Suicide Risk A ssessment Patients with mental health problems may attempt suicide, this can be in various ways although most commonly involves overdose of medication or cutting.  Attempted suicide is also known as self harm. Over 6 patients in the !" annually succeed in ending their own life #statistics from $anuary %&'(, however many more attempt to do so. )n this station you wish to assess the patient*s likelihood to attempt suicide, particularly if they have recently done so. )t is a psychiatric h istory station and in order for the patient to open up and be honest with you, you must gain their trust and establish a good rapport. Sub+ect steps &. )ntroduce yoursel f to the pati ent, cl ari fy t heir ident ity and epl ain that you wish to talk to them about their recent attempt to harm themselves. %. -he asses sment has 6 main components to it '. -he history of the cur rent epi sode of self harm. o /hat precipitated the attempt0 o /as it planned0 o /hat method did they use0 o /as a suicide note left0 o /as the patient intoicated #drugs1alcohol(0 o /as the patient alone0 o /ere there any precautions against discovery #e.g. waited until house empty(0 o 2id the patient seek help after the attempt or were they found and brought in by someone else0 o 3ow does the patient feel about the episode now0 #regret0 do they wish that they had succeeded0(

o Previous suicide attempts

•  Assess the patients mood, especially noting if they are

depressed or angry

• /ill the patient be returning to the same situation, e.g. problems

at home0

• /hat does the patient think about the future0

•  Ask about current suicide thoughts.

• -hank the patient for speaking to you.

9ou should summarise your findings to the eaminer stating the patients

suicide risk. 9ou should also suggest what to do net, e.g. hospitalisation,

outpatient follow:up, or ;P follow:up.
