WEST COTTONWOOD JR. HIGH 7TH & 8TH Grade Dance I hereby give my child _________________________ permission to attend the West Cottonwood Jr. High Graduation dance, Friday, June 2nd from

8:00pm to 11:00pm in the West Cottonwood cafeteria.

Dress Guidelines: As always, students must follow all other dress code guidelines for this dance.

Girls: Girls are to wear dresses. Sensible dresses that are not too extravagant or expensive are fine. Extreme styles should be avoided. Shoes with a medium or flat heel suited for a girl of junior high age are best.

Boys: Boys are to wear dress slacks, long or short sleeved dress shirts (colors optional) and a tie. No athletic shoes. Van’s style shoes will be allowed.

Cell phone use is not allowed during the hours of the dance. If there’s an emergency and your child needs to use the phone, one will be available.

Any 7th grade student that received an F on their report card or has an overdue library book may not attend this dance unless cleared through the office.

Bring this permission slip with you to the dance. NO 7th grade student will be allowed to attend the dance without this form.

____________________________________ is the adult who will be picking my student up from the dance. Students will be released to the parking lot area following the dance. We would greatly appreciate parents support by being on time at 11:00pm to pick up their student(s).

If you want to be able to access these photos online for FREE, your child must turn in this signed permission slip that demonstrates you have given permission to publish these images on the Instagram, Shutterfly, and Facebook accounts created EXCLUSIVELY for this dance as indicated by your signature below. All images will be free to download. Images uploaded on Shutterfly will have a file size large enough to print high quality photos.
