What is an Accountable Care Organization?The purpose of Accountable Care is to improve the overall health of our patients. As an ACO, we take a pro-active approach that promotes healthy lifestyle choices, reduces preventable illnesses and better manages chronic or ongoing conditions. ACOs coordinate your healthcare among primary care doctors, medical specialists and multiple care settings through the use of shared information technology such as our Electronic Medical Record (EMR) system. Treatment plans are developed based on proven, best practices.

ACOs regularly report performance outcomes on quality measures such as prevention, disease identification, ongoing care for chronic illnesses and patient satisfaction to help ensure you’re receiving true quality healthcare.
