1. Organize artwork by class, place each class in a separate folder, and label each folder tab with total

number of art per class, grade, and last name of teacher.

2. Insert this completed form into box with organized art & folders.

3. Add filler to pack tightly to minimize damages.

4. Apply the UPS shipping label, provided by Square 1 Art by email, to the box. DO NOT DROP AT U.S. POST OFFICE.

5. If UPS already comes by your school, give box to UPS driver. 6. If you did not receive label email (check spam folder), need an additional shipping label, or have any questions concerning the UPS shipping method, contact Square 1 Art Customer Care at 888.332.3294 x 2.

Check artwork header and folder tabs. If we cannot read the artwork header identification easily, misspellingscould occur on artist’s personalized catalog. If necessary, overwrite information using a fine-tipped Sharpiemarker or use pre-printed labels.

If any of your original “Reservation Form” information has changed, please contact Customer Care beforeshipping art, so updates can be made to your account.

o Attach check/money order payment to each order excluding cash. DO NOT SEND CASH.

o Write “cash order”on order forms that had cash payments.

o Photograph or copy all orders for your records. Write payment type/amount on bottom of each order form.

o Double check order forms are filled out completely and have accurate quantities and payment totals.

Order/payment processing terms and conditions: DO NOT SEND CASH. S1A will simply deduct any shortageof monies not sent for those orders from your profit. S1A cannot be held responsible for lost shipmentsbeyond our control. Photograph or copy all orders and payments in the unlikely event of a lost shipment.

We do not accept late paper orders. Late orders can be made online/phone until deadline. Orders made after the deadline will be Retail Orders and are not a part of your fundraiser. Profit check will be sent via US Postal Service and arrive up to three weeks after receiving keepsakes. Shipping charge is applied to fundraiserswith less than 50 orders. 2. Apply the UPS shipping label emailed by Square 1 Art, to the gray, tough-bag or other padded envelope.
