วตถประสงค :

เมอผานกระบวนการเรยนรนแลว นกศกษาสามารถ

1. บอกสวนตางๆ ของ pharynx และบอกขอบเขตของมนได

2. บอกชอกลามเนอของ pharynx ได

3. บอกสวนตางๆ ของโครงสรางภายในของ pharynx แตละสวน

4. บอกความสมพนธของ pharynx กบอวยวะทอยทางดาน lateral ของ pharynx

5. บอกชอ nerve supplies และ blood supplies ของ pharynx ได 6. บอกชอโครงสราง และสวนตางๆ ของ ชองปาก ลน และเพดาน ได

7. บอกชอกลามเนอของ palate รวมทงหนาทและเสนประสาททมาเลยง

8. บอกชนดของ lingual papilla และบรเวณทพบได

9. บอกชอกลามเนอของลน รวมทงหนาทและเสนประสาททมาเลยง

10. บอกชอของตอมนาลายแตละชนด รวมทงตาแหนงทเปดออกของทอนาลายในชองปาก

Craniovertebral joints

ประกอบดวย 2 กลมขอตอคอ

1. Atlanto-occipital joints เปนขอตอระหวาง Atlas (C1) และ occipital condyles of the

skull

2. Atlanto-axial joints เปนขอตอระหวาง atlas และ axis (C2)

เปนขอตอชนด synovial และไมม intervertebral disc ระหวางชนกระดก จงทาใหเคลอนไหวได

มากกวาขอตออน ๆ ของ vertebral column

Pharynx

Pharynx หรอ คอหอย หรอหลอดคอ เปนทอกลวง อยกลางลาคอ มความยาวประมาณ 15

ซม. มผนงสวนใหญเปนกลามเนอลาย ภายในบดวย mucous membrane ภายนอกดานหลงและ

ดานขางคลมดวย buccopharyngeal fascia

Pharynx เรมจาก base of skull

ทางดานบน ทอดดงลงไปจนถงขอบ

ลางของ cricoid cartilage ซงตรงกบ

ขอบลางของ 6th cervical vertebra

ตาลงไปจะเปน esophagus. Pharynx

มความกวางมากทสดทระดบ hyoid

bone และแคบทสดทปลายลาง

Pharynx แบงออกไดเปน 3

สวน โดยใช soft palate และขอบบน

ของ epiglottis เปนเสนแบง

1. Nasopharynx อยเหนอกวา soft palate และอยหลงตอ nasal cavity

2. Oropharynx อยระหวาง soft palate และขอบบนของ epiglottis และอยหลงตอ oral

cavity

3. Laryngopharynx อยตากวาขอบบนของ epiglottis และอยหลงตอ larynx สามารถเปด

ตดตอกบสวนบนของ larynx ได

กลามเนอของ Pharynx ม 2 กลม คอ outer circular และ inner longitudinal muscles

Outer circular muscles เปนกลมกลามเนอทคลมอยดาน

นอกของ pharynx ทาหนาทบบตวหรอ

หดตวเพอให pharynx ทาหนาทกลน

อาหาร ประกอบดวย กลามเนอ

superior, middle และ inferior

constrictors มลกษณะเปนวงตามขวาง

เมอรวม 2 ขางจะเปนรปคลายตว U โดย

เปดผนงไปดานหนา กลามเนอทงสาม

นซอนทบกนจากบนไปลางคลายกระเบอง

มงหลงคา โดยกลามเนอมดลางจะอยนอก

สด และมดบนจะอยในสด

ใยกลามเนอสวนลางสดของ inferior constrictor ทเกาะจากกระดกออน cricoid (เรยกวา

cricopharyngeus) จะทาหนาทเปนหรด (esophageal sphincter) ของสวนบนของหลอดอาหาร

ปองกนไมใหอากาศหรอลมผานลงไป และจะคลายตวเมอมการกลนอาหารลงไปในหลอดอาหาร

Inner longitudinal muscles เปนกลมกลามเนอทมใยกลามเนอทอด

ขนลงตามยาว ประกอบดวยกลามเนอ

stylopharyngeus, palatopharyngeus

และ salpingopharyngeus ทาหนาทดง

larynx และ pharynx ขนขณะกาลงกลนหรอ

พด

Nerve supplies of pharynx

ทผนงดานขางของ pharynx (สวนใหญจะอยบนกลามเนอ middle constrictor) จะเปนทอยของ

pharyngeal plexus ซงเกดจาก pharyngeal branches ของเสนประสาทสมองคท 9 คท 10 และ

sympathetic fiber จาก superior cervical ganglion มารวมกน และจาก plexus จะใหแขนงทง

motor และ sensory fibers ไปเลยง pharynx โดย

Motor fibers ทเลยงกลามเนอ constricter m. มาจาก pharyngeal plexus ทเปนแขนงของ

เสนประสาทสมองคท 9 คท 10 สวน longitudianal muscles of pharynx เลยงโดย pharyngeal branch

of vagus nerve ยกเวนกลามเนอ stylopharyngeus ทเลยงโดย glossopharyngeal nerve

สวน mucous membrane ของ pharynx จะนาความรสกออกไปทาง sensory fibers ทมาจาก

glossopharyngeal nerve เกอบทงหมด ยกเวน nasopharynx ซงนาความรสกออกไปทาง maxillary

division ของ trigeminal nerve

Blood supplies of pharynx

Arterial supplies of pharynx ไดมาจากหลายทาง ไดแก

1. Ascending pharyngeal artery แขนงจาก external carotid artery

2. Tonsilar branch of facial artery เลยง palatine tonsil ทอยใน oropharynx

3. Ascending palatine artery แขนงจาก facial artery

4. แขนงจาก superior thyroid artery

5. แขนงจาก inferior thyroid artery

Venous drainages of pharynx จะไปรวมกนท pharyngeal venous plexus แลวเปดส internal

jugular vein โดยผานทาง pharyngeal vein, superior thyroid vein และ inferior thyroid vein

ลกษณะภายในของ nasopharynx

ผนงสวนบนและสวนหลงเปน mucous membrane (mucosa) ทคลมบนสวนลางของกระดก

sphenoid และบางสวนของ basilar part ของ occipital bone มกลม lymphoid tissue ท mucosa

ของบรเวณนเรยกวา pharyngeal tonsil หรอ adenoids

ผนงสวนดานขางบรเวณตอนกลางจะเหนรเปดของ Eustachian tube (pharyngotympanic tube

หรอ auditory tube) ทอนตดตอมาจากหสวนกลาง (tympanic cavity) มความยาวประมาณ 3.5 ซม.

ปลายดาน lateral 1/3 ทอยใกลกบหชนกลาง มผนงเปนกระดก สวนดาน medial 2/3 ทอยใกล

nasopharynx เปนกระดกออน

ปลายดานในทเปนกระดกออนจะคลมดวย mucous membrane ซงปรากฏเปนสนนน คลมอย

ดานบนโคงไปดานหลง เรยกวา torus of auditory tube หรอ torus tubarius ปลายดานลางของ

สนนจะเรยงเลกลง เรยกวา salpingopharyngeal fold ภายในมกลามเนอ salpingopharyngeus

หลงตอ torus tubarius และ salpingopharyngeal fold เปนซอกหรอหลบทอดตามแนวดง

เรยกวา pharyngeal recess

ลกษณะภายในของ oropharynx

Oropharynx เปนสวนของ pharynx ทเปนทางผานทงอาหารและอากาศ มขอบเขตดงน:-

ดานบนเปน soft palate

ดานหนาตอนบน ตดตอกบชองปากทาง oropharyngeal isthmus หรอ fauces (ซงมขอบเขต

ดานบนเปนรอยตอระหวาง soft palate กบ hard palate ดานขางเปน palatoglossal fold ดานลาง

เปนแนวรอง sulcus terminalis)

ดานหนาตอนลางเปนโคนลน หรอ dorsum of tongue (posterior 1/3 of tongue)

ดานลางเปนขอบบนของ epiglottis

รปท 11 ขอบเขตและลกษณะภายในของ oropharynx

Epiglottis เปนแผน elastic cartilage ท

คลมดวย mucous membrane มรปรางคลาย

ชอนทหนสวนเวาไปดานหลง มดานหนาอยชด

สวนลางของโคนลน ทผวดานหนาในแนวกลาง

จะมสนยนไปดานหนา ไปเชอมสวนลางของโคน

ลน เรยกวา medial glossoepiglottic fold ท

ขอบดานขางของ epiglottis มสนยนออกไป

ดานหนา ไปเชอมกบดานขางของโคนลนชดผนง

ดานขางของ pharynx เรยกชอวา lateral

glossoepiglottic folds ดงนนระหวาง folds

ทงสามนจะเกดแองขน 2 แหง เรยกชอวา

epiglottic valleculae

ทผนงดานขางตอนหนาของ

oropharynx จะเปนบรเวณรปสามเหลยมท

เกดจาก palatoglossal และ

palatopharyngeal arches กบ dorsum of

tongue ม floor เปนกลามเนอ superior

constrictor และ pharyngobasilar fascia

เรยกสามเหลยมนวา tonsillar bed

เนองจากตอนกลางเปนทอยของ palatine

tonsil

ตอม palatine tonsil มหลอดเลอดมาเลยงมากมาย แตทสาคญเปน tonsillar artery ทเปน

แขนงของ facial artery ซงจะแทงทะลกลามเนอ superior constrictor เขาส lower pole ของ tonsil

สวนหลอดเลอดดาจะผานออกทาง external palatine vein ผานชด lateral surface ของ tonsil แลวเท

เขาส pharyngeal plexus of veins และ nerve supplies ของ tonsil ไดมาจาก tosillar plexus ซง form

โดย glossopharyngeal และ vagus nerve

Lymph vessels ของ tonsil ผานออกดานขางไปยง jugulodigastric lymph nodes ทบรเวณ

angle ของ mandible ตอมนาเหลองนบางทเรยกวา tonsillar node เนองจากมการบวมโตบอยๆ

รวมกบการอกเสบของตอม tonsil (tonsilitis)

ลกษณะภายในของ laryngopharynx

เปนสวนของ pharynx ทอยหลง larynx โดยสวนบนเรมจากขอบบนของ epiglottis ลงไปถงขอบ

ลางของ cricoid cartilage ซงตรงกบขอบลางของ 6th cervical vertebra สวนลางของ

laryngopharynx จะแคบลงแลวเลยตอไปเปน esophagus ดานหนาตอนบนจะเปดไปส inlet of larynx

ดานหลงสมพนธกบ bodies of C4 – C6 ผนงดานขางและดานหลงประกอบขนจากกลามเนอ middle

และ inferior constrictor และถดเขามาดานในจะเปนกลามเนอ palatopharyngeus และ

stylopharyngeus

บรเวณสองขางของ larynx ทตดตอกบผนงของ laryngopharynx มลกษณะเปนซอกลกไป

ดานหนาเรยกวา piriform recess ดานขาง recess นลอมรอบโดยสวนหลงของ thyroid cartilage

และ thyrohyoid membrane ใตตอ mucous membrane ของบรเวณนจะม เสนประสาท internal

laryngeal และ recurrent laryngeal ผานเขามารบ sensation ของ mucosa บรเวณน โดย internal

laryngeal nerve ผานเขามาทางดานบน และ recurrent laryngeal nerve ลอดใตขอบลางของ inferior

constrictor ขนมา

กลม lymphoid tissue เรยงตวเปนวงลอมรอบบรเวณ superior part of the pharynx เรยกชอวา

“Waldeyer’s ring” ประกอบดวย pharyngeal, tubal, palatine, และ lingual tonsils

Oral Region

ประกอบดวย oral cavity (mouth), teeth, gums (gingivae), tongue, palate รวมทง palatine tonsils

Oral cavity (mouth)

ประกอบดวย 2 สวน คอ oral vestibule และ oral cavity proper

- Oral vestibule เปนซอกแคบ ๆ ทแทรกอยระหวางดานนอกของฟน กบกระพงแกม

- Oral cavity proper เปน space ทอยดานในของฟนและเหงอก ม roof เปนเพดานปาก (palate)

และมชองเปดดานหลงตดตอกบ oropharynx

Teeth (ฟน)

ทาหนาทเคยวอาหาร และมสวนรวมในการออกเสยงพด ฟนม 2 ชนด คอ ฟนนานม (primary

or deciduous teeth) ม 20 ซ และ ฟนแท (secondary or permanent teeth) ม 32 ซ

ฟนแตละซประกอบดวยสวน crown, neck และ root เนอฟนประกอบดวย dentin และม

enamel คลมในสวน crown ม cement คลมในสวน root ตรงกลางของฟนจะมโพรง เรยก pulp

cavity เปนทอยของ connective tissue, เสนเลอด และเสนประสาท ซงมทางตดตอผาน root canal

ออกมาดานนอกของรากฟน โดยผานทาง apical foramen (root foramen)

รปท 15 สวนตางๆ ของฟน A = ฟนตด (incisor) B = ฟนกราม (molar)

ฟนบนขากรรไกรบนไดรบ blood

supplied จาก posterior และ anterior

superior alveolar arteries ฟนบนขากรรไกร

ลางไดรบจาก inferior alveolar artery

ทงหมดเปนแขนงจาก maxillary artery.

Nerve supplied ของฟนบนขากรรไกรบน

คอ posterior, middle, และ anterior

superior alveolar nerves แขนงของ CN V2

และของฟนบนขากรรไกรลาง คอ inferior

alveolar nerve แขนงจาก CN V3

Palate (เพดานปาก)

แบงเปน 2 สวน คอ hard palate และ soft palate

Hard palate

เปนสวนทอย anterior 2/3 ประกอบขนจาก palatine process of the maxillae และ

horizontal plates of the palatine bone มรเปด 3 ร ไดแก

1. Incisive fossa เปนททอดผานของ

nasopalatine nerve ทผานมาทางชองจมก

2. Greater palatine foramen ม greater

palatine vessels and nerve ทอดผาน และ

ทอดไปดานหนา

3. Lesser palatine foramen เปนททอดผาน

ของ lesser palatine nerves และ vessels ไป

เลยง soft palate

Soft palate

เปนสวนทอย posterior 1/3 แบงเปนสวน aponeurotic palate ทตอจากขอบหลงของ

hard palate และสวน muscular palate ทอยหลงกวาและโคงลงลาง มปลายสดเปน uvula

ขอบดานหลงของ soft palate จะตอเนองไปกบผนงของ pharynx มขอบเขตของรอยตอ คอ

บรเวณ palatoglossal และ palatopharyngeal arches ชองทางเปดตดตอระหวางชองปากกบ

pharynx เรยกวา fauces

Superficial features of the palate

mucosa ของ hard palate จะยดตด

แนนกบกระดก ใตตอ mucosa ของ

palate จะม palatine glands ซงทา

หนาทให mucous secretion มทอมาเปด

ออกเปนรพรนคลายผวสม ในแนวกลาง

ชดตอดานหลงของ maxillary incisor

teeth จะเปน mucosa ทยกตวขนมา

เรยกวา incisive papilla ซงเปนทางผาน

ออกของ nasopalatine nerve ดงนน

incisive papilla จงใชเปนตาแหนงฉดยาชา

สาหรบ nasopalatine nerve

จาก incisive papilla จะมสนตามขวางหลายแนวแผออกไปเรยกวา transverse palatine folds

หรอ palatine rugae และมรอง หรอแนวทอดตรงไปดานหลงในแนวกลาง เรยกวา palatine raphe

เปนแนวทบงบอกตาแหนง fusion of the embryonic palatal processes.

Muscles of the soft palate

ม 5 มด ไดแก

1. Levator veli palatini ทาหนาทยก soft palate ขนขณะกลนหรอหาว

2. Tensor veli palatini ทาหนาทดง soft palate ใหตง และเปดรเปดของ auditory tube

ในขณะกลนและหาว เปนมดเดยวในกลมนทเลยงโดยแขนงของ CN V3 (mandibular nerve)

3. Palatoglossus ทาใหเกดเปน palatoglossal arch ทาหนาทยกลน และดง soft palate ในทศ

ทางเขาหากน

4. Palatopharyngeus ทาใหเกดเปน palatopharyngeal fold

5. Musculus uvulae ทาหนาทยก uvula ขน

กลามเนอทกมดของ soft palate ยกเวน tensor veli palatini ถกเลยงโดย pharyngeal branch of

vagus nerve

Vasculature of the palate

Palate ไดรบ arterial blood supply หลายทางดงน

- Greater palatine artery แขนงของ descending palatine artery เลยง hard palate

- Lesser palatine artery แขนงของ descending palatine artery เลยง soft palate และไป

anastomose กบ ascending palatine artery

Veins of the palate เปน tributaries ของ pterygoid venous plexus

Sensory innervation of palate

เปนแขนงจาก pterygopalatine ganglion ซงใหแขนงดงน

- Greater palatine

nerve ไป supplies สวน

ใหญของ hard palate

- Nasopalatine nerve

supplies mucous

membrane ของ anterior

part of hard palate

- Lesser palatine

nerve supply soft

palate

Tongue (ลน)

เปน muscular organ มหนาทสาคญเกยวของกบการพด และการกลน แบงเปนสวน root, body,

apex, dorsal surface และ inferior surface

- dorsal surface จะโคงนน มแนวรป - shape เรยก terminal sulcus ทปลายแหลมของ

จะชไปดานหลง และปลายสดมหลมเลก ๆ เรยก foramen cecum (เปน remnant ของ proximal part of

thyroglossal duct ใน embryo) อาศยแนว terminal sulcus นแบงลนออกเปนสวน anterior 2/3 ซงไดแก

สวน body และ posterior 1/3 ทจดเปนสวน root และมสวนปลายสดของลนคอ apex mucous

membrane ดาน dorsum of tongue จะขรขระ ในสวน anterior 2/3 จะมตมนนเลก ๆ จานวนมาก

เรยกวา lingual papilla

lingual papillae จาแนกเปน

- Vallate papillae มขนาดใหญสด เรยงอยดานหนาของ terminal sulcus

- Foliate papillae ไมคอยเจรญในมนษย

- Filiform papillae มขนาดเลกปลายแหลม มจานวนมาก กระจายอยทวไป ยกเวนปลายลน

- Fungiform papillae รปรางคลายดอกเหด พบประปรายโดยกระจายทวไป รวมทงบรเวณ

ปลายลน และดานขางลน

Vallate, foliate และ fungiform papillae จะม taste bud แทรกอยในชน epithelium ของมน

นอกจากนอาจพบ taste bud ไดในบรเวณ soft palate, posterior wall of oropharynx และ epiglottis

ดาน dorsum ของลน ยงมรองในแนวกลาง เรยกวา midline groove (median sulcus) ซงเปน

แนวของ lingual septum ทแบงลนเปนซกซาย และซกขวา

Mucous membrane ในสวน posterior 1/3 ของลน จะไมม papillae แตจะม lingual lymphatic

follicle ซงเรยกวา lingual tonsil

- inferior surface ของลน (sublingual surface) จะเปนเพยง mucous membrane บาง ๆ ม

fold ของ mucous membrane ทยดตดกบ floor of the mouth เรยกวา lingual frenulum แตละขาง

ของ folds นจะมองเหน deep lingual vein อย และเหน sublingual caruncle เปนกอนนนขนาบอย

สองขางของโคนของ lingual frenulum ซงบรเวณยอดของ caruncle จะมรเปดออกของ submandibular

ducts

Muscles of the tongue

ลน เปนโครงสรางทประกอบขนจากกลามเนอลวนๆ โดยจาแนกเปน intrinsic muscles 4 ค และ

extrinsic muscles 4 ค กลามเนอแตละขางแยกจากกนดวย lingual septum

Extrinsic muscles of the tongue ประกอบดวย genioglossus, hyoglossus,

styloglossus, และ palatoglossus สามมดแรกเลยงโดย hypoglossal nerve (CN XII) แต

palatoglossus เลยงโดย CN X หนาทหลกทาใหเกด movements ของลน และชวยในการเปลยนรปราง

ของลน โดยเฉพาะสวน posterior fibers of genioglossus จะดงใหลน protrude ออกมาขางหนา (แลบ

ลน) ถากลามเนอนหดตวขางเดยว เชนกรณทเสนประสาททมาเลยงเสยไป จะทาใหเวลาแลบลน ลนเฉ

ไปดานตรงขาม

Intrinsic muscles of the tongue ประกอบดวย superior longitudinal, inferior longitudinal,

transverse, และ vertical ทงหมดเลยงโดย hypoglossal nerve (CN XII)

Sensory innervation of the tongue

- General sensation จาก anterior 2/3 ของลน ถกนาไปโดย lingual nerve ซงเปนแขนงของ CN

V3 แตถา special sensation จากบรเวณนจะถกนาไปโดย chorda tympani nerve แขนงของ CN VII

ทฝากมากบ lingual nerve

- ทง general sensation และ special sensation จาก posterior 1/3 ของลน ถกนาไปโดย

glossopharyngeal nerve (CN IX)

- บรเวณเลก ๆ ของโคนลน ทอยหนาตอ epiglottis ไดรบ sensory supplies ทงชนด general และ

special sense จาก internal laryngeal nerve ทเปนแขนงจาก vagus nerve

นอกจากน sensory nerve fibers

ทมายงลนเกอบทงหมด ยงนา

parasympathetic secretomotor

fibers มาเลยง serous glands ในลน

และ lingual nerve ยงนา

parasympathetic fibers จาก chorda

tympani nerve ไป synapse กบ

submandibular ganglion เพอให

postganglionic parasympathetic

fibers ไปเลยง submandibular และ

sublingual salivary glands

ลนทาหนาทรบรสพนาาน 4 ชนด คอ หวาน, เคม,

เปรยว และ ขม โดยมบรเวณทจาเพาะ ตอรส แตละชนด

ดงน

- รสหวาน – apex (tip)

- รสเคม - lateral margins of apex

- รสเปรยว - lateral margins

- ขม – posterior part

Arteries of the tongue เปนแขนงจาก lingual artery ซงแยกมาจาก external carotid artery ผาน

เขาทางดานขางของลนแตละขาง โดยผาน deep to hyoglossus muscle ใหแขนงแยกออกดงน

- Dorsal lingual arteries เลยง posterior part ของลน และยงให tonsillar branch ไปเลยง palatine

tonsil

- Deep lingual artery เลยง anterior part ของลน (สวน body)

- Sublingual branch of the lingual artery ซงเลยง sublingual gland และ floor of the mouth

Vein of the tongue มทาง drain กลบ 2 ทางคอ

- Dorsal lingual veins ทอดคกบ lingual arteries

- Deep lingual veins (ranine veins) ตงตนจาก apex ของลนทอดไปดานหลง มองเหนจาก

ดานใตลนสองขางของ lingual frenulum ไปเชอมตอกบ sublingual vein ซงรบ drain กลบจาก

floor of the mouth และ sublingual salivary gland แลวไปรวมกบ dorsal lingual veins กอนเปด

เขาส internal jugular vein (IJV) (บางรายกไมรวม แตจะตางคนตางเปดเขาส IJV)

ในกรณของผปวย angina pectoris (chest pain) จะใหอมยาใตลน หรอพน spray ในบรเวณน เพอให

ยาดดซมเขาส deep lingual veins ในเวลาไมถงนาท

Lymph จากลนมทางลาเลยง

4 ทาง ดงแสดงในรปท 26 โดย

นาเหลองจากบรเวณใกลแนว

midline จะ drain bilaterally

Salivary glands

ตอมนาลาย ม 3 ค ใหญ ๆ คอ

1. Parotid glands

เปนคทมขนาดใหญทสด อยหนาห สราง purely serous secretion มทอมาเปดท

vestibule of oral cavity บรเวณทตรงกบ upper 2nd molar teeth

2. Submandibular glands

ทอดขนานตามยาวของ body of mandible ม 2 สวน ขนาบอยดานตน และดานลกของ

กลามเนอ mylohyoid มทอ submandibular duct ยาวประมาณ 5 ซม. ทอดแทรกไประหวาง

กลามเนอ mylohyoid และ hyoglossus ม lingual nerve ทอดคลองอยดานลาง รเปดของ

submandibular duct มประมาณ 1-3 ทอ เปดออกท sublingual papilla ทอยชดโคนของ

lingual frenulum

3. Sublingual glands

เปนตอมนาลายทมขนาดเลกทสด และอยลกทสด แทรกอยระหวางกระดก mandible

และกลามเนอ genioglossus สองขางจะเชอมตอกนเปนรป horseshoe – shaped ขนาบทางดาน

ขางของ lingual frenulum มทอขนาดเลกหลายทอเปดท floor of the mouth ตามแนวของ

sublingual folds
