To be able to generate a feedback model, we needed to mathematically express the Henderson- Hasselbalch equation for the haemoglobin buffer, the Haldane effect, to discover in this integrity the shift in the Haldane effect as well as the constant in the Haldane effect in order to make the mathematical transmission of the information in this feedback possible. The precise portion of the metabolic and respiratory components in this model is logically determined by the just discovered parameter – the shift in the Haldane effect. The lungs- inner environment feedback model basically simulates the processes participating in the Bohr Effect stating the integrity of the partial pressure of oxygen, carbon dioxide and the blood ph. This effect is comprised in the integrity of the relationships within the lungs-inner environment algorithm where two buffer systems that integrate the Haldane effect, are formed. This effect represents the relationship between oxygen and the current hydrogen carbonate. However, these relationships are precisely determined by the discovered parameter – the shift in the Haldane effect, by means of which the portion of the respiratory and metabolic components at a specific value of the partial pressure of carbon dioxide is identified. In the mathematical model of the lungs-inner environment feedback the relationships between the partial pressure of oxygen and saturation /HbO2/ are expressed by the Hill equation and its equilibration constant. The above-mentioned lungs-inner environment model type represents a basic cybernetic feedback of the inner environment as a new theory with “electronic blood” concept that introduces the possibility of simulating the processes of the inner environment cybernetics using a computer and its transformation into an invention.

We should mention that the newly defined parameter (“ !ph2 ”) – the shift in the Haldane effect forms one part of the Bohr effect application principle, and without this shift the technical solution would not have made sense. Until now, it has been an unknown parameter, where:

ph = 6.1 + ph1 + ph2

ph = blood ph

ph1 = Bohr shift

ph2 = shift in the Haldane effect (MN DA K)

The Bohr effect is also a part of the basic feedback algorithm of the relationship between: The external environment and of lungs, new theory and inner blood environment cybernetics (MN DA) as a precisely organised biological system. The overall process of parameter and feedback conversion takes place in the device’s ancillary module, which is our company’s developmental software.

Kind of feedback is the relationship of acid-base balance and ionic balance, contains a mathematical determination of individual ions Ca++, Mg++, K+ and in relationship to the current values of the blood pH. Regarding other ions, the determination includes the calculation of the values of Cl-, Na+. These re la t ionsh ips a re mathemat ica l mode l ing th rough “Gastroenterological model internal environment of blood”. Other parameters sugar, urea and osmolality are given by the modification of the formula:

Osmolality = 2Na+ + urea + sugar /glycemia/.

How it works? In order to obtain adequate and accurate results we need: a device for measuring the partial pressure pO2 and pCO2 (today we are useing the device TINA TCM4 Radiometer) software MN DA (which is connected directly to the unit TINA) and finally a computer unit, which displays the results. MN DA software communicates directly with the device TINA, acquires basic information p02 and pCO2 which are converted with our mathematical formulas, and then the software MN DA sends the resulting data on the computer.

Evaluation report of measurements made on continuous non-invasive MN DA analyzer compared to invasive method

Based on the Contract on cooperation with UVN SNP Ružomberok – University Hospital, the Vision Corporation s.r.o UVN SNP Ružomberok performed comparison measurements of MN DA software under the professional auspices of the Head of Out-Patient Hospital KAIM in central intensive care unit (CICU). The patients were informed about the device and overall procedure of measurement, signing an informed consent form to prove it. Comparison testing was performed on December 2, 2015 at a time strictly defined according to methodological specifications of measurements from 10:14 a.m. until 10:59 a.m. via an invasive method and subsequently, a non-invasive continuous visual method applied with visual and audio documentation. Blood sampling was performed via an invasive method by the chief CICU nurse, while blood sampling using a non-invasive method was performed by a professional deputy trained for non-invasive MN DA device handling.

Six samples with data verification were subject to verification. Age category of patients – 55 to 75; male to female ratio 1:1. Patients with following diagnoses were included: after surgery due to bowel obstruction; stenosis a. carotis; after surgery on femur fracture or acute pancreatitis.

Monitoring of internal environment is a basic requirement of resuscitation care. Patient is in a critical status and his/her life is in jeopardy. As a result of enhanced knowledge on ratios in internal environment and their relations with clinical nosological units, a notion of critical metabolic disruption was established. To prevent and treat such disruption, one needs knowledge about a pathophysiological chain of changes occurring in the internal environment. These are considered to be the basic biochemical parameters of intensive monitoring of the internal environment: acid-base balance (Astrup), cations, anions in serum, blood count and hemocoagulation factors. Such an "acute set" should be fully secured all day long in predetermined intervals. A classic invasive method is used for a capillary finger-prick or earlobe blood samples or for arterial blood sampling using a. radialis in most cases. As in all medical procedures, in above mentioned practices, we are also seeking to replace measurements with a non-invasive MN DA computer analyzer method using the parameters of acid-base balance, which is one type of transcutaneous cybernetic analyzers based on mathematical models of inner environment cybernetics for the non-invasive examination of patients’ internal environment. The results of measurements obtained by a non-invasive method only slightly differ from those collected through a classical invasive method; i.e. they are relevant and valid. New approach to patient comfort – non-invasive nature, while values measured are almost identical and applicable for the treatment of the patient and the continuous monitoring of patient’s internal environment parameters.

What is the future of this software?

Today, our software is ready to work in the hospital wards such as resuscitation, pre-operative and post-operative department, preoperative examination.

The measurement electrode is used which heats the skin to release the gas from the body. This electrode is set at one temperature and if we need to be changed we have to do it manually. And so we decided in the future to develop a so-called a thinking electrode that is able to recognize the skin thickness and automatically reduces the temperature.

Another change we would like to do with the calibration device that measures the partial pressure. In the event that the calibration will be electronically, size of the entire device, including our software and display output would change to the size of the current smartphone or tablet.

Another development we would like to be directed towards commercial and that is the reason we decide to do so-called Analytic medical watches that could be used by the general practitioners in their surgeries for fast and efficient measurement values of the internal environment of blood.

For home use such watches or medical assistant will be invented by a simpler version, which will measure the sugar and base values during hard activity. (eg. diving, mountain climbing, marathons ……etc).

What is the advantage of this system? Already today we can measure blood purely non-invasively (not as is presented by other companies), it means that we have undeniable product. The advantage is that this measurement is not painful and it is especially continuously. It gives the possibility of faster decision-making in patients' problems and lastly it is less expensive than continuously measuring invasive methods.
