Applications are invited fol engagement of Block ASHA Facilitator on purely contractual basisunder ASHA programme for Bishnupur Sub-Division. The Monthly salary of Block ASHAFacilitator will be Rs. 7500.00 (rupees seven thousand and five hundred) only. In addition, he/she will be eligible for a monthly mobility support of, Rs. 1500.00 (one thousand and fivehundred) only. Block ASHA Facilitators will be contractually engaged for a period of one yearrenewable based on budgetary sanction and performance.

1.1) Master's degree in Social Science/ Sociology/ Social Anthropology lsocial Work (MSW) /Business Administration (MBA) / Economics / Rural Development / Mass Communication

Graduate degree in any discipline with minimum2 years experience in health projects

Preference will be given to candidates having working experience in ASHA programme.

Knowledge in MS Office & Internet

Ability to communicate effectively

Willing to travel extensively

Should be resident of the same Sub-division where he/she is applying

Age should not exceed 40 years as on 01.01 .2}l4.Upper age relaxation will be 5 years forSC/ST and 3 years for OBC.

Selection Process:

The selection will be based on merit (academic result) and experience in health projects,written test and Computer Skill Test .Weightage will be given as per guideline vide G. O No -HFW / NRHM I 2721 2010 I 875,Dated -26102114.

Mode of application:-

1. Candidates should submit applications in the format annexed as "Annexure- A" to thisnotification. Application form will be downloaded from www.bankura.gov.in.

2. The application should be on good quality paper (Legal Size) using one side only.3. Candidates should ensure that the application is made on a single sheet conforming to the above

specification.

4. Candidates should fill up the application form in his/ her own handwriting in English with blueball point pen only.

5. Applicant must enclose the followinq documents with the application form:-

o Self attested photocopy of certificate as proof of the Date of Birth. (Only Admit CardlBoard'sCertificate of 1Oth class will be accepted.)

. Self attested photocopy of proof of residence (EPIC card/driving license/Bank Passbook

etc/Electric bill/telephone bill etc).

. Self attested photocopy of Mark sheet of Secondary or its equivalent examination.o Self attested photocopy of Mark sheet of Higher Secondary or its equivalent examination.. Self attested photocopy of Mark sheet of Graduation.

o Self attested photocopy of Mark sheet of Master's degree (if applicable).o Self attested photocopy of Computer Certificate.o Self attested photocopy of Proof of Experience (if any).

{Continuous engagement in a health related project with an assigned designation, duringthe entire tenure furnished as experience will only be considered as "experience in healthproject". No certificates specifying that the candidate has worked as volunteer e.g. in Pulse Polio,social work, leprosy etc will be considered as experience in health related project.)

o Self attested photocopy of Caste Certificate from Competent Authority (for reserved category).o Two self addressed envelope pasted with postal stamp @ Rs.5 on each envelope.o Demand Draft of Rs. 100"00 (Rupees One hundred) only for UNRESERVED categories and Rs.

50.00 (Rupees Fifty) only for RESERVED categories, issued from any Nationalized Bank infavour of "S.D.O. BISHNUPUR" payable at Bishnupur.

o A recent passport size colour photograph must be pasted on the application form.. On the envelope of application containing the application, the applicant must write "Application

for the post of Block ASHA Facilitator".6. Last date and address for submission of application

Application with all relevant documents must be submitted through post or applicant may dropthe application personally in the specific box placed at Office of the SDO, Bishnupur. The last date ofsubmission of application is 30/10/2015 up to 05:00 p.m.

7. Invalid applications:-

Applications suffering from the following deficiencies will be rejected. The list is onlyillustrative and not exhaustive.

. Application received after the closing date & time.

. Application not in prescribed format or which is incomplete in any manner or in nonstandard size of paper.

. Candidate is not in possession of the required educational qualification on the date ofapplication or is over aged as on 01 .01.2014.

o More than one application submitted by the same candidate.

. Application without the recent colour photograph affixing on it.

o Application without full signature.

o Copies of required enclosures as mentioned above (under point no 5) are not enclosed.

. Any other discrepancies noticed in the application at the time of scrutiny will be liablefor rejection of his / her candidature.

o Incomplete Application in any point of view will be summarily rejected.

8. General Conditions :-

. After verification, scrutiny of applications, the list of valid candidates, fulfilling theeligibility criteria will be published in ww:ylgilkuaggy$. Only eligible candidates

will be called for Computer and written test.

o Valid list of candidates and Notice for date, time, venue for written test and computer

skilled test will be published in rvi,vw.bankura..sov.in. Candidates are requested to visitthe website time to time regularly.

All original certificates will be verified at the time of computer test/written test.

No TA/DA will be allowed to attend the written test and computer skill test.

Mere selection and empanelment does not confer any right of appointment to the

candidates.

Admission of a candidate at all stages of recruitment will be purely provisional, subject

to satisfiring prescribed conditions.

Sub-Divisional Level Selection Committee of Block ASHA Facilitator reseryes the rightto alter the mode of examinations or conduct re-interview or cancel part or whole of any

process of recruitment at any stage, if needed.
