Studi farmacodinamici e farmacocinetici hanno mostrato

che la risposta anticoagulante e’ prevedibile in condizioni

cliniche “standard”.

Da ciò è derivato:

1) Somministrazione a dosaggio fisso giornaliero

2) La non indicazione al monitoraggio di laboratorio

routinario

3) Non “necessita’” di antidoti (breve emivita)

A Case Report

- An 84 year old woman (weight 40kg) was admitted to the emergency department for abdominal pain and rectal bleeding

- She had been treated for 4 months with dabigatran etexilate, 75mgx2/die, for AF, associated with amiodarone 200mg/die.

- At CT scan she was diagnosed as having faecaloma with no other abnormalities

- On admission: CrCL=32mL/min/1.73m2 Plts=381.000mmc, aPTT R=6.43 (NV<1.16), PTsec >60 sec (NV=6-13)

- She developed massive rectal bleeding a few hours after digital evacuation of the faecaloma. Soon after a cardiac arrest, she died because of haemorragic shock

- Plasma concentration of dabigatran was very high= 5.600ng/mL (expected range=31-225ng/mL)

INTERAZIONI FARMACOLOGICHE

Interactions should be properly evaluated. Whenever a concomitant therapy is ongoing with a drug likely to

interfere with NAO, a lab control should be performed (Pengo, 2011).

Many of these drugs interact with warfarin, but INR levels allows dose adjustment, which mitigates the risk of

concomitant treatment (Schulman S et al, 2012)

LA MISURA DELL’ATTIVITA’ ANTICOAGULANTE

• PT e aPTT : non sufficientemente sensibili alla misura delle concentrazioni farmacologiche; ampia variabilita’ reagente dipendente;possono essere modificati per altre cause (es. epatopatia, LAC)

• Tempo di trombina: troppo sensibile a dabigatran. Se nella norma esclude concentrazioni residue di dabigatran con quasi tutti i reagenti
