Outline of the training programme in North Trent

The programme is sufficiently flexible to meet the individual training needs of each Specialist Trainee. Education plans will be agreed at yearly intervals to enable the Specialist Trainee to obtain specified clinical experience to meet defined preset training objectives as defined in the new Acute Medicine curriculum. Specialist Trainees will be based at each hospital for 6 months but may work with different consultants during that period of time in order to achieve training targets.

The training period will last three to four years, at the end of which the Specialist Trainee will obtain a CCT in Acute Medicine. It is envisaged that each Specialist Trainee will spend two to three years in a district general hospital and one to two years in a teaching centre. The four year training period may be reduced if competencies according to the new curriculum for Acute Medicine are/have been achieved sooner.

Completing the curriculum

Training will be targeted to complete the required elements of the new curriculum for Acute Internal Medicine. This will be provided by clinical attachments to specialities including:

The Specialist Trainee will continue to participate in emergency work in acute unselected general medicine for the duration of the training period, unless on dedicated secondment. This will mean participating in the medical on-take Specialist Registrar rota, and in most Yorkshire and the Humber centres this will involve full shift working. Educational timetables will include a commitment to continuity of care in ward areas for a substantial portion of the patients admitted, depending on local arrangements. Some of the Trusts now have ward-based systems which inevitably restricts the opportunity for continuity of care.

The weekly timetable will include attendance at outpatient clinics with the opportunity to see new and follow-up patients both in the speciality and on an unselected basis.

Generic Teaching

Deanery

The Deanery provides educational courses for the generic curriculum including management, teacher training, ethical and legal issues courses.

Individual opportunities for audit will be included as usual within the Specialist Training programme, and specific research relevant to training in General Internal Medicine and Acute Medicine will be encouraged.

General Internal Medicine

All Specialist Trainees training in the medical specialties are required to attend 75% of the formal training programme in GIM comprising two full day CPD/Specialist Registrar training days, and five half day Specialist Registrar training days on a rolling programme. These training events are case-based and interactive. Time is devoted to generic aspects of the curriculum within the programme.

Acute MedicineAll Specialist Trainees are expected to attend the monthly teaching days organised within the SpR rotation. It is expected that SpRs will make every effort to attend the Regional Updates in Medicine and also attend at least one of the two annual Society for Acute Medicine (UK) conferences.

Our Hospitals

Although the Trust has overall management responsibility for the services provided, our hospitals each have their own history, which we are rightly proud of. Between them they offer just about every kind of service available in the modern NHS with over 12,000 members of staff working within them.

The Northern General is the largest of our hospitals with 1100 beds. Located to the north of the city, the hospital is spread out over a large site. Amongst its specialist services are orthopaedics, spinal injuries and heart and lung services. Sheffield’s accident & emergency department is also based here.


The other Trust hospitals are based closely together on a campus to the west of the city centre.

The Royal Hallamshire is a major acute teaching hospital based around a ‘tower block’ of 760 beds and a large outpatient department. Specialist services here include neurology, urology and ophthalmology. Linked to the Royal Hallamshire by a footbridge, the Jessop Wing is Sheffield’s 260 bed women’s hospital. Opened in February 2001 it has brought together all of the city’s obstetric, gynaecology and neonatology services in a state-of-the-art, purpose built, £24 million building.

Weston Park is one of only three purpose built specialist cancer hospitals in the UK and provides a full range of non-surgical cancer services to the population of South Yorkshire, North Nottinghamshire and North Derbyshire. It also enjoys national and international recognition for certain specific treatments. It has 112 beds and is home to a specialist cancer research centre.

The Charles Clifford is a dental teaching hospital linked to the Sheffield University School of Clinical Dentistry. The hospital provides dental outpatient services and emergency dental services for the city.

As a teaching Trust, all of the hospitals have close links to the University of Sheffield, home to Sheffield’s medical school, and Sheffield Hallam University. It is a major teaching centre for future health professionals.

Emergency Medicine - Northern General Hospital

The Emergency Department at the Northern General Hospital sees over 80,000 patients a year and the majority of patients admitted to hospital following attendance are medical patients.

This six month post on the rotation will offer the successful candidate exposure to a full range of medical emergencies.

The SpR will be supervised by two Emergency Physicians working in the Department currently.

The duties of the post include:-

1. Providing an experienced medical input into the management of medical patients attending the department.

2. Supervising the SHOs from the Emergency Department and General Medical and managing the emergency medical take.

3. Facilitate the use of early investigation where necessary to shorten hospital stay and engaged in discharge planning from the point of admission.

4. With the Consultants in Emergency Medicine, develop strategies to avoid unnecessary hospital admission and make full use of emergency out patients and other specialist clinics in the community.

5. The Registrar will be expected to spend at least one session on the medical admissions unit participating in the post take ward round and general patient management activities. Emphasis again will be upon promoting efficient working practices and early discharge.

6. Have responsibility for patients seen within the Clinical Decisions Unit which has been established in the Department. Patients using the Clinical Decisions Unit are those with low risk chest pain, DVT’s and pulmonary emboli. The appointee will be expected to give daily support to the nursing staff-managing patients within this facility.

7. The Registrar will also participate in the clinical management team overseeing the management of the Emergency Department and medical admissions unit.

8. The successful candidate will have the opportunity to develop teaching skills and will be expected to participate in the training and education of SHOs and medical students in the Emergency and General Medicine Departments and medical students. There will also be the opportunity to teach on ALS courses.

9. The Emergency Department has two Senior Lecturers and one clinical lecturer who are engaged in active clinical research. The Emergency Department has a track record for producing good clinical research and the successful candidate will be encouraged to participate in this.

10. O/C duties will be to the Emergency Department, hence the successful candidate will have to have past experience of A&E medicine and become an ATLS provider.

Medical Assessment Unit, Acute Medicine, Northern General Hospital

The Medical Assessment Unit at the Northern General Hospital sees approximately 12,000 new acute admissions every year. This post provides experience and input into the management of these patients on the Medical Assessment Unit. The post holders work will be supervised by the


Acute Physicians and those Consultants providing unselected General Medicine input on post take ward rounds.

The duties of the post include:

1. Providing experienced medial input into the management of medical patients admitted to the MAU.

2. Supervising the SHOs from MAU and managing the emergency medical take.

3. Facilitate the use of early investigation to ensure reduced hospital stay in the discharge planning from the point of admission.

4. With the Consultants in Acute Medicine develop strategies to avoid unnecessary hospital admission and make full use of emergency out patients and other specialist clinics in the community.

5. SpR will be expected to review those patients that have stayed on MAU more than 24 hours to ensure that appropriate continuity occurs and that problems are quickly identified.

6. The Registrar will have ongoing responsibility for those patients admitted to MAU regardless of the time of day during his or her working hours and provide senior medical input into those patients whose clinical need dictates it.

7. The successful candidate will have the opportunity to develop teaching skills and would be expected to participate in the training and education of SHOs and medical students on the MAU.

8. Participation in the Acute Medicine clinic is encouraged.

9. The successful candidate will be encouraged to participate in the clinical audit and research project of his/her choice with the help and supervision of the Acute Medicine Consultant and the Senior Lecturers in Emergency Medicine.

10. On-call duties will not normally be part of this persons duties but, because the service is developing all the time, this may be required in accordance with the on-call duties of existing General Medicine and specialty Registrars

Critical Care, Northern General Hospital

This attachment is based on HDU/ITU at the Northern General Hospital. This is an extremely busy 16 bedded unit with plans underway to build a purpose build critical care block housing up to 35 critical beds in the near future.

This post provides an unparalleled opportunity to receive education and training in Critical Care and will primarily be based on HDU. Whilst on HDU, the Trainee will receive educational supervision from the Critical Care Consultants and participate in ward and outreach work.

At present this post is unbanded but opportunities to participate in the on-call rota (with appropriate remuneration) may be provided.

It is expected that the Trainee will maintain a weekly link with Acute Medicine as well as participating in Critical Care liaison with acute medical patients.

ROTHERHAM GENERAL HOSPITAL NHS TRUST

THE HOSPITALS

Rotherham General Hospital is a third wave Trust, the services are managed by Clinical Directorates, of which there are 11. The General Hospital is situated 2 miles from the town centre, was opened in 1978, expanded in 1984 and 1994 and now has over 800 beds on this site.

With the exception of Psychiatry, all of the hospital’s specialities are run by Rotherham General Hospital NHS Trust. The Psychiatric and Community Services are run by DASH T.

This is a busy District General Hospital with 9 operating theatres (2 of which are ultra-clean air) for Orthopaedics, an Intensive Care Unit, HDU and a Coronary Care Unit. In addition to acute Medical, Surgical, Orthopaedic, Obstetric and Gynaecological, Paediatric and Anaesthetic Departments, the hospital has departments of Ophthalmology, ENT Surgery, Maxillo-Facial Surgery, Dermatology, Genito-Urinary Medicine, Rheumatology and Urology.

There is an x-ray department with modern CT, ultrasound scanning facilities and on-site MRI facilities.

A comprehensive laboratory service is on site as well as an active Medical Physics Department. There is an Education Centre in which regular postgraduate meetings are held. A well-stocked library with an excellent information service and an active Medical Illustration department are available. There is an admissions ward sited adjacent to the Accident & Emergency department.

GENERAL

The hospital has a catchment population of approximately 252,000, principally comprising the Metropolitan Borough of Rotherham. Geographically this is a compact district covering approximately 100 square miles and, with good communication links, patients can travel easily from all over the district to the General Hospital.

Although the area is mainly industrial, the long-standing emphasis on coal, iron and steel is diminishing and is being successfully replaced by developing light and service industry. There are many surrounding localities where farming still predominates and there are a number of quiet villages within the required distance of residence. The hospital is situated approximately two miles from the centre of town in one of the extremely pleasant suburbs.

Housing in the Rotherham area is generally reasonably priced with a wide range of choice in terms of type, price and urban or rural locations.

Sheffield lies only six miles away and provides good cultural and sporting facilities. Meadowhall shopping complex lies between the two centres. A wide range of recreational, leisure and cultural activities are provided for in and around the area. In addition to the normal range of facilities, Rotherham is well placed for access to the Peak District National Park, the southern Pennines, for day visits to coastal resorts, for areas such as the Yorkshire Dales or the various historical centres in the East and West Midlands. As a consequence of Sheffield hosting the 1991 World Student Games, the area now offers many international-standard sporting facilities.


Rotherham is served by excellent public transport facilities, with regular rail services to all parts of the country via Sheffield or Doncaster. There is a well-developed road system connecting with the M1 and M18 motorways, which intersect the area. There are international airports within easy travelling distance at Manchester, East Midlands and Leeds/Bradford.

Leeds, York, Nottingham, Manchester, Lincoln and Hull are all within a 50-mile radius of Rotherham.

Rotherham District General Hospital - Diabetes/Endocrinology

General Medicine

The firm provides a general medical input on call for unselected medical emergencies admitted through this busy District General Hospital.There are usually about 25 admissions per day.There are two post-take ward rounds – one in the evening (supervised by the Consultant in Acute Medicine) and one the following morning by the Consultant on call.The trainee will be on a full shift rota and work 1 in 9/10 giving 4 acute take session per month.

The trainee will supervise the management of ward patients.There is a weekly out patients clinic for follow up of general medical and endocrinology patients and a weekly new patient clinic for general medical and new referrals in diabetes and endocrinology.

Diabetes and Endocrinology Unit

General Internal Medicine / Acute Medicine on the Medical Admission / Assessment Unit at Rotherham Hospital

The rotation will meet the training objectives for both a clinical and managerial role for a trainee who wishes to take a lead role as an Acute Physician on the Medical Assessment / Admission Unit. This is a 29-bedded unit (combined medical/ elderly medicine/ surgical / A&E), has a high turn over of patients with a continuous review of patients

Duties of the post:

b) Unselected take of above 16 years, averaging about 40-50 admissions a day with a wide variety of acute problems referred from GPs and A&E. This would mean involvement in Resus in A&E, HDU, ITU and CCU;

c) Supervising the junior medical team on-call;

d) Attending the daily post-take ward rounds/ continuous reviewing of patients;

e) Attending two general medical outpatient clinics;

f) There is a daily assessment clinic run Monday to Friday in the 3-bedded Assessment Bay seeing patients referred by GP and A&E needing urgent medical opinion, targeted diagnostic tests and discharged back to the community on the same day with a clear line of management. This is an opportunity to develop skills (relevant for an acute physician)

g) On-call duties are current 1:9 (split rota)

Participate in training and education of junior doctors in General Medicine and medical students posted in the unit; also, an opportunity to teach on ALERT course. Involvement with the multi-disciplinary team in place consisting of physiotherapists, occupational therapists, speech & language therapists, pharmacy-led management service, access to CARATS (Community Assessment Rehabilitation and Treatment Service) for assessment and discharge back to community.

The trainee will be encouraged to attend regular management meetings within the unit and also within the Division of Medicine, regular audits and research.

BARNSLEY HOSPITAL NHS FOUNDATION TRUST

Barnsley Hospital NHS Foundation Trust serves a population of 220,000 and has a complement of 510 beds. There is a block theatres, together with an intensive therapy unit and other supporting services. Pathology services are provided in purpose built laboratories. There is a medical physics department, a department of physiology measurement and full rehabilitation facilities.


Located just over one mile from Junction 37 of the M1 motorway and only 15 minutes walk from the town centre, Barnsley Hospital NHS Foundation Trust is a modern 510 bedded hospital providing the principal acute services for the district. Barnsley Hospital became an NHS Trust on 1 April 1993. Since that time it has continued to develop and expand its services and became one of the first Foundation Trusts on 1st February 2005. The Trust is a single site hospital, employing 2.300 staff and operates with a budget of approximately £90 million. It has been awarded Associate Teaching Hospital Status in recognition of its contribution to medical education. The hospital serves a population of approximately 220,000 and the area it services is coterminous with that of the Barnsley Metropolitan Borough Council. Barnsley Primary Care Trust is the Trust’s major purchaser with only 5-10% of activity being purchased by other commissioners.

The hospital has close links with Sheffield University Medical School and undergraduates are taught in the hospital.

The hospital has comprehensive clinical specialities, including Emergency Medicine, Surgical, Medical, Paediatric, Obstetrics and Gynaecology and other specialists units.

The Trust provides excellent facilities including refurbished Medical Illustration Facilities, including a part-time medical photographer, a Clinical Audit Department and a newly well-equipped Postgraduate Medical Education Centre and Medical Library. A Multidisciplinary Team Meeting Room is located in the Post Graduate Centre and facilities are available for projection of histological slides and radiological images together with IT support.

The Trust has an extensive R&D infrastructure and has been recognised for its excellent Research and Development capability by receiving one of the largest percentage increases in funding allocation in the country.

Library Facilities / Postgraduate EducationThe education centre is based on the Barnsley Hospital site, but is separate from the main hospital. Its facilities include a state of the art lecture theatre and a mixture of seminar, tutorial and classrooms. The education centre is the central point for the education activity of Barnsley District General Hospital. It is now being developed as a facility to support education activity for the whole health communication of Barnsley. It is also the focal point for library and information services for the Barnsley health community. There is a well-stocked medical library within the education centre. A professional librarian is in attendance and books are available from the national collections. The University library is available in Sheffield, the medical section being situated in the Royal Hallamshire Hospital.

Other FacilitiesThere is a hospital sports and social club located within the grounds of the DGH. Doctors’ sitting room and television is also provided in the medical officers’ residential block. Car parking is provided within the hospital grounds and public transport from and to the town centre is excellent.

DONCASTER ROYAL INFIRMARY

The Doncaster Royal Infirmary is a modern 837 bed acute District General Hospital which, together with Mexborough Montagu Hospital, comprises the specialities of surgery, obstetrics and gynaecology, medicine, ophthalmology, ENT, oral surgery, paediatrics, rheumatology, traumatic and orthopaedic surgery, dermatology and genito-urinary medicine, together with a major Accident


& Emergency Department and Intensive Care Unit. In addition, there are twice-weekly oncology clinics and weekly plastic surgery clinics and cardiothoracic surgery clinics. There is a large medical imaging department, with 24-hour CT scanning facilities. Acute psychiatric services are available on site.

The hospital has an active postgraduate education centre and a well-stocked library.

The Accident & Emergency department, which is part of the diagnostic block, is large and well-conceived in design with reception area, waiting room, trolley area for 13 patients and a resuscitation room able to take 4 patients. In addition there are 5 cubicles for “walking wounded” and a children’s room. There is also a children’s play area. A purpose-built decontamination room is situated near the resuscitation room.

The department is able to offer a wide range of adult and paediatric accident and emergency experience.

Accident & Emergency is part of the Trauma and Orthopaedic Directorate with a Clinical Director responsible for the management of the directorate to the Trust Management Board.

THE METROPOLITAN DISTRICT OF DONCASTER

The Metropolitan District of Doncaster covers an area of approximately 200 square miles and is part of the South Yorkshire Metropolitan County, the other districts being Sheffield, Rotherham and Barnsley. The local authority headquarters are located in Doncaster, which provides a natural focus for the district and which expands from Askern in the north to Bawtry in the south and from Thorne in the east to Mexborough in the west.

Communications and transport are good, with easy access to the A1(M), M1, M18, M180 and M62 motorways, providing convenient motorway access to the rest of the country, including airports. Doncaster is also more than well served by public transport and, in particular, the British Rail high-speed train (Intercity 1256) providing a direct service to London in a matter of only 90 minutes. National coach systems provide regular services on the London/Edinburgh route and local buses run frequently, both within the town and to surrounding villages.

Doncaster town boasts a first-class shopping centre with branches of most of the great multiples and Doncaster market, held each Tuesday, Friday and Saturday, is famous throughout the north. A new shopping centre, the Yorkshire Outlet, has opened at the side of the M18 link. The Meadowhall Shopping Centre is only half an hour’s drive away, providing one of Europe’s larges shopping arenas.

Housing in Doncaster and surrounding districts is cheap and plentiful with availability of excellent property at prices, which compare extremely favourably with most other areas of Britain.

The town is served by schools located near all the main housing estates and by the Institute of Higher Education situated in the centre. There is an excellent public library, museum and art gallery. A multi-million pound leisure park (The Dome) has been opened in the town and contains all the latest recreational and leisure facilities (including the largest indoor complex in Britain). Doncaster is also proud of its Grade 1 race course which is the home of the St Leger, the world’s oldest classic race, first run in 1776. On non-race days, the race course is used as an exhibition venue where the Doncaster Motor Show is staged every year.

This post will offer the successful candidate 1 in 5 acute general medical exposure shared with the SPR in cardiology supervised by two new GIM consultants as yet not appointed. In addition cardiology experience under the supervision of 4 consultant cardiologists.

The duties of the post will include:

1) Providing experienced medical input to the medical on take patient shared on a 1 in 5 basis with the cardiology SpR.

2) Perform coronary care ward rounds both supervised by the cardiology consultants and on there own.

3) Will look after new patient referrals to the rapid access chest pain clinics and other suitable new cardiology referrals. In addition will see cardiology follow ups gaining experience in all aspects of cardiology.

4) At Doncaster Royal infirmary we have on site permanent pacing, coronary angiography, echocardiography, nuclear medicine, 24 hour monitoring, exercise testing, The successful candidate will be expected to familiarise themselves with all aspects of invasive and non invasive cardiology although not necessarily be able to perform these procedures.

5) Educational opportunities include a weekly grand round, journal club. Audit is carried out on a monthly rolling half-day programme and the successful candidate will be expected to participate with a suitable audit project.

* The fixed sessions will be cancelled to accommodate the on call sessions which will rotate through the week. During the flexible sessions it will be possible to watch /learn echocardiography pacing angiography depending on the interests of the candidate

Administrative Duties

Specialist Registrars are expected to undertake administrative duties as required.

Research

Protected time for study and research will be provided in accordance with the regulations currently in force for Specialist Registrar training. Specialist Registrars are encouraged to carry out research and will receive guidance as necessary.

It is expected that the programme will expand in 2009 to include extra acute medicine placements at Doncaster and Bassetlaw Hospitals.
