ANNUAL MEETING UNRESTRICTED GRANTSCompanies may contribute unrestricted educational grants to the AnnualMeeting education Fund to underwrite the cost of the meeting. Contributionsstart at $1,000. Companies that contribute at least $7,500 are eligible to selectbooths in sponsorship row if they wish and if their grant has been received byMay 1. Support for the following opportunities cannot be accepted for a singleeducational activity (e.g. one lecture or workshop), but can be accepted todefray the expenses for the Main Podium program and Hands-on Workshops,Allied Staff Programs, Poster Sessions, and Table Clinics.

CORPORATE PRESENTATION OPPORTUNITIESNew Trends, Techniques and Technology Programs (Available to GoldSponsors and higher only) are 20-minute lectures available to Gold Level orhigher sponsors on October 23, 2013 to the full audience in attendance. The AAID Annual Meeting Committee has finalapproval of the company presentations.

Hands-On Workshops: (Available to Presidential Sponsors and higher). 90-minute or 180-minute half-day length with a limit of 25 to 50 attendeesdepending on the program. Restricted to hands-on and designed to familiarizeattendees with particular techniques or materials. At least 75 percent of classtime must be devoted to hands-on to qualify. The AAID Annual MeetingCommittee has final approval of the company presentations. AAID will assign aroom. EXHIBIT SET-UP AND STAFFINGWednesday, October 23, 2013 – 2:00 pm - 8:00 pmAccess to the exhibit hall begins at 2:00 pm, and all exhibits must be setupby 8:00 pm on Wednesday, October 23. As a courtesy to the attendees andfellow exhibitors, please open your exhibits on time and staff them throughoutthe meeting. Exhibitors will be allowed in the exhibit hall two hours before theexhibits open each day and may remain in the hall one hour after closing.

EXHIBIT DISMANTLINGSaturday, October 26, 2013 – 1:00 pm - 5:00 pmNo packing of equipment or literature, or dismantling of exhibits is permitteduntil exhibit closing time, 1:00 pm, Saturday, October 26. Any exhibit notdismantled by 5:00 pm - or for which arrangements for tear-down have notbeen made - will automatically be dismantled at the exhibitor’s expense andliability for proper dismantle. The floor must be cleared by 5:00 pm. Exhibitorswho dismantle early without express permission of show management willforfeit all points earned at the 2013 meeting.

ABANDONED SPACEAny space not completely set-up by 9:30 am on Thursday, October 24, exceptwith express permission of AAID Show staff, or last in, first out as noted onfloor plan, will be regarded as abandoned and may be resold or reassignedby AAID. Each exhibit must be open for the full duration of show hours. Anexhibitor who abandons its space prior to the official show close (except withpermission of AAID Show Management) may forfeit their points earned in2013. AAID has sole authority on whether an excuse for delay orabandonment is acceptable.

EXHIBITOR BOOTH PERSONNEL REGISTRATIONThere will be a limit of three booth personnel per contracted booth space. Forexample, if you contract for three booth spaces, you may have nine boothpersonnel (3 booth spaces x 3 booth personnel = 9 booth personnel).If exhibitor personnel registration exceeds this limit, there will be a fee of$295 per additional person. To avoid on-site delays, exhibitors areencouraged to register their exhibit personnel in advance. A registration form

is available online at www.aaid.com. AAID will issue badges for boothpersonnel and have them ready at the registration desk upon check-in if theexhibiting company submits names by September 20, 2013.

ELIGIBILITY TO EXHIBITSubmission of an Application/Contract for Exhibit Space or othercommunication requesting exhibition space at the Annual Meetingconstitutes an offer on the Applicant’s part to lease exhibition space. Bysubmitting an application or making other communications regarding theleasing of exhibition space, the applicant acknowledges that AAID is underno obligation whatsoever to accept the application or other request forspace. Nothing contained in the Exhibitors’ Prospectus Application or otherinformation, or material prepared or provided by AAID, grants or confers anyright or rights on an applicant to obtain, lease or rent exhibition space.

This exhibition is designed for the display and demonstration of productsand services relating to the practice and advancement of the art andscience of oral implantology and the professional education of AAID members. AAID reserves the exclusive right to determine the eligibility of allexhibitor space applicants.

CONTRACT FOR SPACEThe information contained in this Exhibitor Prospectus, the Application forSpace, and the Formal Notice of Assignment constitute a contract for theright to use the space allocated.

BOOTH ASSIGNMENT PROCEDURESponsor booths are selected first. Because non-sponsor exhibitors will begiven the opportunity to select their booth space beginning May 1, 2013,sponsors who apply for sponsorship after May 1, 2013, may not necessarilybe guaranteed premium exhibit location. Non-sponsor exhibitors will begiven the opportunity to select their booth space after May 1, 2013 basedon a priority point system:• 1 point awarded for each year of Annual Meeting exhibit hall participation(1993-2013)

• 1 point for each 8' x 10' booth space occupied (1993-2013)• 1 point for every $2,500 donated to the AAID Foundation (May 1, 2012through April 30, 2013). In-kind and auction donations excluded.

• 1 point for every $5,000 advertising dollars spent in AAID publications(payments received between May 1, 2012 and April 30, 2013)

• Exhibit hall participation and booth space points are the only points thataccrue annually.

The company contact will receive an e-mail from the Academy with a floorplan from which booth selection can be made. Full payment is requiredprior to a booth assignment. AAID reserves the right to adjust the floor plan,relocating exhibit booths as necessary. AAID will periodically add boothnumbers to the list of exhibiting companies on the AAID Web site. In the event of a company merger/acquisition, only the higher priority pointtotal of the two merging companies will be recognized. Merging companiesmay not combine their priority points to achieve a higher ranking. The AAIDExhibition Manager must be notified in writing in order for the acquiringcompany to assume the higher point total.

SUBLETTING SPACEThe subletting, assignment or apportionment of the whole or any part of thisexhibit space by any exhibitor is prohibited. No exhibitor may permit anyother party to exhibit in its space any goods other than those manufacturedor handled by the contracting exhibitor in the regular course of its business,nor permit the solicitation of business by others within its space. Advertisingmaterial or signs of persons or firms other than those actually engaging thespace are prohibited.

EXHIBIT HALL INFORMATIONThe Exhibit Hall is carpeted. Booth cleaning is mandatory after move-in and is NOTincluded in the exhibit rental fee. The Academy has the right to order daily cleaningif booth appearance is unsightly. Cost will be charged to the exhibiting company. Theexhibition will be in the Grand Canyon Ballroom. Carpet is multi-color. Ceiling heightvaries between 17' and 21' inside the exhibit hall. See the floor plan on page 12.

FACILITY PROTECTIONNothing may be taped, posted, nailed, screwed or otherwise attached to columns,walls, drape, floor or any interior or exterior surface of the exhibit hall or furniturecontained in the facility. The use of stick-on decals is discouraged and thedistribution and display of helium balloons are prohibited in the exhibit hall. Signs,parts of exhibits, supplemental lighting or any other exhibit material suspended fromthe ceiling or attached to the ceiling of the exhibit hall must be approved by AAID.

GENERAL BOOTH CONSTRUCTION AND ARRANGEMENTS GUIDELINESBooth decorations are to be professional, standard booth design. When designingexhibits and planning for the display of products and literature, for both in-line andisland booths, good judgment and consideration for neighboring exhibitors andattendees should be the exhibitor’s primary objective. No signs, parts of exhibits,supplemental lighting or any other exhibit material may be suspended from orattached to the ceiling of the exhibit hall. Exposed or unfinished sides and/or backsof exhibits and displays must be draped or finished so as to present an attractiveappearance when viewed from aisles or adjoining exhibits. All exhibits will beinspected during setup and, at the direction of the AAID, the decorator will installdraping at the exhibitor’s expense to any part of the exhibit deemed objectionableby other exhibitors or AAID.

STANDARD BOOTHAll linear booths are 80 sq ft (8’ x 10’) unless otherwise noted. Booths have a backwall drape that is approximately 8’ high, with sidewall drapes that are 36” high. Ifbooth layout design exceeds 8’ in height, booth design must be approved by AAIDstaff. All display fixtures over 4’ in height and placed within 10 linear feet of anadjoining exhibit must be confined to that area of the exhibitor’s space that is atleast 5’ from the aisle line. The intent of the height and depth restrictions is thateach exhibitor is entitled to a reasonable sight line from the aisle regardless of thesize of exhibit. Exhibits are not permitted to span an aisle by ceiling or floor covering.AAID reserves the right to relocate any booth in the hall.

AMERICANS WITH DISABILITIES ACTEach exhibitor shall be responsible for compliance with the Americans withDisabilities Act (ADA) within its booth and assigned exhibit space.

REGULATIONSChildren under 12 years of age are not allowed into the Exhibit Hall under anycircumstances. This policy has been established for the safety of the children ofAAID members and other meeting attendees. Your support and compliance areappreciated.

LIABILITYIt is mutually agreed that the Academy and the facilities in which the AAID meetingsare being conducted shall not be held liable by an exhibiting company for any damageto or for the loss or injuries to the exhibitor resulting from any cause. All claims for anysuch loss, damage, or injury are expressly waived by the exhibiting company. Eachexhibitor by signing an application to exhibit, expressly understands that he/shethereby releases the American Academy of Implant Dentistry and The JW MarriottPhoenix Desert Ridge from and agrees to indemnify each against any and all claims for any such loss, injury or damage. Each party agrees to be responsible for itsown property through insurance or self-insurance and shall hold harmless theAcademy and the facilities in which the AAID meetings are being conducted for anydamage caused by theft and those perils normally covered by fire and extendedcoverage policy.

CERTIFICATE OF INSURANCEAll exhibitors must send an original certificate of insurance to the AmericanAcademy of Implant Dentistry by September 20, 2013. No exhibitor will be allowedto set up without the required certificate of insurance.Your certificate of insurance should include:1) Your company must be listed as the insured.

2) You must carry and maintain a minimum of $1 million insurance coverage thatprovides general liability coverage, coverage against damage to persons andproperty, and hazard insurance, insuring the exhibitor’s property and its exhibitspace, to fully protect it and the AAID and the additional insured (see below)against all risks in connection with its exhibit at the AAID 62nd Annual Meeting,or under the 2013 Rules and Regulations, or otherwise.

3) Required dates must cover the dates of the expo, including set-up andbreakdown: October 23 - 26, 2013.

4) All insurance shall name as additional insured Shepard Exposition Services andthe JW Marriott Phoenix Desert Ridge.

SECURITYAs a courtesy to exhibitors, the Academy will provide security service in the exhibitarea. The furnishing of such service is in no case to be understood or interpretedby exhibitors as guaranteeing them against loss or theft of any kind.

CANCELLATION BY AAIDIt is mutually agreed that in the event the AAID Annual Meeting is cancelled due todisasters, strikes, governmental regulations or causes that would prevent itsscheduled opening or continuance, then and thereupon this agreement will beterminated and the management of the American Academy of Implant Dentistry shalldetermine an equitable basis for the refund of such portion of the exhibit fees as ispossible, after due consideration of expenditures and commitments already made.

FIRE AND SAFETY REGULATIONSFederal, state and city fire laws must be strictly observed. Aisles must be kept clearof boxes and crates during installation and dismantling. Empty boxes and cratesmay not be stored in exhibit booth or behind drapery. Wiring must comply with firedepartment and underwriters’ rules. All decorations must be flame retardant to thesatisfaction of the Fire Department and the Fire Marshall’s requirements. All tablecoverings, walls, paper, or any decorative material whatsoever must have aflameproof certificate or tag. All certificates must be prominently attached to thematerial so they can be easily seen by the facility Fire Marshall. This does not applyto your product or any decorations provided by the official service contractor.

HAZARDOUS WASTE DISPOSALHazardous waste is any material being stored, recycled, or thrown away that couldcause injury or death, or pollute air, land, or water. Exhibitors who utilize materialfitting any of these criteria should inform the AAID and the JW Marriott PhoenixDesert Ridge staff of the presence and planned disposition of hazardous waste atthe time of space application to allow for thorough planning and preparation.

Any exhibit not set up by 8:00 am, Thursday or for which arrangements toset up have not been made, will automatically be set up at the exhibitor’sexpense and liability. Any booth not occupied by 9:30 am, Thursday may beassigned to another exhibitor unless the AAID staff has been made awareof extenuating circumstances. There will be no refund to the originalexhibitor. Exhibitors who need to work past 8:00 pm Wednesday mustobtain permission from AAID.

DISMANTLINGSaturday, October 26, 2013; 1:00 pm - 5:00 pmNo packing of equipment or literature, or dismantling of exhibits ispermitted until exhibit closing time, 1:00 pm Saturday, October 26. Any exhibit not dismantled by 5:00 pm - or for which arrangements fortear-down have not been made - will automatically be dismantled at theexhibitor’s expense and liability for proper dismantle. The floor must becleared by 5:00 pm. Exhibitors who dismantle early without expresspermission of show management will forfeit all points earned at the 2013 meeting.

LABORShepard will provide the labor for setup, dismantling, and material handling.Labor will be available based upon advance orders. To assure that the correctcraftsmen are available, exhibitors are urged to order labor in advance.

MATERIAL HANDLING/STORAGE/HAULINGThe official material handling contractor will move all freight from the docksto the exhibitor’s booth. This service includes:• Storage prior to the show• Transportation from warehouse to booth• Storage of empty crates• Reloading at show site• Clerical assistance and delivery to a common carrier at the loading dock at the close of the show.

SHIPPINGComplete shipping instructions will be included in the service kit e-mailedby Shepard in July and posted on www.aaid.com after July 31.

DISPLAY AND EXHIBIT WORK- Installation, Dismantling, and DecoratingExhibitors may install and dismantle their own booth as long as thefollowing conditions are met:

1. Set-up personnel are full-time employees of the company the exhibitbelongs to.

2. No power tools are allowed.

3. Exhibitors are allowed to unpack and repack their own products and areallowed to do technical work on their machines. Installation of graphicsand small signs are allowed if they are attached with pre-cut Velcrostrips.

Exhibitors may also “hand carry” display materials if they can be broughtinto the Exhibition Hall in one trip. No hand trucks or carts are permitted.This rule will be strictly enforced.

Exhibitors who wish to order labor for their booths must use union labor forinstallation and dismantle. Labor order forms will be included in theExhibitor Kit.

EXHIBITION/JOB SITE DRAYAGEAll drayage handling, forklift operations, and work requiring materialhandling equipment is to be performed by Shepard.

SAFETYStanding on chairs, tables or other rental furniture is prohibited. Thisfurniture is not engineered to support your standing weight. Shepardcannot be responsible for injuries or falls caused by the improper useof rental furniture. Please assist in our efforts to provide a safe workingenvironment for everyone.

GRATUITIESShepard specifically requests that exhibitors do not tip its employees.Requests for tips should be reported to the on-site Shepard Service Desk.

The AAID Rules and Regulations are based on thosedeveloped by the Healthcare Convention and ExhibitorsAssociation (HCEA) and are in place to serve the bestinterest of all parties and provide a fair and equitableenvironment for all. A mutual commitment of good faithand cooperation to all parties involved will ensure thequality of the AAID Exhibition.

The Rules and Regulations are subject to change at thediscretion of AAID. By applying for exhibit space, allapplicants and exhibitors understand and agree theywill be bound by the AAID Exhibition Rules, Regulationsand Guidelines that appear in the Exhibitor’sProspectus and all AAID materials.

IMPORTANT PLEASE READ CAREFULLYAll exhibitors must adhere to the Rules andRegulations and any others promulgated by AAID, all ofwhich are incorporated by reference as part of theApplication/Contract for Exhibit Space agreements.AAID reserves the right to interpret and make finaldecisions regarding all rules and regulations.

DISTRIBUTIONS OF PRINTED MATERIALSAND CANVASSING BY INDUSTRYCanvassing in any part of the facilities utilized by theAAID is strictly forbidden, and anyone doing so will berequested to leave the building.

Distribution of advertising or printed material by theexhibitor outside of the exhibitor’s allotted space willnot be permitted unless the distribution or advertisingis organized by the AAID.

Regulations also apply to all hotels used by the AAID tohouse meeting attendees. These rules begin at 8:00am Wednesday, October 23 and continue until 11:59pm, Saturday, October 26.

BOOTH ACTIVITY ENTERTAINMENT/PROMOTIONAL ACTIVITYDemonstrations or live interviews must be confined tothe limits of the space contracted. The use ofentertainment is prohibited unless exhibitor has writtenpermission from the Academy. The use of celebritieswho are not routinely employed as spokespersons forthe exhibiting company is discouraged. Specialpromotional activities must have AAID writtenpermission and must be contained within the spacecontracted.

The AAID may withhold or withdraw permission todistribute souvenirs, advertising, or other material itconsiders objectionable.

Signage for product or activity promotion is NOTpermitted outside the space contracted unlessspecifically authorized by AAID.

INDEPENDENT EDUCATION PROGRAMSCompanies are prohibited from sponsoringindependent programs in the headquarters hotelfrom 8:00 a.m., Wednesday, October 23 through11:59 pm Saturday, October 26.

OPPORTUNITY TO HOST PRIVATE FUNCTIONSExhibitor-sponsored private functions are separatelyorganized events during which exhibitors meet withAAID participants outside of the exposition for thepurpose of networking, entertaining and continuingbusiness begun on the exhibit floor. These privatefunctions may not be used for education programs.Exhibitors sponsoring any type of private function arerequired to adhere to the following guidelines:1. All costs related to private functions are theresponsibility of the host company.

2. AAID must be informed in writing of and giveapproval for all planned social and hospitalityfunctions.

3. Hospitality and social functions may only bescheduled during times that will not interfere withofficial AAID scheduled activities.

4. Social and hospitality functions should be handledon an invitation only basis. Host companies mustmake it clear to their guests that the event is not anofficial AAID function.

5. Host companies agree to assume all liability arisingout of or in conjunction with such functions andagree to indemnify AAID against any and all liabilityand claims and demands arising out of or inconnection with the foregoing undertakings.

6. Sponsoring companies requiring hotel servicesshould contact AAID at 312.335.1550.

7. Nonexhibiting firms are prohibited from hostingfunctions or distributing information during theAnnual Meeting.

SOUND AND AUDIOVISUALAudiovisual and other sound effects must beregulated so that they do not disturb neighboringexhibits. The AAID reserves the right to determine atwhich point sound interferes with others and must bediscontinued. Live performance of music is notpermitted. Video monitors or projection screens must beplaced inside the booth so viewers do not block aisles.

Noise from electrical or mechanical apparatus mustnot interfere with other exhibitors. No exhibitor shalloperate equipment or voice reproducing machines thatwould cause annoyance to other exhibitors. Earphonesmust be provided or such recording devices must be enclosed in a special booth. AAID reserves the rightto determine at which point sound constitutesinterference with others and if it does, it must bediscontinued.

NO SMOKINGThe Academy has a no-smoking policy for all AAIDmeetings that applies to the exhibit hall, registrationarea, and scientific sessions.

PERSONNELOnly those individuals directly responsible for the setupand dismantling of the booth will be permitted in theexhibit hall during move-in/move-out times. Childrenunder the age of 12 are not allowed in the exhibit hallduring the move-in/move-out times.

FOOD AND BEVERAGE PRODUCTSFood and beverage may only be distributed if they arethe products being promoted at the exhibit or theyare used to demonstrate the company’s product(s).Care should be taken with food so as not to cause litter in the exhibit area. Exhibitors are responsible forkeeping their booths litter free. Any exception to thispolicy must be approved by the Academy prior to theexhibition.

POTENTIALLY DANGEROUS ITEMS IN BOOTHCylinders containing oxygen, compressed air or othermedical gas must be secured by a strap, stand or cartin an upright position to prevent tip-over. Exhibitors notcomplying with this rule are not permitted to use suchcylinders in the exhibit area. X-ray equipment may bedisplayed but not operated. Combustible materialsused in the exhibit hall must be flame proof inaccordance with City of Phoenix Fire DepartmentCodes. Use or storage of flammable liquids, gasses or solids is strictly forbidden. Noxious fumes emitting froma booth should be contained in consideration toattendees and neighboring exhibitors.

CHILDRENChildren under 12 years of age are not allowed into theExhibit Hall under any circumstances. This policy hasbeen established for the safety of the children of AAIDmembers and other meeting attendees. Your supportand compliance are appreciated.

INTERPRETATION OF RULESAND REGULATIONSEach exhibitor understands and agrees that all ofthe restrictions and regulations are by referencea part of the executed contract for exhibit space.Should a disagreement arise relating to themeaning or interpretation of these rules andregulations the final decision shall be left to thejudgment and discretion of the Executive Directorof AAID, whose ruling shall be final and bindingon all parties. Each party to the controversyagrees to abide by that decision.

NON-EXHIBITING FIRMSNon-exhibiting firms are prohibited from hostingfunctions or distributing information during theAnnual Meeting.
