Back in 1977, Lorraine Berndt, who worked as a public health nurse, was very concerned about the home health needs of the elderly population in Cowlitz County. She borrowed $10,000 and started Community Home Health, which she incorporated as a nonprofit agency with community support.

Your gifts these past 35 years have ensured that patients are not turned away because of their financial situation. Today, we continue to care for people at home by providing home care private duty, home health, hospice and grief support services. Your gifts benefit families right here where you live.

FACING CHAllENGES TOGETHER

In 2012 we celebrate 35 years of caring for families in Southwest Washington. You have supported us all along as we delivered the highest quality, compassionate and dignified healthcare to individuals in their chosen place of residence. Thank you for entrusting us to care for you and your family. We are grateful, too, for your financial support and volunteer time given over the years, allowing us to provide excellent patient care and satisfaction.

We face the future with excitement, confident in our partnership to meet the challenges ahead. Our agency is prepared to manage the many changes taking place in the healthcare world. Costs are escalating, regulations are rising and government reimbursement is decreasing. In these difficult economic times, many are hard-pressed to afford healthcare, increasing the use of our charity care program.

We are making key investments to strengthen and grow our agency to ensure we are here for future generations. One investment includes a recently-installed state-of-the-art clinical information system so our staff can spend even more time focused on patients. We also implemented a new cost-saving telemonitoring program to help patients stay safe by staying in touch. We are expanding in Clark County to serve the growing senior population which is projected to double through 2030.

As a local, community-based nonprofit, our success depends on you—our patients, donors and volunteers. In this newsletter, you will see listed our many supporters who give their time, talent and treasure. We look forward to facing the challenges together.

MEETING INCREASED NEEDS

During fiscal year 2010-11, more patients and families required care than ever before. We served 9% more home health patients and 13% more home care clients than the previous year. Our hospice service increased the most. We served 1,146 hospice families, which is an increase of 22% over the prior year. See our Community Benefit Report on page 13 for more information.

FREE PET CARE FOR HOSPICE PATIENTSPet Peace of Mind

Imagine a hospice patient who is unable to care for their beloved pet and worried if it will have a home in the future. With more than 63% of U.S. households owning pets, this situation confronts hospice patients every day.

Our agency is the first in Washington State to offer Pet Peace of Mind in partnership with Banfield Charitable Trust. This program helps hospice patients keep their pets home with them through their end-of-life journey by providing food, exercise, litter and other pet care. It will also assist with new home placement if needed.

Many pet owners consider their pets as part of their family. Studies show that a pet’s unconditional love and acceptance may be a very powerful coping mechanism for an owner experiencing major life transitions. Yet it may be difficult to care for a pet while on hospice. Fearing they will be separated from their pet’s emotional and spiritual support, some patients choose between keeping their pet or getting the hospice care they need.

The Pet Peace of Mind program keeps pets and hospice patients together, providing current pet care and future pet placement. Knowing their pet will be cared for provides great peace of mind to hospice patients and families.

PET vOlUNTEERS NEEDED!

Share your love of pets and support local hospice patients. Volunteers are needed to help care for hospice patients’ pets to include walking, feeding, cleaning and other pet chores.

Attend the first volunteer training in Longview on Thursday, February 23, 2012.

SCHOlARSHIPS CHANGE lIvES

A passion for education inspired Rob Quoidbach to establish the Community Home Health & Hospice Continuing Education Endowment. He made his initial gift through a tax-free distribution from his IRA and a bequest in his estate plans. Rob personally knows the value of scholarship support. As a student at Stanford in Palo Alto, CA, one of Rob’s professors recommended him for a scholarship based on his academic performance. The scholarship helped Rob graduate with a civil engineering degree in 1948. Rob also met his future wife at Stanford and they returned to Longview and built a successful construction business.

Rob endowed the fund so our employees can improve their professional skills while continuing to work. The power of education makes for successful stories like Sandra Hoven, RN. Sandra joined our agency as a Hospice Care Center and home care aide, earning RN and BSN degrees through scholarship support. She appreciated the valuable experience she was gaining as well as the flexible scheduling while working to obtain her nursing degree. Today, Sandra is a registered nurse and case manager for our agency.

Rob’s passion for education is reflected in the professionals and students who benefit from his philanthropy. In recognition of Rob’s gift, we are pleased to include him among the founding members of the Community Legacy Society. The Community Legacy Society recognizes and celebrates generous people like Rob who include Community Home Health & Hospice in their estate plans.

vOlUNTEERS DElIvER CARE AND COMFORT

In fiscal year 2010-11, we welcomed 36 new volunteers. A total of 215 volunteers gave 19,466 hours of their valuable time. That’s the equivalent of 9.4 full-time employees!

Dedicated volunteers work with grieving adults and teens, participate in Camp Cedars (children’s grief camp), provide patient care in the Hospice Care Center and patients’ homes, perform office duties, give spiritual support to patients and families, work at the Hospice Thrift Shop, plan events, serve on our Board of Directors and committees and help with vital community relations.

EvERYONE SHOUlD HAvE ACCESS TO END-OF-lIFE CARE WHEN THEY NEED IT: ClARK COUNTY ExPANSION UPDATE

As a local, community-based nonprofit provider of hospice and bereavement services, our mission is to ensure that hospice services are available at the moment a patient or family needs it. This belief is the driving force behind our Clark County expansion in the Salmon Creek area.

In addition to our hospice program, we’ve provided home health and private duty home care in Clark County for nearly 25 years. We’ve seen our Clark County services grow by 500% over the past six years. With the population

of people 70 years and older projected to double over the next two decades, we’re building now to meet the current and future needs of Clark County families.

Our new 10-bed hospice care center will be the second hospice facility in the area, giving increased access to patients and families. Nestled among three acres of wetlands, the new facility will offer a homelike, serene setting with convenient interstate access. Also included will be space for visiting nurses, therapists, aides and other staff who travel throughout four counties to care for people wherever they call home. In addition, the grief support center, 150-seat conference center and an outdoor memorial garden will be added services for our community.

In Longview-KelsoDedicated volunteers have coordinated the Tree of Remembrance fundraiser in Cowlitz County for more than 20 years. They accepted donations for ornaments placed on the tree in memory of loved ones. Three Rivers Mall shoppers also purchased raffle tickets for items donated by local businesses.

In VancouverIn a four-year collaboration with Legacy Salmon Creek Medical Center, we invited hospital patients and families, visitors and staff to hang an ornament on the Tree of Remembrance & Healing. This year, 50% more ornaments were hung as this meaningful event grows in popularity.
