Registration

Participants must register in person at St. Paul’s L’Amoreaux Centre at the Customer Service Desk. Registration is on a first come, first served basis. (Payment may be made by cash, cheque, debit or credit card). Office Hours: Mon - Fri, 8:30am-5:00pm. General inquiries 416-493-3333 (English) & 416-490-2970 (Chinese)

Registration Schedule (Monday to Friday, 9:00am – 3:45pm)

Spring Classes (Apr - Jun 2015) - Registration starts on March 5, 2015 (Thursday)

Summer Classes (Jul - Sept 2015) - Registration starts on May 28, 2015 (Thursday)

Cancellation Policy The Recreation Department reserves the right to cancel or reschedule programs, should the minimum class requirement during registration not be met, or due to reasons such as poor weather, or site unavailability. In the case of class cancellations, refunds will be given according to the number of classes remaining.

Class Withdrawal / Refund Policy (Please refer to the information on receipt) To ensure everyone has an equal opportunity to enrol in a class, a $10 administrative fee will be charged for each withdrawal. Notification of withdrawal is required at least 3 business days prior to the second class. Computer class participants must withdraw one week before the date of class. Notification has to be made in-person at the Customer Service Desk or by fax 416-493-3391. Refunds will be issued within three weeks.

Registration for special events is non-refundable.

Trip Withdrawal / Refund Policy

Withdrawals can be made 14 days prior to departure to receive a refund but a $10.00 charge will apply. The refund will be processed in 3 weeks. If less than 14 days notice is given prior to departure, but still before 48 hours of trip departure, a 50% refund will be issued. The refund will be processed in 3 weeks. Ticket transfers can be arranged by the participant at the Customer Service Desk before 48 hours of departure but a $10.00 charge will apply. No refunds will be issued with less than 48 hours notice of departure. (Please refer to the trip registration form for more information)

Membership Registration

Sign up for a Recreation Membership today for price discounts. Membership is available to adults who are 55 years and older. Membership fee is non-refundable and non-transferable.

One Year Membership: $20.00 from enrolment date

Membership registration is available at the Customer Service Desk of St. Paul’s L’Amoreaux Centre. Please bring valid identification.

Guidelines for Recreation Programs’ Participants

Appropriate Behaviour Expectations

Appropriate behaviour and conduct guidelines are created to ensure a safe and

enjoyable environment. All participants are expected: 1. To be respectful towards the staff, volunteers, program instructors, guests and other

program participants; 2. To follow the programs, instructor's / convener’s guidelines and requirements; 3. To inform staff/ instructor / convener immediately of any safety concerns. St. Paul's L'Amoreaux Centre will not tolerate any use of physical and / or verbal violence that threatens the safety and well-being of staff, volunteers, instructors, other participants and /or property. The Recreation Department reserves the right to suspend or terminate the participation of any person who has demonstrated inappropriate, rude, intimidating, aggressive or unsafe behaviour.

Client Safety

If you have any flu symptoms (fever, bad cough, sore muscles and /or sore throat) or feeling unwell, please stay at home.

Recreation Program Emergency Procedures

The health and safety of our clients is our top priority. St. Paul’s adheres to safety and risk prevention measures for all our programs and services.

It is important for staff, instructors, conveners, volunteers and participants to be well prepared for an emergency.

If an emergency occurs, stay calm. Call 911, if the situation is out of control. Pull the emergency cord at the nearest location. Stay there and wait for help.

When the emergency cord is pulled, the emergency response staff will arrive at the pulled cord location. Lead the emergency staff to the scene, and let the staff handle the situation. The emergency response staff will assess the situation. They will call an ambulance if necessary.

There are two emergency pull cords located in the recreation room. One is near the recreation office & the other one is located by the church doors.

Fire Alarm

When the fire alarm is activated, Recreation Staff / Instructor / Convener / Volunteer will stop the activity to pay attention to the announcement for instructions. Stay calm.

All Recreation program staff / Instructor / Convener / Volunteer must be familiar with location of the assembly point, and alternate emergency exits.

Dance Programs (There are 10 lessons for each class unless specified)

Remark: We recommend that you consult your doctor before registering for any fitness programs and have a medical check up before attending these programs. St. Paul's will not assume financial responsibility for any medical expenses or compensation for any injuries sustained either during, or resulting from participation in these programs. We strongly advise those participants enrolled in the fitness programs to observe the respective program requirements such as wearing suitable clothes and protective shoes accordingly.

Warm-up: We strongly advise program participants to do warm-ups before starting the program.

Level of Intensity: ٭ Mild: Requires little or no skills; low physical impact.

Trips / Outings April - June 2015

Important Guidelines for Participants of Trips

1. Participants must be at least 18 years of age, independent, and able to : • Understand and follow instructions • Load on and off buses

• Remain with the outing group • Walk at least 30 minutes

• Climb several stairs

2. Completion of the Trip Registration form, both Pre-Day Trip Questionnaire as well as the Personal information, is essential to ensuring the safety and well-being of all participants.

3. Participants must provide an emergency contact number upon registration/ to the Recreation Coordinator prior to the outing. Participants must also have their OHIP card with them during the outing.

4. Participants with health issues must consult their family doctor before registration, and stay home if not feeling well on the day of the outing.

5. All Trips participants wishing to park their car during the outing MUST park in the upper Southeast parking lot in the Bridlewood Mall at designated areas (see more details on last page)

Please contact St. Paul’s customer service desk for a visitor parking sign and display it on your dash-board.

6. Four seats are assigned for walker users for most events; they are allocated on a first comes first served basis. You must inform Customer Service Representative upon registration if you require stor-age for your walker.

7. All coaches and school buses are NOT wheelchair accessible.

8. If any participants require assistance with walking or have difficulty following or understanding instructions, they must have an escort on board. Escort /Caregiver must be 18 years of age or older.

9. Oxygen Tank / Cylinder users are responsible for carrying and maintaining their own equipment and ensuring they have sufficient oxygen for the duration of the trip. St. Paul's is NOT responsible for the maintenance of any equipment.

10. The Recreation Department reserves the right to suspend or terminate the participation of any person who has demonstrated inappropriate, rude, intimidating, aggressive or unsafe behaviour.

11. For refund policy please refer to the first page of the program guide for details.

12. Anyone with food allergies or special dietary needs must make arrangements for themselves.

Join us for lunch at local restaurants. Transportation is $4.00 per person for members & $6.00 for Non-members.

Meals will range in price from $10 to $30. Each person is responsible for the cost of their meal.

Please contact St.Paul's Customer Service Desk for a visitor parking sign and display it on your dashboard. Vehicles are subject to ticketing and/or towing if not parked in designated areas of Bridelwood Mall and if visitor parking signs are not displayed.
