Initial projects Though it has been in operation less than twoyears, SPARC has already undertaken more than20 projects. They include:
! Patient exam roomsThe team equipped exam rooms with movablefurniture and a computer monitor that can bepositioned so both physician and patient canview information. The edges of desks and tableswere rounded in an attempt to make them morewelcoming. Since family members or significantothers often accompany patients toappointments, the size and seating capacity ofthe rooms were increased. The team alsoincreased the amount of space designated as thepatient area, hypothesizing that the additionalspace would lead patients to feel more involvedin their care.
“The day after we set up the exam rooms, Iwas seeing patients in one of them and gettingimmediate feedback about what worked,”saysAlan Duncan, M.D., SPARC’s medical director.“One thing patients responded well to was theroom’s rounded table. Sitting next to me at atable, rather than across from me at a desk,made patients feel like a partner in their ownhealth care.”
! Decision aidsPhysicians often wrestle with the problem ofpatient compliance, wondering why patients failto adhere to the treatment plans they are given.
“We’ve found that being ‘given’a treatmentplan is a big part of the adherence problem,”says Victor Montori, M.D., SPARC’s director of research and education.“When patients aresimply handed a prescription or told to makelifestyle changes to improve a condition, they maynot fully comprehend the importance of therecommendation. Furthermore, they may not getto express concerns and challenges they may facein implementing the recommendation. Indeed,the recommendation, while optimal for most, maynot be the right one for this patient.”

Nicholas LaRusso, M.D., chair, Department of Medicine at Mayo Clinic in Rochester, believesthat SPARC has the potential to elevate both the quality of care and level of service offered at Mayo.
“Studying the patient care process is going togive us great insight into how efficiently andeffectively we are delivering our service tocustomers,”says Dr. LaRusso.“That informationwill enable us to better match the right patient tothe right physician at the right time, which willimprove patient satisfaction.”
Rapid and wide-ranging benefitsMedical research typically takes place over manyyears. But research into the delivery of patientcare has the potential for immediate results.
“That’s one of the things that’s most exciting about SPARC,”says Dr. Montori.“It’s highly
gratifying to be able to have an impact on patient care over the period of a few weeks or monthsinstead of a few decades. We get the satisfactionof seeing our research benefit today’s patients,rather than just hoping the research will benefitthe patients of tomorrow.”
And while medical research is generally targeted toward a specific disease or group ofpatients, discoveries made at SPARC have a much broader scope.
“Since we’re studying the patient care process,our work can be applied in virtually any patientcare setting,”says Dr. Duncan.
The SPARC team also plans to report theirfindings in peer-reviewed journals, making itpossible for other health care organizations toadopt its innovations.

The SPARC Innovation Program at Mayo Clinic
SPARCing advances in clinical care SPARC inspires novel thinking about the bestways to deliver health care. As part of the effort,industrial engineers observe physicians andpatients interacting, and then develop ways toimprove not just efficiency, but the overall patient experience.
Many SPARC activities take place in speciallydesigned and constructed practice spaces in theMayo Building. There consenting patients areobserved as they interact with physicians. Thefocus of the observation varies and has includedhow a redesigned exam room affects the patient-physician relationship; how a new educationaltool impacts a patient’s understanding of his orher condition; and how a self-service check-insystem influences patient satisfaction.
“By observing people, you uncover needs thatwould never come out if you asked directly,‘What could we be doing better,’”says RyanArmbruster, SPARC’s director of operations anddesign.“Observing situations allows us to get atneeds people are unable to articulate becausethey aren’t even aware they exist.”
“We’re looking to identify better ways ofdelivering care through hypothesis-drivenresearch, which is similar to the scientific methodused to unlock the mysteries of human diseasesand formulate effective treatments,”says Dr. Brennan.“ In the process, we’ve createdsomething novel: a clinical laboratory where we can study the how of health care delivery.”

Why now?In the health care industry, like so many others,customer satisfaction is becoming increasinglyimportant. As the quality of medical care availablebecomes more standardized, service is emergingas the differentiating feature among health careorganizations. And while evaluating the technicalskill of a surgeon or the diagnostic ability of aradiologist may be difficult for most people,nearly everyone recognizes quality service.
