Potential population targets in preclinical stages of AD
« Amyloid first » biomarker profile
« Neurodegeneration first » biomarker profile
• Mayo Clinic Study of Aging • 11/26 (42%) incident amyloid-
positive subjects had evidence of neurodegeneration prior to incident amyloid positivity
• « Amyloid first » and « Neurodegeneration first » (SNAP group) biomarker profile pathways to preclinical AD exist
• What is the best population target in preclinical stages: stage 1, 2, 2 or SNAP?
Reisa A. Sperling et al. Toward defining the preclinical stages of Alzheimer’s disease: Recommendations from the National Institute on Aging-Alzheimer’s Association workgroups on diagnostic guidelines for Alzheimer’s disease. Alzheimer’s & Dementia 7 (2011) 280–292. Clifford R. Jack et al. Amyloid-first and neurodegeneration-first profiles characterize incident amyloid PET positivity. Neurology 2013.
Evolution of preclinical stages of AD
Progression to CDR at least 0.5, symptomatic AD by preclinical AD stage
• Strong association between preclinical AD and future cognitive decline makes preclinical AD an important target for therapeutic intervention
• Rate of cognitive decline was low compared with that in individuals with MCI or dementia
• Large sample sizes or a longer follow-up to identify effects on cognitive outcome measures
• Stage 3 and 2 +++ could represent a good target population

Specific Target, Strong and Specific Intervention, a limited period of time • Amyloid: identify with amyloid PET or CSF, treat with anti-
amyloid compound (A4)
Tau: identify with tau PET or CSF, treat with anti-tau agents
(tau PET added to A4, API, DIAN and ADNI)
• amyloid PET best for selecting trial participants, tau PET best for assessing disease-stage and maybe response to treatment.
• Expensive, cost effectiveness issues
• Ethics, safety, cultural issues, prognosis ?

• General population 50+ :
- General intervention: nutrition, physical, cognitive exercise, risk factor control
• Subjective Memory Complaints, family history:
- Deeper and more specific nutrition intervention, physical and cognitive exercise, risk factor control (MAPT)
Combination During Life Course of Non Specific and Specific Interventions
• Early MCI:
- Biomarkers:
- Negative: Muti domain Intervention, symptomatic treatments
- Positive: eg oral BACE inhibitors, plus multi domain intervention
• If Progression to Late MCI, Prodromal AD:
– monoclonal antibody plus multi-domain intervention
