Step 1: Transition Planning at Initial IFSP

Service Coordinator will…. discuss transition planning at the initial IFSP

Step 3: Transition ReferralEach Service Coordinator is responsible for

sending directory information using transition referral form.

Referral must occur between age 2 (24 months) and 2.5 (30 months)

If parents consent, additional info can be shared.

If parents do not consent, directory information is still sent to school district.
