A) & subject codes (Annexure B), and specimen copy of Preference form for practice. Candidates should read the information brochure of Preference system carefully and should bring the downloaded copy of the same for counseling.

3. Scrutiny Form: Scrutiny form will be issued to candidates. He/She should fill all details as per instructions. They should arrange documents in order as per list given on scrutiny form in two sets - one originals and another of attested Xerox copies.

4. Verification of Documents: Candidates will be called by State Merit List number. They should submit scrutiny form with a set of attested photocopies and show the originals. The documents will be verified by scrutiny officer & eligibility will be endorsed on the scrutiny form. The original certificates will be returned to the candidate immediately after verification.

5. Seat distribution chart: The seat distribution of state quota seats for PGD CET 2016 is already made available on DMER website i.e www.dmer.org, it contains distribution of available seats. The candidates should bring downloaded printout of the same for counseling. The seats reserved for in-service candidates and also for person with disability (PWD) candidates have been shown in the chart. The college-wise, subject-wise and category-wise distribution of Government/Corporation/Private Unaided and minority institutions is as per PGM/PGD -CET 2016 Information Brochure Annexure-C is available on website. It should be noted that eligible category candidate can avail open merit seat by virtue of merit. Category seats, if at all remain unclaimed by a particular category candidate are made available to other categories & to open category candidates as per inter-se category round procedure. Seat distribution chart is subject to change as per instructions of DCI / Transfer of PG Teacher / availability of surrendered seats from All India Quota, affiliation from university etc.

6. Person with Disability (PWD): As per Govt. letter No.MED-1006 /CR-72 /06 /

Edu-2, dated 07/3/2006, three (3%) percent of the total available Post-graduate seats at Government /Corporation Dental Colleges are reserved for the Physically Handicapped candidates.

1) As per Postgraduate Medical Education Regulation 2000 and Amendment Notification No. MCI.18(1)/2008-Med./54470 Dated 25th March 2009 following is the criteria for reservation under Physically Handicapped Category 3% seats of the annual sanctioned intake capacity shall be filled up by candidates with locomotory disability of lower limbs between 50% to 70%.

Provided that in case any seat in this 3% quota remains unfilled on account of unavailability of candidates with locomotory disability of lower limbs between 50% to 70% then any such unfilled seat in this 3% quota shall be filled up by persons with locomotory disability of lower limbs between 40% to 50% - before they are included in the annual sanctioned seats of State Quota.

Provided further that this entire exercise shall be completed by each Dental college/institution as per the statutory time schedule for admissions and in no case any admission will be made in the Postgraduate Dental course after 31st of May”

2) The Candidate with a disability more than 70% will not be eligible for admission to Health Sciences (Post Graduate) Courses. No other disability shall be allowed as per the MCI/DCI regulations. However, such a candidate will be required to undergo medical examination by Medical Board constituted for this purpose by the district Civil Surgeon from the State of Maharashtra. The Medical Board after ascertaining his/her disability must be satisfied that the candidate is physically fit to undergo the course despite his/her disability.

3) Candidate is required to submit the proof of his/her disability by way of a certificate (issued in year 2016) in prescribed proforma from the authorities stated in rule no 2 of this brochure; at the time of verification of documents during Preference form filling process. Those who have obtained disability certificate for All India Quota PG admission 2016 from either: i) All India Institute of Physical Medicine and Rehabilitation, Mumbai OR ii) Vardhaman Mahavir Medical College & Safadarjang Hospital, Ansari Nagar, New Delhi OR iii) Institute of Post Graduate Medical & Research, Kolkata OR iv) Madras Medical College, Chennai institution, can produce the same disability certificate at the time of Preference Form Filling Process.

7. Eligibility Criteria for in-service candidates: As per Government Resolution NO. PGM-1012/C.R.01/12/Edu.2 dated 30 January, 2012 and corrigendum dated 2 March, 2012 candidates who have appeared for PGD CET 2016 should fulfill the following eligibility criteria to be eligible for In-service reservation for academic years 2016-2017 seats.

a. ELIGIBILITY for In Service Candidates will be as per provisions mentioned in G.R. ihth,e&1012$iz-d-1$12$f'k{k.k&2 fnukad30 tkusokjh]2012 o ledzekafdr 'kklu fu.kZ; fnukad 2 ekpZ 2012-

b. To be eligible for in-service seats candidates should have passed BDS examination in maximum three attempts from DCI recognized Dental College, Situated in Maharashtra State.

c. Candidates working on the post of Dental Surgeon for Minimum 3 years in Government Dental College & Hospital or Government Medical College & Hospital are eligible for In-Service Reservation. Candidates working on temporary Dental Surgeon Post are not eligible for In-Service reservation.

d. Age of the candidates should not be more than 45 years at the time of submission of application for PGD CET.

e. Candidates must appear the PGD CET 2016 Examination and should qualify the examination as mentioned in rule no 11.3 of information Brochure of PGD CET 2016.

f. Separate Merit list will be prepared for in-service Candidates. For every one year service as dental surgeon 10% of obtained marks up to maximum 30% marks shall be added in the PGD CET obtained marks. Candidate qualifying

PGD CET are only eligible for this additional weightage mark. This additional weightage marks will be granted to the candidates serving at Government Dental College & Hospital.

g. There will be constitutional reservation in in-service quota. Roster for 5 years for this reservation will be maintained.

h. The In-Service reservation will be 15% of the seats available with Competent Authority excluding all India Quota. There will be no In-service reservation in the all India Quota Seats Surrendered to the State.

i. In-Service candidates after obtaining MDS qualification must serve The Government for 10 years otherwise he/she has to pay Rs 50,00,000/- (Fifty Lakhs Only) as a penalty.

8. Preference form : Preprinted Preference form will be issued to eligible candidates after document verification. The candidate should verify the relevant information of his Roll Number, State Merit List Number, Category Merit Number, PGD Marks etc. Preference form should be handled carefully and should not be folded, stapled or spoilt in any way.

9. In case the candidate is not eligible for admission according to rules, the Preference form will not be issued.

10. Candidate will be issued only one preference form. No duplicate/second Preference form shall be issued.

11. Candidates remaining absent for the process of preference from filling will not be issued preference form after the programme is over.

12. The subject & college combination code is a four digit number specific for each college & Subject combination. Candidate can fill minimum one preference and maximum 150 preferences only. It is not compulsory to fill all Preferences. The first two digit of the code indicates specific college / institution and last two digit indicates particular subject. For Eg.

Code no Dental College with code Subject with code 0101 GDC, Mumbai(01) Prosthodontics (01) 0202 Nair Dental College(02) Periodontics(02) 0905 Yogita Dental College Ratnagiri(09) Orthodontics(05) 2102 VSPM Dental College Nagpur(21) Periodontics(02)

13. College & Subject codes are given to courses existing in these colleges. For some of these codes the seat(s) may not be available for PGD-CET-2016 Quota during round 1. In the subsequent rounds, some seats may become available. Therefore, candidates who desire to opt for these seats can exercise the preference in the preference form for these seats considering eventuality of availability of such seats.

Candidate should read all instructions before he / she starts filling the preference form. Candidate should note carefully the subject & college combination codes. Candidate must first fill the specimen copy of Preference form given in the brochure with pencil for practice. Once the candidate is satisfied with his choices filled on specimen copy of Preference form, then he should start filling the actual Preference form using black ball point pen.

14. Candidate should write his most preferred college and subject combination code as Preference no.1 followed by his next choice as no.2 and so on. While filling the college and subject combination code number should be written in boxes.

15. The same subject and college combination code should not be filled more than once. In case the candidate fills same code more than once, its higher Preference number will be considered and subsequent repeated number will be ignored. If the candidate fills wrong code number it will not be considered.

16. The candidate should submit his duly filled Preference form at the specified counter. Candidate shall inform regarding any correction in the name if required at the counter.

17. Data Response Sheet & Verification:- Data response sheet of Preference Form in duplicate will be generated and will be given to the candidate for verification. Candidate should check that the data i.e. SML No. Name, Category etc. and his Preferences and subject college combination codes have been correctly printed. After verification the candidate will report any discrepancy in the data and it will be duly corrected according to his/her preference form. If the candidate does not report any discrepancy, it will be presumed that the data entered against his/her name is correct and subsequently corrections/alterations will not be made at later date.

The candidate should sign the data response sheet copies. Candidate will be given a copy of data response sheet.

18. Candidate should note that the Preferences of subject and college combination codes, once entered in the record/data, after verification by the candidate, shall be final and irrevocable. Thereafter there can not be any change, addition or deletion of Preferences entered by the candidate. This final data shall be used in entire selection process for the admission to MDS courses.

19. If a candidate gets selected for a particular MDS course, then the candidate is required to join that course as per the procedure. In case he / she does not join the course, he / she will not be eligible for subsequent admission process of PGD-CET-2016.

20. For filling of vacant seats after first round, same preferences will be considered for subsequent computerized rounds and no separate preference form will be required to be filled by the candidate (except for final round by personal appearance).

21. Fees:-

The fees for Government and Municipal Corporation Dental Colleges is as below as per GR. No. AMC-1007/628/CR.148/07/Edu-02 Mantralaya Dated 1 March, 2011 (subject to revision by the Govt./ appropriate authorities):

Status Retention: In case if a candidate to whom a seat has been allotted in the preference form round(s) and who has joined the college accordingly, is satisfied with the allotment, such a candidate must submit a Status-Retention Form in the given format as per Annexure C on or before prescribed date with the Dean of the College where such a seat has been allotted to such a candidate. The Dean of every Dental College to which admissions are so made in terms of these rules, may appoint an appropriate Officer to receive such Status Retention Forms. Upon receipt of such Status Retention Forms, an acknowledgement in writing shall be issued immediately to the concerned candidates to acknowledge the receipt of such a Status Retention Form within the prescribed time. It is mandatory for every such candidate submitting a Status Retention Form to insist for issuance of acknowledgement thereof and to preserve such acknowledgement till the present admission process is finally concluded. In absence of such an acknowledgement, the claim of submission of Status Retention Form within the prescribed time shall not be entertained. Candidate who has submitted status retention forms will not be eligible for personnel counseling round.

22. FINAL ROUND BY PERSONAL COUNSELING:

22.1 The Competent Authority will issue notification of final round by Personal Counselling on DMER website indicating the schedule of selection process. There shall be a final round for allotment of seats remaining vacant after completion of the earlier rounds of allotment of seats by way of preference forms. This final round of allotment of seats shall be conducted by way of personal counseling instead of allotment on the basis of preference forms and there will be instant seat allocation.

On account of allotment of seats in this final round of admission, no fresh vacancies consequent upon allotments made in this round will be created. This is principally because this final round of allotment is meant to fill up (as far as possible all) available vacant seats without creating a new or fresh vacant seat on account of such allotment.

At this round, eligible candidates will appear personally (Proxy will not be permitted), seeking allotment of seats of their choice. The personal counseling for such allotment will be held strictly in the order of State Merit List (SML) number. Each candidate will be offered all seats that have remain vacant and are available for allotment as per merit and category, only at the time of his/her turn of counseling i.e. State Merit Position.

22.2 Before personal counselling round all available vacant seats (due to non-allotment/non-joining/resigning etc.) will be notified on the website. All the Candidates should carefully go through the vacant seats as notified on website and decide as to whether retain the allotted seat to him/her or to go for Final Personal Counselling round. All the candidates who have joined in the previous round(s) and not filled status retention form earlier, will be given a stipulated period to fill status retention form.

It is hereby made clear that the period provided for submission of Status Retention Form is crucial and vital. This period shall not be extended for any reason whatsoever. Submission of the Status Retention Form is an irrevocable and irreversible act for the candidate submitting such Status Retention Form. It is made clear that under no circumstances whatsoever, a candidate will be allowed to submit a Status Retention Form after the aforesaid last date. Under any circumstances whatsoever, a candidate who has submitted a Status Retention Form shall not be allowed to withdraw the same either before or any time after the aforesaid last date. All the candidates therefore, are hereby called upon to carefully consider their case before submission of such a Status Retention Form.

All such candidates to whom seat has been allotted in the earlier round and who have accordingly joined the college if do not submit the ‘status retention form’ as set out herein will loose the seat so allotted and joined by them. All such candidates will have to compulsorily appear at the ‘final personal counseling round’ and get an allotment made in their favour in accordance with their relative merit, choice and availability, failing which such candidates will not get any seat whatsoever.

22.3 All the candidates who have submitted the Status Retention Form as above within the prescribed time and the seats occupied by them will be removed out of consideration for the remaining admission process.

22.4 Eligibility for the personal counselling round will be as follows:

22.4.1 The following candidates are Eligible for Personal Counseling round

(a) All PGD-CET-2016 candidates who have filled the preference form and to whom a seat has not been allotted in the previous round(s) of admission.

(b) All such candidates to whom though a seat was allotted in the previous round(s) and though such candidates joined the college accordingly by completing necessary formalities under these Rules, did not submit the Status Retention Forms as mentioned hereinabove on or before the date/s prescribed by the Competent Authority.

(c) All such candidates to whom though a seat was allotted in the All India Postgraduate admission process in a College situated in Maharashtra, but voluntarily surrendered the seat by giving an application before the prescribed date and the said seat is added to the State quota.

(d) All such candidates to whom a seat is allotted through PGD CET 2016 in the previous round(s) and joined the college, but resigned the seat will be eligible for personal counselling round.

It is made clear that no other candidate shall be held eligible for this round of admission.

22.4.2 Following candidates are Not Eligible for the personal counselling round.

(a) All PGD-CET-2016 candidates who have not filled the Preference Form. (b) All PGD-CET-2016 candidates to whom a seat has been allotted in previous

round(s) but have not joined are not eligible for personal counselling round.

(c) All the candidates to whom a seat has been allotted in the previous round(s) and who have joined the college accordingly and have further submitted Status Retention Forms by the aforesaid prescribed date will be removed out of consideration for the remaining admission process.

(d) PGD-CET-2016 candidates who have been allotted a seat thorough All India Postgraduate quota in Dental Colleges in Maharashtra and joined the College and have not surrendered the seat to the State quota by giving an application before the prescribed date.

23. The aforesaid final rounds of admission shall be conducted by personal counselling. For the purpose of this round, following seats shall be considered as available for admission which will be displayed on the web site before the commencement of Personal Counselling round :-

(a) All the seats that have remained vacant, if any, at the end of previous admission round(s) on account of non-allotment

(b) All the seats that were allotted in the previous round(s) but were rendered vacant on account of the candidate to whom such seats were allotted either did not join or resigned or joined but did not fill in Status Retention Forms.

(c) All such seats that may revert back to the State Government quota from All India Quota for any reason whatsoever, any seats remaining vacant from all India Post Graduate quota and In-services quota will be reverted back to State quota, after these seats are surrendered by the Concerned Authorities. Seats surrendered from All India Quota will retain their original Category character. All India Quota surrendered Seats having OBC character will be allotted to PGD CET candidates, of VJ, NT1, NT2, NT3 & OBC including SBC Category candidate (group together) as per inter-se merit (If OBC Reservation is included in All India Quota).

All India quota surrendered seats having unreserved (Open) Character will be allotted to PGD CET candidates, by common merit list which includes all category (SC, ST, VJ, NT-1, NT-2, NT-3, OBC including SBC) and open merit candidates.

(d) All such seats that may become additionally available on any account whatsoever, including but not limited to seats if any additionally recognized by Dental Council of India/MUHS, Nashik or made additionally available on account of any Order that may be passed by the Courts etc.

24. In the aforesaid final round of admission by personal counselling, preference forms submitted by all the eligible candidates will not be considered and the same shall be treated to have been cancelled. In other words, this final personal counselling round shall not be conducted on the basis of choices given in preference forms submitted by the candidates.

25. On account of allotment of seats in this final round of admission, no fresh vacancies consequent upon allotments made in this round will be created. This is principally because this final round of allotment is meant to fill in (as far as possible all) available vacant seats without creating a new or fresh vacant seat on account of such allotment.

At this round, eligible candidates will appear personally, seeking allotment of seats of their choice. The personal counseling for such allotment will be held strictly in the order of State Merit List. Each candidate will be offered all seats that have remain vacant and are available for allotment only at the time of his/her turn of counseling i.e. State Merit List position. In this round, allotment shall be made on the basis of choice made on the spot by the candidate at the time of personal appearance counseling. All the candidates to whom the seats will be allotted in accordance with the choice and available seats at his/her merit position at the time of personal counseling, he/she will be required to join the concerned college and complete all necessary formalities (by submitting all original documents and paying prescribe fees) for taking admission accordingly within the prescribed time. Allotment of seat once made in this personal counseling round, shall

not be changed and/or cancelled at any count whatsoever. Therefore, while

giving the choice on the spot at the time of this personal counseling round,

the candidates must consider the available seats thoroughly and opt for a

seat which they must join once allotted.

It is made hereby clear that in case such a candidate does not join the

concerned College and/or fails to comply with the necessary formalities of

the admission within the prescribed time, such a candidate shall not be

eligible for appearing at next two succeeding PGD-CET conducted for grant of

admissions to post-graduate courses in the State of Maharashtra i.e. PGD-

CET 2017 & 2018 and will have to pay Rs. 10,00,000/- (Rupees Ten Lacs

Only) as penalty for lapse of post graduation Dental seat.

26. THE CONDITIONS TO BE SATISFIED BY THE CANDIDATE AFTER SELECTION: 26.1 A candidate will have to pay the prescribed fees (i.e. tuition + deposit + others).

In the form of separate Demand Drafts, (One for tution fees and another for other

fees) In favour of Dean/Principal of concerned institution. He/she must deposit

all relevant original certificates/documents in the concerned institution.

26.2 All selected candidates will have to submit DD/Pay order of 650/- drawn on any

Nationalized/Scheduled Commercial Bank in favour of "NATIONAL INSURANCE

COMPANY LIMITED, KOLHAPUR" Payable at Kolhapur for Amartya Shikshan

Yojana as per GR.MED/100/880/CR-129/Edu-1, dated 13/10/2003 submitted

to the concerned Institution.

27. CANCELLATION AND REFUND: 27.1 Candidate who has confirmed his/her admission may cancel it by submitting

an application to the concerned Dean/the head of the institution. (As per Annexure-D)

27.2 Candidate cancelling his/her admission on or before prescribed date, which will be notified in due course of time, will be eligible for subsequent PGD-CET examination.

27.3 Candidate cancelling his/her admission after Prescribed date will not be eligible for subsequent two year for PGD-CET.

27.4 Refund of fees by the college after change of college, course or cancellation of admission: a) On or before prescribed date Rs  1500/- (Rs. One Thousand Five Hundred

only) to be deducted and rest of the fees to be refunded. b) After prescribed date - No refund.

27.5 If a candidate expires or becomes unfit to undergo PG course as per DCI Norms and decided by notified Medical Board within 3 months from the date of admission, full refund shall be made.

28. CONDUCT AND DISCIPLINE:

28.1 Failure of the candidate in making full and correct statements in the application form and/or suppression of any information would lead to disqualification of the candidate, even at later date. Such a candidate will be debarred from the entire selection process.

28.2 It is responsibility of every candidate to submit proper documents. Any attempt to submit documents which are not genuine will lead to cancellation of the admission of the candidate, forfeiture of the fees, deposits and expulsion of the candidate from the college by the Competent Authority or by his authorised official. The name of such candidate/s shall be deleted from the State Merit List and he/she will not be eligible for further rounds of the selection process and will be debarred from the selection process. All such action against the candidate if leads to lapse of seat then additional penalty of Rs 10,00,000/- (Rupees Ten Laks Only) will be recovered from the candidates. If deemed fit even criminal proceeding may be initiated by the Competent Authority against such candidates/ their parents.

28.3 If any candidate/Authorised representative or person accompanying is found obstructing the counseling and/or admission process, or trying to influence in unlawful manner/creating nuisance, the candidate shall be liable for disqualification from the process of selection and appropriate legal action will be taken. The decision taken by the Competent Authority in this respect shall be final and binding.

28.4 The registration once granted is valid only if the teacher and the Dean are satisfied about regularity, diligent pursuance of studies by the student as well as good conduct.

28.5 All candidates selected for admission shall be subject to the disciplinary rules of the Institution, MUHS, Nashik, and Government.

28.6 Maharashtra University of Health Sciences, Nashik shall take the decision regarding eligibility, enrolment and registration and granting of the term of selected candidates. The selection of a candidate by the Competent Authority at a later date does not place any onus to accord approval for grant of term.

28.7 Canvassing directly or indirectly for the allotment of seat would disqualify the candidate for admission. Influencing the staff by unfair means, will lead to cancellation of allotted seat and such candidate will also face disqualification for appearance in the subsequent PGD-CET examination.

28.8 A candidate selected for postgraduate course will not be allowed to change subject/college/institute on mutual basis as per the rules of Institution/ MUHS regulations.

28.9 In case of dispute in the matter of allotment of seat/course/institution or any other matter pertaining to the provisional selection or cancellation of admission, the decision of the Competent Authority shall be final and binding.

28.10 Legal Jurisdiction: All disputes pertaining to the conduct of examination and selection process shall fall within the jurisdiction of courts in Mumbai only.

29. PENALTY & BOND:

29.1 Candidate who has joined Postgraduate Course should complete PG studentship, failing which he/she will be required to pay Rs.10,00,000/- (Rupees Ten Lakhs Only) for the default (i.e. Non Completion of PG Studentship). However this is not applicable to the candidates of PGD-CET 2016 resigning the PG Registration before personal counselling round.

29.2 Any candidate responsible for lapse of post graduate Dental seat will have to pay Additional penalty of Rs.10,00,000/- (Rupees Ten Lakhs Only) as stated in rule no 14.7 Any such candidate will not be eligible for appearing at succeeding two PGD-CET i.e. PGD-CET 2017 & 2018.

29.3 As per Government Resolution (G.R. No. MED 1010/CR-184/10/Ed-2 dated 28th May 2010 and any G.R issued in this regard from time to time), candidates joined against the seats of Government/Municipal Corporation colleges for admission to Post graduation Courses either through All India Post Graduate quota or through PGD CET 2016 will be required to sign a bond to serve the Government of Maharashtra or local self government or Defence services for a period of one year, failing which he/she will be required to pay Rs. 50,00,000/- (Rupees fifty Lakhs only) for the default.

Note: In this process of preference from filling and subsequent round/s (If any), admissions to those post graduate course will be done which are recognized/permitted/Not recognized by Medical Council of India. Hence the candidates are instructed to fill the preferences for all such courses accordingly, if they desired so. However in case of not recognized course the candidates will have to submit an under taking in the prescribed format at the time of admission to respective college/Institute.

30. Any issue not dealt here-in above will be dealt with, when arising, fully and finally by the Competent Authority. Any amendments made by Government of Maharashtra from time to time will be implemented.

31. Final round by personal appearance will be held only in Mumbai, but not for Inservice Quota.
