There has never been a more exciting or inspiring time to be a part of Platte Valley Medical Center. Our staff, physicians, and volunteers make the difference as they seek to bring their best to every patient and guest through every interaction.

This year’s annual report is a reflection of our culture, innovation, insight, and evolution. It is organized by the six strategic pillars we identified and defined in 2012: People, Community, Growth, Service, Quality and Safety, and Finance. Through a strategic process, we align each department’s objectives by pillar to ensure everyone is working toward common goals.

This collaboration, together with the Planetree spirit of patient-centered care, is transforming our medical center. Through our focus on exceptional safety and delighting our patients with extraordi-nary care, we were recognized in 2012 as a Top 10 Hospital by Consumer Reports. We also positioned ourselves in the top 8% of U.S. hospitals through our Electronic Medical Records system’s meaningful use designation.

In addition, we introduced new technology and a higher level of treatment availability in the areas of Cardiac Electrophysiology, Neurosurgery and Spine Care, Critical Care, and Advanced Foot and Ankle Services.

While we are Brighton’s third largest employer with 605 staff, we recognize and value that one of our greatest strengths continues to derive from the friendships and alliances found throughout the Hospital.

With clinical staff comprising 60% of our employee base, we have adopted a nationally recognized teamwork system to improve patient safety, communication, and leader-ship skills. This evidence-based program, Team Strategies and Tools to Enhance Performance and Patient Safety (Team-STEPPS), is designed for health care profes-sionals and is scientifically rooted in more than 20 years of research and lessons from the application of teamwork principles.

Every other year, our employees participate in a staff satisfaction survey. In 2012, PVMC’s ratings improved in every area—work life, leadership, mission, quality and safety—with 40 out of 41 metrics scoring higher (17% higher, on average) than in our 2010 survey.

In 2012, the Hospital Employee Appreciation Team (HEAT) team hosted a number of celebrations not only to recognize the out-standing efforts of staff but to nurture and enhance closer relationships between col-leagues. We believe such efforts contribute to the lowest turnover rate of any Denver Metro hospital.

PATIENT-CENTERED CARE FUND

This fund helps personalize, humanize and demystify the patient and family experience. Your contributions have provided the gift of a journal kit for all patients in the Special Care Nursery and Oncology Clinic. Each new mother also leaves PVMC with a meal to enjoy with her new family.

TEChNOLOgY FUND

Surgical Services received a new neurosurgical microscope to support the needs of our Neurology patients.

wOMEN’S SERVICES CAMPAIgN

Without a doubt, the highlight of the year was launching a successful two-year campaign to raise $500,000 for Women’s Services. By December 31, 2012, the Foundation had reached 52% of this goal. Your support has already provided new state-of-the-art digital mammography equipment. Through continued giving, this campaign will make locally accessible, comprehensive patient-centered services a reality for the communities we serve.

Through innovation and strategic planning we continue to raise the bar to meet the growing needs of our community. In 2012, through partnerships and the integration of specialized services, we expanded our Cardiovascular, Neurosurgical, Critical Care, and Podiatry services.

cardiac electrOPhysiOlOgy sUrgeries

New equipment in the Cardiac Cath Lab enables one of our newest cardiologists to perform advanced procedures in Brighton. Dr. Christopher Stees of High Plains Heart and Vascular Center is a cardiologist with a special emphasis in electrophysiology. His expertise means that patients can now undergo cardiac electrophysiology proce-dures such as nodal ablations, including the placement of pacemakers, close to home. Electrophysiology services correct irregular-ities of the heartbeat, helping our patients enjoy a better quality of life.

neUrOsUrgery & sPine care

With experience in the latest techniques in spinal oncology, minimal access surgery, and complex spinal deformities, our newest Neurosurgical and Spine Specialist Sharad Rajpal, M.D. offers patients local access to progressive spine care.

advanced fOOt & ankle services

Ankle replacement surgery, a still relatively uncommon procedure, is now available in Brighton thanks to the surgical expertise of Podiatrist Kyle Haffner, D.P.M. Dr. Haffner joined the orthopedic team at Mountain View Orthopedics in 2012. He is also the first physician born at Platte Valley Medical Center to come back and join the Hospital’s medical staff.

critical care/intensivist PrOgram

Six physicians from Critical Care, Pulmonary & Sleep Associates began working with our hospitalized Intensive Care patients in 2012. A critical care physician, also known as an Intensivist, is specially trained to care for critically ill patients who may require organ support, life support or intensive monitoring. These physicians are board certified in internal medicine and have subspecialty board certifications in critical care. Our intensivists provide exceptional patient care through a multidisciplinary team approach that includes critical care nurses, respiratory therapists, clinical pharmacists, nutrition specialists and social workers. This coordinated team, led by the intensiv-ist, is designed to provide our patients with the highest level of care.

Darrell Messersmith and his Information Services team’s efforts to bring eCareNet online places us in the top 8% of hospitals nationally.

Electronic Medical Records (EMR) places secure patient information at the caregiver’s fingertips. When information about medica-tions, test results, and procedures is accessible in real time, caregivers are better equipped to help patients develop a plan for care during and after hospitalization.

electrOnic medical recOrds sUccess

Our decision to implement eCareNet with one “Big Bang” launch—meaning all features of the Hospital’s Electronic Medical Record (EMR) solution went live on the same day—paid big dividends in 2012. By October, Platte Valley Medical Center was recognized for achieving stage 6 status of the U.S. EMR Adoption Model. According to the Healthcare Information and Management Systems Society (HIMSS) Analytics, reaching Stage 6 is a result of focus, good direction, and a steadfast commitment from leadership. Stage 6 institutions have established clear goals for improving safety, minimizing errors, and prioritizing IT implementations.

Implementation of our EMR lays the foundation for the launch of a private Health Information Exchange (HIE) initiative in 2013. In partnership with MobileMD, PVMC is developing eCareNet Exchange, a private health information exchange service enabling our community of providers to securely access and share the records necessary for coordinating patient care. As health information technology evolves, these tools will help hospitals and clinics work together to assure our patients and community are receiving the best possible care.

Through eCareNet Exchange, patients will soon be able to access a patient portal from the PVMC website where they can access their personal health record.

Our team of stroke specialists, led by Neurologist Rai Kakkar, M.D., are ready with the skill and technology to provide fast, accurate, and high-quality emergency services. Our advanced Medical Imaging technologies rapidly identify strokes so that appropriate treatments can improve chances for recovery.

strOke center

In 2012, we treated 56 stroke patients and entered a lengthy period of measure-ment for The Joint Commission’s (TJC) Primary Stroke Center Certification. Certification is based on the recommenda-tions for primary stroke centers published by the Brain Attack Coalition and the American Stroke Association’s statements and guidelines for stroke care.

“Our commitment to excellence in the care and treatment of acute stroke is another step in our expansion of evidence-based medi-cine, supporting technologies, systems, and staff expertise,” says PVMC’s Vice President of Operations Kurt Gensert. “We know that access to this kind of local, community-based specialty care is vital to Brighton and the sur-rounding region’s health and wellbeing.”

strOke recOvery sUPPOrt grOUP

For patients and their families who have experienced a stroke, the emotional strain can often be overwhelming. A Stroke Recovery Group was initiated in 2012 to help patients and their families connect with others while learning about valuable com-munity resources. The group has had great success hosting stroke survivors and their family members. The group meets on the first Monday of every month and is facil-itated by one of PVMC’s licensed stroke rehabilitation therapists.

antimicrObial (ab) stewardshiP Plan

Our ongoing efforts to address issues of infection control and prevention, antibiotic use, and antibiotic resistance were advanced in 2012 by our AB stewardship plan. We identify which organisms respond to different antibiotics, ensure the right amount is given to patients, and stop antibiotics as soon appropriate to reduce overuse. Our partnership with Rocky Mountain Infectious Disease physicians, along with use of electronic medical records and clinical decision support surrounding antibiotic use, further emphasize our com-mitment to optimal antibiotic use and high quality patient outcomes.

new advancements in the fight against breast cancer

Breast cancer will affect an average of one in eight women at some point in their life-time. Mammograms are a key component to early detection. In our continuing quest to advance women’s services, we installed the latest digital mammography equipment in 2012. In addition, Computer-Aided Detection (CAD) equipment and a voice recognition system was added to not only assist radiologists in reading images, but help them turn around results quickly for diagnostic tests should abnormalities be discovered.

Patient satisfactiOn ranks high

PVMC is widely regarded as the gold standard for providing patient satisfaction and overall service. In 2012 we focused our service efforts on Courtesy and Respect, Listening, and Explanation. Every depart-ment participated, creating their own plans for reaching higher patient satisfaction scores. Patient exPerience

We receive feedback from three types of patients—inpatient, emergency depart-ment and outpatient—through a telephone satisfaction survey. Our patients rated us higher in 2012 than in 2011 for overall satisfaction with our care, and whether they would recommend us to others. Improve-ment targets were also met for questions relating to communication, responsiveness of hospital staff, pain management, and discharge information. Our satisfaction scores continue to surpass those of most hospitals across the Denver metro area, the state, and the nation.

The spirit of Planetree patient-centered care is dramatically impacting our patient satisfaction. In 2012, our patients’ rating of the Emergency Department (ED) rose 15% in one year due to the ED team’s focus on enhancing patients’ experiences, such as sitting at the patient’s eye level, dimming room lights to create a calm environment, and offering frequent updates on tests and results. This 75% percentile score placed our ED in the top quarter of the nation’s emergency departments.

As a Planetree affiliate, PVMC strives to reinforce and build on evidence-based, patient-centered care, throughout the organization. The Planetree philosophy enhances the best of PVMC’s traditions and supports the ongoing initiatives of evidenced-based medicine.

Planetree fosters the concept of personal-izing, humanizing and demystifying the health care experience for patients and their families. It empowers patients and families through information and education, and encourages “healing partnerships” with care givers.

In 2012, significant progress was made in enhancing the core Planetree components of patient-centered care.

• 113 employees and volunteers attended Planetree Retreats. To date, 94% of all full-time and part-time employees have attended a Planetree Retreat.

• Aromatherapy was initiated in patient care areas throughout the organization.

• Patient, staff, leadership, board members and physician focus groups were formed and results from these assessments are currently being utilized to develop specific strategies to achieve a truly patient-centered care environment throughout the organization.

We believe that “Every Interaction is an Opportunity” to demonstrate the care and innovation that makes PVMC unique. As a Planetree Affiliate, we are moving forward to a truly patient-centered environment that will continue to Foster Optimal Health for All.

The Spirit of Planetree Awards promote patient-centered care by publicly recogniz-ing individuals who personalize, humanize and demystify the health care experience and support extraordinary achievement in patient-centered care.

These awards positively impact our staff through a process that includes and reflects the appreciation of peers and administration by recognizing those who demonstrate true patient-centered care.

The character and quality of the caregivers and health care organizations serving the community are recognized.

Physician chamPiOn award

This award recognizes a physician who champions the Planetree model of patient-centered care and whose actions provide a role model for other medical staff members. The 2012 Physician Champion is Behzad Molavi, M.D., an Interventional Cardiologist with High Plains Heart and Vascular Center.

caregiver award This award recognizes a caregiver who has a direct role in positively influencing the way health care is delivered at PVMC. Emergency Nurse Brad Lehmann, R.N. received this award for 2012.

theraPy animal award This award acknowledges an animal and handler who substantially contribute to the patient-care experience through their time and service. The 2012 Pet Partner Team award went to Kathy Limm and Ne’ele.

Clinical Quality is measured internally and externally through many indicators identified as important to providing the highest in quality care. More than a hundred records are reviewed internally each month to confirm patients are receiving the best care. In 2012, PVMC had a very successful year in meeting targets related to Quality and Safety.

electrOnic alert system fOr infectiOn cOntrOl

Keeping our patients safe from hospital acquired infections and from other harm related to care continues to be our focus. As of December 31, 2012, it had been 1,714 days since there was a ventilator associated pneumonia case, 623 days since there was a patient who had a central-line associated infection, and there were no patients harmed from medication errors.

This success is due in part to using advanced medical record technology to support the best possible care, including alerts and reminders for providers. We recently developed electronic system alerts to identify patients who require special infection precautions when they come back to the hospital. This reduces the risk of spreading infections. The ability to use technology to provide decision support is another reason we continue to excel in our safety performance.

To be recognized as a National Top Performing Hospital by The Joint Commision, a score of 95% must be achieved.

In 2012, PVMC achieved a 98% composite score for Heart Failure, 98% for Pneumonia, 99% for

Surgical Care, and a 1oo% score for Acute Myocardial Infarction.

We again anticipate being recog-nized as a Joint Commission Top Performer for 2012 quality measure performance.
