Please follow the structure of this template when preparing your entry. It has been designed to ensure that the important aspects of your work are presented in a way that will enable the experts to make an effective assessment against the evaluation criteria. Page limits: Part B should not be longer than 50 pages. You might be also asked to provide additional documentation during an onsite visit. Supporting documents such as tables, data sets, drawings, pictures and other clarifications that can be used when quantifying the results of the suggested solution in terms of key performance metrics can be provided as an annex and do not count towards the 50 page limitation.

For Part B the font allowed is Times New Roman and the minimum font size allowed is 11 points. The page size is A4, and all margins (top, bottom, left, right) should be at least 15 mm (not including any footers or headers).

If you attempt to upload an entry longer than the specified limits above, you will receive an automatic warning, and will be advised to shorten and re-upload the entry. After submission, any excess pages will be overprinted with a 'watermark', indicating to evaluators that these pages must be disregarded.

Please respect the page limit and do not take it as a target either! It is in your interest to keep your text as concise as possible, since experts rarely view unnecessarily long entries in a positive light.

1. OVERVIEW OF THE APPLICATION

1.1. The project team

Describe qualitatively the team that implemented the project.

1.2. The hospital

Describe the hospital building(s) and its (their) history.

1.3. The project

Describe the overall project, the main ideas, concept and assumptions involved. Explain the innovativeness of the solution.

2. DESCRIPTION OF THE INSTALLED ENERGY SYSTEM BEFORE THE PROJECT

Describe the solutions that had been previously (i.e. before this project) implemented for the supply of energy (heat, cooling and electricity) to the hospital. Provide quantified data on the energy consumed and its seasonal and daily variation before the project.

3. DETAILED DESCRIPTION OF THE PROJECT

3.1. The new combined heat and power system

Describe the new system chosen to provide energy (heat, cooling and electricity) to the hospital. Present the quantified theoretical data of the system. The contribution of each renewable energy technology needs to be clearly presented. The energy storage component needs to be clearly presented. Describe the assumptions leading to the solutions.

Present the time line of your project from inception to operation with the major milestones.

Present the results of the operation of the new energy solution. Provide quantified data on the energy produced and consumed, on the overall efficiencies and on the duration of operation. Present the seasonal and daily variations. If it is relevant, present relevant information on meteorological parameters during the period of operation (e.g. quality of the solar resource, quality of the wind resource).

3.2. Reliability, easy maintenance and safety of operation

Provide a description of and quantified information on the incidents occurred and interventions needed during the commissioning period and during the period of operation of the new installation.

4

3.3. CO2 emission reduction and sustainability aspects, potential energy savings

Provide relevant quantified data to compare the new and the old installation. If biomass is used in the new installation, provide relevant information on biomass logistics.

3.4. Minimal/non-invasive impact on premises

Provide information on the quality of the integration of the new energy system on the hospital premises through drawings, sketches and photos.

3.5. Low operation and maintenance costs

Provide a cost analysis of the new energy system and a comparison with the previous system.

3.6. Involvement of the public

Describe how the potential users (e.g. medical staff and patients) and the neighbourhood (e.g. inhabitants, municipal authorities, NGOs) were involved, how they interacted with the project and how their opinion was taken into account.
