Required: Home School Community Tool Optional: Sample Questions for Strength Assessment – Family: Wraparound Coffee Chats Guiding Questions Optional: Consider use of Strengths Needs Profile to document results. Facilitator documents parent consent on Tier 3 Informed Consent. Log documentation of consent in Infinite Campus (IC) under RtI contacts. Crisis/safety plan is developed if needed. Facilitator guides family/student in selecting a team. Enter Tier 3 Behavior Educational Wraparound plan in IC. Progress monitoring of Phase One may include continued monitoring of a Tier 2 intervention, monitoring attendance, DPR cards, or monitoring work completion. 4 Wraparound facilitator completes Education Information Tool in interview with teacher/appropriate school staff. 5 Wraparound facilitator completes the following sections of the Student Action Plan and Progress Update:

Strengths

Safety/Crisis Plan (if needed)

Big Needs StatementsReview these items with the parent/family before the first meeting to ensure that this represents family voice. Facilitator creates ground rules and first meeting agenda with family.

6 Wraparound facilitator arranges first meeting, selecting a time/place that is comfortable for family.

Transition Protocol: If you become aware that a student transfers to a different school in MPS for any reason:

1. Complete the Tier 3 Student Discharge Tool. Link is below.2. Create a contact log in IC under RtI, stating student was receiving a Tier 3 intervention at the sending

school.3. Attach the Tier 3 Student Discharge Tool in RtI tab.4. Contact the new school’s building intervention team (BIT) coordinator as soon as possible, and

notify him/her that the student received or is receiving a Tier 3 intervention.5. Ensure that Action Plans are attached in IC under RtI.6. Transport maps to the new school, if a RENEW student.

The BIT that is receiving the student: 1. Determines some level of support for the student.2. If the team determines that the student should continue with the Tier 3 intervention, inform a trained

facilitator in the intervention of the referral.

Phase 2: Initial Plan Development

Step Objective Date Completed

1 Initial Wraparound meeting: Parent/guardian must participate in the initial Wraparound meeting in some capacity (i.e., in person or phone conference). Team determines progress monitoring.

1. Facilitator introduces ground rules that have been developedwith the family.

Using the Student Action Plan and Progress Update:2. Team completes sign-in sheet.

3. Facilitator reviews student strengths and adds others, if moreare identified by team.

4. Facilitator reviews Big Needs, and the team prioritizes these.

5. Team develops a Mission Statement. This addresses themost important, overall quality-of-life needs of the studentand family. Check for family voice.

6. Team uses the Section 6: Wraparound Plan to generatestrategies to meet the student’s needs and to identify memberswho are responsible for each strategy. Check for family voice.

2 Wraparound facilitator:

Schedules next team meeting.

Distributes Section 6 Wraparound Plan to each team member. Holds meeting with school administration to discuss case

(i.e., expectations, supports, progress monitoring).3 Optional: Family completes Family–Caregiver Satisfaction Tool.

Phase 3: Plan Implementation and Refinement

Step Objective Date Completed

1 Team continues to meet, review progress, and modify strategies as needed. Items from Student Action Plan and Progress Update are reviewed at each meeting.

2 Team continues to monitor progress in Infinite Campus (IC).

3 Facilitator provides a copy Section 6 Wraparound Plan to all team members at the end of the meeting. Meet with school administration to discuss case (i.e., expectations, supports, progress monitoring).

Phase 4: Plan Completion and Transition

Step Objective Date Completed

1 Team meets and discusses slowly “unwrapping” and transitioning to lower-level interventions.

2 Team discusses access to future services and provides parent(s) with a list of contact information for team members.

3 Facilitator provides a copy of the completed plan with all team members at the end of the meeting.

4 Optional: Team completes Tier 3 Student Discharge Tool.

5 Tier 3 plan is ended in IC. Student Action Plan is added as an attachment in IC.
