This document will help you understand specifics about your medical and, if applicable, dental plan renewal and changes as we move from 2016 to 2017. The enclosed letter goes into more detail.

HEALTH PLAN RATE ADJUSTMENTS

We do all we can to keep rates as low as possible while maintaining great benefits and the highest quality of care. Rates go up due to several factors and, this year, those include changes in the Washington marketplace, rising medical and pharmaceutical costs, and a higher number of claims by plan users than predicted. The rate you pay is also determined by where you live, your plan choice, and age. IMPORTANT DENTAL PLAN INFORMATION

About pediatric dental coverage

• If you have family members on your medical plan under the age of 19, pediatric dental coverage is required.

• If you have chosen to cover family members under the age of 19 on another plan, you’ll need to attest to this and send in proof annually of the other pediatric dental coverage to Kaiser Permanente within 60 days of Jan. 1 2017.

– The Pediatric Dental Attestation (proof of dental coverage on a different plan) can be found at kp.org/wa/if.

– Complete and email it to [email protected] or send in a paper copy to Kaiser Permanente, Individual & Family Plans, 320 Westlake Ave. N, Suite 100, Seattle, WA 98109.

If you purchased your medical plan directly from Kaiser Permanente (formerly Group Health), here’s how to keep, add, change, or cancel dental coverage. If action is required, the deadline is Dec. 31, 2016 for coverage on Jan. 1, 2017.

IMPORTANT: If you are currently enrolled on the HealthPays Connect Bronze HSA plan that will be discontinued on Jan. 1, 2017, use the above Dental options grid to determine your coverage needs for 2017. Since pediatric dental coverage will no longer be embedded in your medical plans in 2017, you will need to ensure you have pediatric dental coverage by Jan. 1, 2017 for dependents under the age of 19.
