YourOrganization/District/SchoolhasrequestedyoucompleteanddigitallysignonlineAthleticPre-ParticipationWebForms.Studentsareabletocompletetheformsbeforeaparent/legalguardianaccountislinked.

Youwillseeacombinationofthefollowingtypesofformsrequiredatyourschool:

1. Web-form-thistypeofformallowsyoutoenterinformationonlineandresemblestheprinted-paperforms.Oncealltheinformationisfilledout,youclickontheSign&Submitbuttontodigitallysignandsubmittheformtoappropriatestaffatyourschool.

2. UploadOnly-thisallowsforpaperformsthatcannotyetbecompletedviaweb-forms.Forexample,thephysicalexamformconductedbyaphysicianorabirthcertificate.Theuploadoptionallowsyoutoscan/takeapictureoftheformanddirectlyuploadit.

• Eitherparentorstudentcaninitiatethecompletionoftheforms.IfthestudentcompletesanddigitallysignstheformfirstthenPlanetHSwillsendanSMSandemailnotificationtothelinkedparent,alertingthemapendingactionisrequiredfromthem.IftheparentcompletesanddigitallysignsaformfirstthenthelinkedstudentwillreceivetheSMSandemailnotificationalertingthemapendingactionisrequiredfromthem.Inadditiontothecourtesynotifications,bothparentandstudentcanloginatanytime,clickontheAthleticPre-Participationlinkreferencedabove,toviewformstatuses,reviewsignaturesneeded,andcompletenecessaryformrequirements.

WhathappensafterIdigitallysignoruploadaform?

MostPre-Participationweb-formsrequirethedigitalsignatureoftheboththestudentandparent. Ifarequiredsignatureisoutstanding,PlanetHSwillsendacourtesynotificationtotheremainingrequiredperson.ThispersonwillverifyresponsesareaccurateandthendigitallysigntheformbyclickingontheSign&SubmitFormbuttonatthebottomofthepage.Youwillseethedigitaltimestampandsignatureforanypersonswhohavealreadycompletedandsignedoffonthatparticularform.UploadOnlyformsonlyrequireeitherthestudentORparenttouploadtheform,notboth.

9. Ifallrequiredsignatureshavebeenmet,theathleticdirectororappropriatefaculty/staffmemberatyourschoolwillbenotifiedtoreview,acceptandfinalizetheform.

10. Reviewthestatusofeachformandrepeattheprocessuntilallformsarecompleteanddigitallysigned.
