Sub-acute care facilitiesSub-acute facilities provide sub-acute care for the special nursing needs of patients who do not need acute care but are too ill to be cared for by most skilled nursing facilities (SNFs). This includes patients in transitional settings following acute hospitalization (such as those discharged from hospitals but not fully recovered).

Definitions of General Medical SpecialtiesNot all specialties are offered in all regions.

Acupuncture: Stimulates certain points on or near the surface of the body by the insertion of needles to prevent or modify the perception of pain or to normalize physiological functions, including pain control, for the treatment of certain diseases or dysfunctions of the body.

Adolescent medicine: Practice specializing in the evaluation, treatment and management of health problems associated with the adolescent stage of development. A subspecialty of Internal medicine.

Allergy and immunology: Diagnosis and treatment of allergic conditions and diseases of the immune system.

Anesthesiology: The delivery of anesthesia to achieve a painless state for the patient undergoing surgery or other painful procedures.

Board certification: Board certification in a medical specialty is a process that requires the doctor to successfully complete an approved residency training program and pass a comprehensive examination in that particular specialty. Doctors may be board certified in more than one specialty.

Cardiology/Cardiovascular disease: Diagnosis and treatment of the diseases of the heart, lungs and blood vessels. A subspecialty of Internal medicine.

Cardiovascular surgery: A specialized branch of surgery related to organs within the chest cavity, including the heart, major blood vessels, lungs, and esophagus.

Chiropractic: Medical practice based on the theory that diseases result from a disruption of nerve function. This interference is thought to stem primarily from displaced vertebrae, which chiropractors massage and manipulate manually in order to relieve pressure on nerves.

Dermatology: Diagnosis and treatment of skin diseases and disorders.

Dermatopathology: The use of microscopy, immunohistochemistry and laboratory management in the evaluation and treatment of skin disorders. A subspecialty of Dermatology.

Note: The applicable waiting time for a particular appointment may be extended if the referring or treating licensed health care provider (or the health professional providing triage or screening services, as applicable), acting within the scope of his or her practice and consistent

with professionally recognized standards of practice, has determined and noted in the relevant record that a longer waiting time will not have a detrimental impact on the health of the covered person.

Services That Can Be Obtained without ReferralPPOHealth Nets Preferred Provider Organization (PPO) product does not require an enrollee or the enrollees treating provider to request approval for a referral of an enrollee to an in-network or out-of-network provider. Enrollee referrals to out-of-network providers are at both the enrollee and the enrollees treating providers discretion, as enrollees have coverage for out-of-network providers. While an approval for referral to an out-of-network provider is

not required, the enrollee will incur higher outof-pocket expenses when receiving services or supplies from an out-of-network provider.

This does not waive any prior certification requirements that may be applicable to the types of services obtained. Please refer to your Certificate of Insurance for more information about services requiring certification, or contact the Customer Contact Center at the number on your ID card.
