Person Centred Support

Person centred support is care or support that respects the person as an individual. It promotes dignity, choice, independence and well-being.

Person centred support puts the persons individual needs at the heart of their care.

treating children in which the child's needs and wishes are the most important thing

the person at the centre of care. Before this it was service-led. A change was brought about because people felt strongly that care for the dying could be much better and they started the hospice movement in 1967 with St Christopher’s Hospice.St Christopher's Hospice

What is a care plan?A care plan is an agreement between

you and your health professional (or social services) to help you manage your health day to day. It can be a written document or something recorded in your patient notes. Everyone who has a long-term condition can take part in making their care plan.
