Psychological dependence
Social dependence
Nicotinic cholinergic receptor activation promotes the release of a variety of neurotransmitters
Benowitz NL. Clin Pharmacol Ther 2008;83:531‐41.
Effects of nicotine on other organ systems
• Dopamine‐‐‐‐‐‐‐‐‐ Increase GI movement, Nausea/ vomiting • Acetylcholine‐‐‐‐ Increase acid secretion• Serotonin‐‐‐‐‐‐‐‐‐ Increase GI movement, Nausea/ vomiting • Norepinephrine‐ Increase heart rate, sympathetic symptoms
Model for the nicotine addiction cycle during daily cigarette smoking
Benowitz NL. Clin Pharmacol Ther 2008;83:531‐41.
Nicotine withdrawal symptoms• At least 4 or more symptoms following abrupt cessation or rapid decreasing of nicotine products within 24 hours. • Decreased heart rate• Anxiety • Depression • Unstable mood, emotional sensitive• Insomnia• Agitation • Increased appetite, craving

Mechanism of actions of anti‐smoking drugs
Benowitz NL, et al. CNS Drugs 2000;13:265‐85.
Nicotine Antidepressants: Bupropion, Nortriptyline, Doxepine, Fluoxetine
Valenicline, Cytisine
Clonidine
Nicotine plasma concentration during the smoking day
วธิีการแปะ nicotine patch ‐ 1 cigarette = 1‐3 mg of absorbed nicotine‐ about 50% of loaded nicotine in gum is absorbed
Step 1: 6 weeks • 1 tab q 1‐2 hours
Step 2:3 weeks
• 1 tab q 3‐4 hours
Step 3: 3 weeks
• 1 tab q 6‐8 hours
