INTRODUCTION• Particularités du patient âgé & cancer VADS / sujet
jeune : Plus grande proportion de cavité orale Plus grande proportion de femmes Moins d’intoxication tabac/OH Moins d’infection HPV Plus de comorbidités Envahissement ganglionnaire moins important
• Etudes cliniques souvent < 70 ans
• Rapport bénéfices / risques le plus étroit possible…
INTRODUCTION
Décison RT-CT : n’a jamais débuté ttt
RT-CT, bonne compliance rémission
RT-CT, toxicités +++, interruptions +++ DCD 6 mois après
RT-CT, toxicités ++, interruptions + DCD 1 an après
Chimio induction
CHIMIOTHERAPIE D’INDUCTION
• « Indications » : – préservation laryngée +++,– risque méta (N2c N3), doute M+,– tumeur obstructive ou nécessité d’action rapide, – oropharynx HPV+ ?
Vermorken, Berlin 2013

TROUBLES DE DEGLUTITION
Conclusions: Dose–effect relationships exist for dysphagia and trismus. Therefore treatment plans should be optimized to avoid these side effects.

Bonner & al, Lancet Oncol, 2010
“In conclusion, there is accumulating evidence from both prospective and retrospective studies that suggests that it may be premature to substitute EGFR inhibitors for cisplatin outside of a clinical trial. Until there is level I evidence from a randomized phase III noninferiority trial that demonstrates equivalence in outcomes between these two agents, we believe CDDP remains the preferred concurrent treatment. Fortunately, RTOG 10-16 is addressing this question, although results will not be available for several years”
