1. GENERAL

1.1 The responsibility of an effective safety and loss prevention

Programmed lies in the protection of human lives against injury and death as well as to

protect properties, equipments and materials against damage and destruction.

1.2 It is the intent of WFE, to provide a safe work environment and an effective safety

provide a safe work environment and an effective safety for employees.

2. SAFETY ORGANIZATION

2.1 The WFE Safety Structure was based primarily on the concept that safety is

everybody’s business. The orders will emanate from the top down to the bottom

whereas any accidents or incidents that have occurred will be reported in a vice versa

direction as outlined in the attached Safety Organization Flow Chart with resume. The

Safety officer who coordinates and monitors all Loss Prevention Activities in the

projects will report his findings, observation and recommendations to the Manager.

2.2 The over-all participation of the Contractor’s employees is essential and will reinforce

the program.

2.3 The WFE Safety Organization is a Committee type of organization. As deemed

appropriate top man from each department/sections will be selected to form a

committee which will meet once a month to discuss matters exclusively relevant to

safety. This group of people forms the nucleus of the WFE Organization.

3. BASIC SAFETY POLICY

3.1 When instructing crew members in the performance of their duties, make every

effort to show them safety way to do the job.

3.2 Trains and instruct employees to work safety in areas that may be classified as

hazardous.

3.3 Do not get impatient. This just adds to confusion and leads to accidents.

3.4 Know your fellow works. Some will require more instructions and repetition of

procedures than others.

3.5 Should an unsafe act of fellow worker be observed, call his attention immediately.

If an individual continues to disregard safety, it is your duty to advise your

supervisor in order that the situation can be remedied.

3.6 Never take chances. Work in a safe manner. This will prove the fastest and most

efficient way of carrying out your duty.

3.7 Use common sense and the benefit of experience. When in doubt, ask.

3.8 Have respect for others as human being regardless of color, race or creed and help

them protect themselves. In so doing, you are helping yourself.

WFE SAFETY AND LOSS PREVENTION PROGRAM

3.9 Keep all places of work and surrounding clean, with rubbish disposed of, idle of tools and

equipment to be out of the way and walkways clean at all times.

3.10 Horseplay, jokes or scuffling will not be tolerated.

3.11 All employees are expected to participate in the weekly safety meeting.

3.12 PLANT FIRE: Do not go to a plant fire, follow the fire drill procedure.

3.13 TRUCK & TRAILER RIDING: Employees are forbidden to ride on loads, tenders,

running boards & tail gates. Drivers must not drive the trucks or trailer without checking

the inside & outside immediate area around to avoid unforeseen incident.

3.14 MOBILE EQUIPMENT: Employees are forbidden tide on or clamp up on any mobile

equipment to talk to the operator.

3.15 HOISTING EQUIPMENT: Employees are forbidden to ride on any load that is being

hoisted or on any hook. Keep clear of suspended loads.

3.16 ELECTRICAL & MECHANICAL EQUIPMENT: All employees must keep clear of

mechanical & electrical equipment at all times. Do not operate or work w/out approval

from the supervisor.

3.17 COMPRESSED AIR: Employees must not use compressed air to blow dirt from hands, hair or

clothing. They must not play or waste with compressed air blow it at another person.

3.18 SMOKING: Smoking is prohibited on the plant except at designated smoking areas. Do

not bring matches or lighter in the plant.

3.19 SANITARY FACILITIES: Employees must use the sanitary facilities provided in the plant.

3.20 TOOLS: Use the right tool for the job. No make shifts.

3.21 WORKING ON PIPES OR OTHER EQUIPMENTS: For working on pipes or other

equipment in addition to the working permit the equipment shall be properly identified

and marked by the person signing the work permit.

3.22 SAFETY PROCESDURES: Follow all safety procedures applicable to the job being

performed.

3.23 All site staff will wear all required PPE’s, should be provided by the contractor for their

employees.

4. SAFETY ACTIVITIES

4.1 Safety Meeting:

4.1.1 The Contractor will hold ten minutes weekly talks for each department on a

day designated by the Safety Officer, who will preside. All employees involved

will attend. The team of the meeting shall be taken from:

a. The Clients Safety Manual

b. Actual Accidents/Injury Cases

c. Topics relevant to present work in progress

4.1.2 A five minutes safety question and suggestion period will be held during the

meeting.

4.1.3 When a jobsite situation requires it, five minutes safety meeting, presided by

the crew supervisor, will be utilized prior to start of work to point out safety

about the specific job.

4.2 Safety Orientation/Indoctrination of New Employees:

4.2.1 The safety officer will coordinate with the supervisor in charge of new

employees to conduct a safety orientation associated with their job prior to the

start or work.

4.2.2 The crew supervisor will advise new employees of the consequences and

penalties brought by failure of adhere to Loss Prevention Regulations.

4.3 Inspection

4.3.1 Daily inspection by Safety Officer of both the project site and the Camp Site to

ensure that correct safety practices are observed and to check any possible

hazardous conditions that may exist.

4.3.2 Back-up or follow-up inspections by superior in their respective working areas.

They will be responsible for the safety of each employee and for the protection

of equipment under their direction.

4.3.3 Weekly inspection throughout the entire jobsite will be conducted jointly by a

Safety Committee Member and a Loss Prevention Member. Any unsafe

condition discovered by this inspection will be promptly corrected.

4.4 Injury Treatment/Mode of Transport

4.4.1 WFE will adopt the provision for the client on first aid facilities and attendants.

4.4.2 The victim will be transported via the safety officer’s/supervisors vehicle if he

requires further treatment at the Contractor clinic.

4.4.3 Serious cases will be brought by the ambulance to the nearest hospital after the

injured person received medical treatment at any hospital/clinic.

5. WORKS PERMIT

5.1 Work permits involve a coordinated effort between the Safety officer and the various

supervisors who will inspect and investigate the work please before any job activities

start.

5.2 After identifying the type of work permit needed, the Contractor’s personnel with

valid work authorized will request such permits.

6. EQUIPMENT INSPECTION REQUIREMENT AND TRUCKING

6.1 Equipment/vehicle/truck is to be inspected on a daily basis, including observation

during operations, for any defects which might appear between regular inspections.

Any deficiencies will be examined carefully and determinations made as to whether

they constitute a safety hazard.

6.2 A master file of all equipment/vehicle/truck inspection reports will be kept for

evaluating each equipments physical conditions and performance.

6.3 Equipment/vehicle/truck will be conducted by the Equipment Coordinator with the

assistance of the Motor Pool Foreman and qualified mechanics.

6.4 All equipment, vehicles & trucks to be subjected for inspection.

WFE – QUALITY ASSURANCE AND QUALITY CONTROL MANUAL

PURPOSE

This procedure provides services and ensures the manpower supply shall meet or

exceed the Client/s requirements in particular. It likewise establishes high quality job

performance and assurance through inspection procedures, documentations and

corrective actions prior to the completion of the project/s.

SCOPE

Establishing inspection procedure is applicable for all the projects executed inside

the Kingdom of Saudi Arabia under Work Force Energy Contracting Est.

QUALITY PLANNING

The objective plan is to attain proper coordination within the organization, ensure

and exhibit commitment with the client as per agreement and completion of the

contract stipulated in the target dates.

CONTROL OF MONITORING & MEASURING DEVICES

Progress activities are monitored the duly approved work schedules, i.e. bar

graphs, S-curves which in the schedule percentage is incorporated thru weekly basis.

The weekly percentage is measured and compared with actual percentage against

planned percentage. After careful realization the resulting outputs of the process of

computation by s-curves can be compared against the planned schedules to

measure variance for evaluation the efficiency and effectiveness. When results do not

meet or deviate from the planned requirements, necessary correction or corrective

actions are then identified and implemented as required. The construction schedules,

S-curves, bar charts are measuring devices be used.

RESPONSIBILITY

1. Project Engineer (PE):

1. Prepares and submits documents related to i.e. fabrication, construction, execution, inspection procedures, related procedures, QA/QC plan & documents for review / approval prior to the start of Construction/ Fabrication activities.

2. Coordinate with QA/QC Engineers / Inspectors for inspections on regular basis.

3. Communicate for any major adverse inspection results notified by QA/QC Engineers / Inspectors until it is resolved.

4. Arranges all inspections for Site acceptance to QA/QC Engineer / Inspector.

5. Assure access to QA/QC Engineers / Inspectors to those parts or areas of site and where and when work, or testing / Inspection on any contracted portion of a project is being performed.

6. Ensure compliance observation report received from the QA/QC Engineer / Inspector is completed and signed off within 5 working days.

7. Retain equipment / package units Operation and maintenance manuals to be retained as permanent documents.

2. Site Engineer / Supervisor (SE/S):

1. Attends Inspections as per approved ITP & the Inspection Boundary Matrix.

2. Forwards the copy of completed RFI’s for inspections executed QA/QC for information.

3. Secures / ensures availability of approved drawings / material submittals / inspection procedures issued by the client.

4. Assures all Inspections are being done with safe work permit obtained from the client and ensures any necessary corrective action as required.

5. Reviews QC documents and ensures all inspections done by the site team as per approved ITP. Such documents are signed-off when required by the client.

3. QA/QC Engineer / Inspector :

1. Reviews/inspects site materials and insures approved specification before installations. Material substitute must be in accordance with the Clients approval.

2. Conducts routine inspection as per approved ITP and ensures corrective action when necessary and all Inspections reports to be signed-off within 3 days.

3. Reviews / approves all documents related with QA/QC such as FAT / SAT procedures, Project QC Plan, & ITP as required by the Client.

4. Recommends to Construction Supervisor (CS) / Project Engineer (PE) to stop the job provided that the concerned observation / Nonconformance Report will affect the safety & quality.

5. Expedites the resolution of actions raised in the Observation / reports.

PROCEDURE

1. Quality Assurance

1.1. Ensures the receipt of Request for Inspection RFI for notice of readiness of inspection at least 24 hours prior to the time of the Inspection.

1.2. In case of urgent /emergency and shutdown related work, QA/QC shall deputes inspector at short notice.

1.3. Checks/screens the RFI per approved ITP or the Inspection Boundary Matrix and forward related RFI’s to QA/QC for necessary action.

1.4. Conduct Inspections as per ITP, sign off RFI as Satisfactory or Unsatisfactory & Inspection forms.

1.5. No work shall be allowed to proceed beyond the “HOLD POINT” established by ITP without written authorization of the Client’s Project Incharge.

1.6. Ensures the Construction approved construction drawings & material submittals at the time of inspection.

2. Material control

2.1. All delivered materials and equipment supplies shall be examined and

necessary test of materials if required shall be conducted to ensure suitability acceptable to the Client.

2.2. Documentation and record storage – drawing details, test results certification, details of fabrication and other special requirements shall be placed to record for references.

2.3. All materials, equipment, tools and supplies shall be in proper places of storage for accountability purposes prior to the time of the Inspection.

3. NONCONFORMANCE REPORT (NCR) / OBSERVATION NOTE:

3.1. The Observation Report shall be used to record communication between QA/QC Engineer / Inspector, when any discrepancy or deficiency related to safety & quality matters is observed.

3.2. The Observation/Non-Conformance Report shall be sent to the concerned through the Client Engineer for feedback and corrective action.

3.3. Corrective action for the NCR shall be done immediately and provide feedback as early as possible, but not later than 5 working days of receiving the Observation/Non-Conformance Report.

3.4. Review the action taken and sign the action accepted or rejected. A site visit may be required to review and confirm the physical situation.

4. QUALITY CONTROL DOCUMENTATIONS

4.1. wFe shall compile all original documents related to material submittals & data sheets, inspection procedures, approved ITP, field test and inspection record, third party / Vendors test reports and certificates, Calibration reports, NCR/Observation reports, Operation & Maintenance manuals & As Built Drawings.

4.2. Review / comment / approve the QC documents. The comments if any shall be rectified.

4.3. After approval from QA/QC Engineer / Inspector, Q.C documents shall be retained for a minimum of One (1) year or until the end of Warranty period, whichever is later. (Note: Equipment / Package Units Operation and Maintenance manuals to be retained as permanent documents)

Codes and Standards

Codes and standards for the supply, installation, construction, fabrication,

inspection and/or testing of materials, services, or equipment for all SABIC Affiliates

construction projects under SPM, shall be in accordance with those specified in the

contractual documents, purchase order, work request, requisition, specifications, data
