All of us at LVI want your time here to be the best learning experience possible. It is most

important that your patient selection is to your advantage. In view of that, we must require that

your case selection comply with our guidelines. In our optimal scenario cases should exhibit the

following characteristics:

A medical history that is not compromised (cancer, diabetes, heavy smoker, etc.)

Patients must show good oral hygiene with recare within the past year.

These are general guidelines you are asked to follow. They are guidelines put in place to enable

us to follow all legal standards for this course. Any patient could have sub-clinical problems that

are not intended to be diagnosed at this time. As providers of dental care, we all must realize

that possible TMD problems are not always predictable, and changing the occlusion may cause a

hidden problem to arise. These possibilities should be addressed on the patient informed

consent form.

Please read this prior to reviewing the Risk Management Documents with your patient.

It is imperative that your patient is thoroughly informed of the procedures to be performed on

them here at LVI Global. It is also imperative that your patient realizes that although you are a

licensed dentist that you will be in a training situation and applying newly learned techniques. It

is also imperative that your patient understands that they have options to the proposed

treatment for this program including no treatment at all. It should be explained to your patient

that they have the right to change their mind and refuse treatment prior to the treatment plan

being started.

Do not leave any portion of the Risk Management forms blank. Please make certain the patient

consent form is completed and explained before being signed by your patient.

As a doctor it is important that you and your patient understand that there is always some

potential harm in having any procedure performed. The more forthright you are in relaying and

explaining the possibility of adverse effects to your patient the better protected you both will

be; no matter how obscure you may perceive these effects to be.

Please note that we do require that your information is current each time you participate in a

live patient program. As of January 2008, we are now required to obtain a certification of

licensure, sometimes referred to as verification of license. This must be requested from your

state board and mailed directly to LVI Global. This is not to be confused with your certified

license as those are only sent to you and should stay at your practice. We will keep your license

on file and update it online for you for future live patient courses you attend~provided your

state has this feature available.

I have been informed and I understand the purpose and the nature of the implant

surgery procedure. I understand what is necessary to accomplish the placement of

the implant under the gum or in the bone.

My doctor has carefully examined my mouth. Alternatives to this treatment have

been explained. I have tried or considered these methods, but I desire an implant to

help secure the replaced missing teeth.

I have further been informed of the possible risks and complications involved with

surgery, drugs, and anesthesia. Such complications include pain, swelling, infection

and discoloration. Numbness of the lip, tongue, chin, cheek or teeth may occur. The

exact duration may not be determinable and may be irreversible. Also possible are

Inflammation of a vein, injury to teeth present, bone fractures, sinus penetration,

delayed healing, allergic reactions to drugs or medications used, etc.

I understand that if nothing is done, any of the following could occur: bone disease,

loss of bone, gum tissue inflammation, infection, sensitivity, looseness of teeth,

followed by necessity of extraction. Also possible are temporomandibular joint (jaw)

problems, headaches, referred pains to the back of the neck and facial muscles and

tired muscles when chewing.

My doctor has explained that there is no method to accurately predict the gum and

the bone healing capacities in each patient following the placement of the implant.

It has been explained that in some instances implants fail and must be removed, I

have been informed and understand that the practice of dentistry is not an exact

science; no guarantees or assurance as to the outcome of results of treatment or

surgery can be made.

I understand that excessive smoking, alcohol, or sugar may effect gum healing and

may limit the success of the implant. I agree to follow my doctors home care

instructions. I agree to report to my doctor for regular examinations as instructed.

I agree to the type of anesthesia, depending on the choice of the doctor. I agree not

to operate a motor vehicle or hazardous device for at least 24 hours or more until

fully recovered from the effects of the anesthesia or drugs given for my care.

Diagnosis. After a careful oral examination and study of my dental condition, my dentist has advised me that I have periodontal disease. I understand that periodontal disease weakens support of my teeth by separating the gum from the teeth and possibly destroying some of the bone that supports the tooth roots. The pockets caused by this separation allow for greater accumulation of bacteria under the gum in hard to clean areas and can result in further erosion or loss of bone and gum supporting the roots of my teeth. If untreated, periodontal disease can cause me to lose my teeth and can have other adverse consequences to my health. Recommended Treatment. In order to treat this condition, my dentist has recommended that my treatment include bone regenerative/grafting surgery. I understand that a local anesthetic will be administered to me

as part of the treatment. I further understand that antibiotics and other disinfecting substances may be applied to the roots of my teeth. During this procedure, my gum will be opened to permit better access to the roots and to the eroded bone. Inflamed and infected gum tissue will be removed, and the root surfaces will be thoroughly cleaned. Bone irregularities may be reshaped. Graft material will be placed in the areas of bone loss around the teeth. Various types of graft materials may be used. These materials may include my own bone (autograft), synthetic bone substitutes (alloplast), bone obtained from animals, notably cattle and pigs (xenografts), bone obtained from tissue banks (allograft), or combinations of these product categories. Safety and sterility of all products used is on the utmost importance to your doctor and staff. Membranes or barriers may be used to cover the bone graft. My gum will be sutured back into position over the above materials, and a periodontal bandage or dressing may be placed. I further understand that unforeseen conditions may call for a modification or change from the anticipated surgical plan. These may include, but are not limited to, (1) extraction of hopeless teeth to enhance healing of adjacent teeth, (2) the removal of a hopeless root of a multi-rooted tooth so as to preserve the tooth, or (3) termination of the procedure prior to completion of all of the surgery originally outlined. Expected Benefits. The purpose of bone regenerative surgery is to reduce infection and inflammation and to restore my gum and bone to the extent possible. The surgery is intended to help me keep my teeth in the operated areas and to make my oral hygiene more effective. It should also enable professionals to better clean my teeth. The use of bone, bone graft material, or the placement of a membrane is intended to enhance bone and gum healing. Principal Risks And Complications. I understand that a small number of patients do not respond

successfully to bone regenerative procedures. This procedure may not be successful in preserving function or appearance. Because each patient’s condition is unique, long-term success may not occur. In rare cases the involved teeth may ultimately be lost. I understand that complications may result from the periodontal surgery involving bone regenerative materials, drugs, or anesthetics. These complications include, but are not limited to post-surgical infection, bleeding, swelling and pain, facial discoloration, transient but on occasion permanent numbness of the jaw, lip, tongue, teeth, chin or gum, jaw joint injuries or associated muscle spasm, transient but on occasion permanent increased tooth looseness, tooth sensitivity to hot, cold, sweet or acidic foods, shrinkage of the gum upon healing resulting in elongation of some teeth and greater spaces between some teeth, cracking or bruising of the corners of the mouth, restricted ability to open the mouth for several days or weeks, impact on speech, allergic reactions, and accidental swallowing of foreign matter. In the event that donated tissue is used for the graft, the tissue should have been tested for hepatitis, syphilis, and other infectious disease. Nevertheless, there is a remote possibility that tests will not determine the presence of diseases in a particular donor tissue. The exact duration of any complications cannot be determined, and they may be irreversible.

There is no method that will accurately predict or evaluate how my gum and bone will heal. I understand that there may be a need for a second procedure if the initial surgery does not meet pretreatment expectations. In addition, the success of bone regenerative procedures can be affected by medical conditions, dietary and nutritional problems, smoking, alcohol consumption, clenching and grinding of teeth,

inadequate oral hygiene, and medications that I may be taking. To my knowledge I have reported to my dentist any prior conditions that might in any way relate to this surgical procedure. I understand that my diligence in providing the personal daily care recommended by my dentist and taking all medications as prescribed are important to the ultimate success of the procedure. Alternatives To Suggested Treatment. Alternatives to periodontal surgery with bone regenerative/grafting

surgery include: (1) No treatment – with the expectation of possible advancement of my condition which may result in premature loss of teeth, (2) extraction of a tooth or teeth involved with periodontal disease, (3) non-surgical scraping of tooth roots and lining of the gum (scaling and root planing), with or without medication, in an attempt further to reduce bacteria and tartar under the gum line. The risk of no treatment is the worsening of my condition and the premature loss of teeth. Necessary Follow-up Care and Self-Care. Existing restorative dentistry can be an important factor in the success or failure of periodontal therapy. My dentist may make recommendations for the placement of restorations, the replacement of existing restorations or their modifications, the joining of two or more of my teeth, the extraction of one or more teeth, the performance of root canal therapy, or the movement of one, several, or all of my teeth. I understand that the failure to follow such recommendations could lead to ill effects, which would become my sole responsibility. I recognize that natural teeth and their artificial replacement should be maintained daily in a clean, hygienic manner. I will need to come for appointments following my surgery so that my healing may be monitored and so that the dentist can evaluate and report on the outcome of surgery upon completion of healing. Smoking or alcohol intake may adversely affect gum healing and may limit the successful outcome of my surgery. I know that it is important (1) to abide by the specific prescriptions and instructions given by my dentist and (2) to see my dentist and hygienist for periodic examination and preventative treatment. Maintenance also may include adjustment of prosthetic appliances. No Warranty Or Guarantee. I hereby acknowledge that no guarantee, warranty or assurance has been given to me that the proposed treatment will be successful. In most cases, the treatment should provide benefit in reducing the cause of my condition and should produce healing, which will help me keep my teeth. Due to individual patient differences, however, a dentist cannot predict certainty of success. There is a risk of failure, relapse, additional treatment, or even worsening of my present condition, including the possible loss of certain teeth, despite the best of care. Publication of Records. I authorize photos, slides, x-rays or any other viewings of my care and treatment

during or after its completion to be used for the advancement of dentistry and reimbursement purposes. My identity will not be revealed to the general public, however, without my permission.

complications. In Oral surgery the most common of these complications include leaving a small

piece of root in the jaw if removal of the root would require extensive surgery, post-operative

bleeding, swelling or bruising, discomfort, stiff jaws, loss or loosening of dental fillings. Less

common complications can include infection, loss of or injury to adjacent teeth and soft tissues,

nerve disturbances (i.e., numbness of lip, chin and tongue), broken jaw, sinus exposure and

swallowing or inhaling of instruments, fillings or teeth into lungs.

I further consent to the administration of local anesthesia, antibiotics, analgesics or any other

drugs that may be deemed necessary in my case, and understand that there is a slight element of

risk inherent in the administration of any drug or anesthesia. This risk includes adverse drug

response (e.g., allergic reactions), heart stoppage, and inhaling of stomach contents into lungs.

I realize that it is mandatory that I give as accurate and complete medical and personal history as

possible, follow any and all instructions as directed and permit prescribed diagnostic procedures.

I further realize that in spite of the possible complications, my contemplated surgery is necessary

and is desired by me. I am aware that the practice of dentistry and surgery is not an exact science

and I acknowledge that no guarantees have been made to me concerning the results of the

operation or procedure.

Registration fees are non-refundable and must be exercised within two years. LVI Global, LLC

(“LVI”) reserves the right to cancel courses 45 days prior to the scheduled date of a course or

activity. Should LVI cancel a course or activity, LVI will apply the full value of any deposits and

fees related to said course or activity to future LVI course or activities. Should LVI cancel a

course or activity, you may also have the option of having the deposits returned to you. Fees

What is the weather like in Las Vegas?

In the winter months temperatures range from 15-60. In spring the weather is nice with highs

between 70-80. Summer months are hot, highs up to 110, with nice warm summer nights. In

the fall it cools down with temperatures back around 70-80 degrees.

What should I wear when I come to LVI?

Business casual. We tend to keep the building cold so you might want to bring a light

sweater.

What should I wear if I am treating a patient in the clinic?

Just as you would in your office, appropriate Clinical Attire is expected at LVI. Attire should

conform to OSHA/CDC guidelines and regulations, and should include protection like closed toed

shoes for all of the team in the clinical setting.

Is food served at LVI?

A continental breakfast is served at 7:00 each morning and lunch is provided each afternoon.

Snacks are also available throughout the day.

How far is the Las Vegas Strip from LVI?

Approximately 12 miles. It could take up to 30 minutes with traffic.

Do you provide transportation to LVI?

LVI provides transportation only from the J.W. Marriott and The Red Rock Hotels. Check

with the Bell Stand for pick up times on course days.

Where do I check-in when I first arrive at LVI?

For every course you attend at LVI, you must check-in on the first day in the Hillwood Building

(the main building). However breakfast will be served in the Bistro located in the Hillshire

Building (the new building).

How many CE hours can I expect to receive from this course?

After completing this program, you will receive a CE form of the appropriate AGD approved

continuing education credit hours. These credits represent the lecture and participation portion

of the course.

When will I receive my CE credits?

Your CE form will be presented along with your attendance medallion and/or letter. Please keep

a copy of this form in your office records.

Does LVI submit my CE credits for me?

We will submit your CE credits to the AGD if you provide us with your AGD number. It is your

responsibility to keep the CE form indicating your credits on file in your office and, if necessary

submit your CE hours to the appropriate organization(s) (i.e.: your state/territory, etc.).

What happens if I lose my CE letter?

Once you receive your CE form, hold on to your originals and send copies when submitting your

organizations. If your original letters are misplaced, LVI must charge a $30.00, per course,

processing fee for necessary research. Replacement CE letters can take up to 3 weeks to

receive.

Discuss various flap designs and suturing techniques.

Utilize ridge Augmentation techniques.

Complete advanced extraction cases.

Recognize when to graft or do an immediate implant placement.

Discuss the benefits of tunnel grafting, ridge splitting with immediate implant placement, block

grafting, and bone harvesting.

Discuss the advantages of Pieazo Electric surgery.

Utilize the applications for tenting screws and titanium- reinforced membranes for hard and soft

tissue grafting.

Utilize the technique of Lateral Sinus Grafting.

Discuss potential surgical complications and management.
