• One phone call to Navigator to screen &direct families to appropriate care, including mental health, developmental services, medical care, social services needs
• Collaboration of 4 Local CBOs expert in working with young children and families
• Free evidence based Triple P – Positive Parenting Program training to ANY provider who works with children or families
• Free training to family practice providers in evidence based social/emotional screening

COLLABORATIVE (OAC) A six agency collaborative,
dedicated to the Prevention and Early Intervention (PEI) of
depression and suicide in the senior population. Service delivery is achieved by providing
culturally competent, case management
and mental health services to individuals residing in Sonoma
County.
