Preface The Registered Nursing Applicant Handbook is revised annually and is designed to provide information that will assist the applicant in becoming better acquainted with the policies and procedures governing entry into the nursing programs. As it is revised annually, the applicant is responsible for determining how the revisions, if any, would affect their application. The RN Admission Selection Criteria are approved by the College’s Curriculum Committee and Board of Trustees. These requirements are part of a statewide plan coordinated through the California Community Colleges Chancellor’s Office. The Merced College Catalog contains information about academic policies and course descriptions. It should be read in conjunction with this Handbook and is available at the Merced College Bookstore or online. Merced College reserves the right to revise A.S. Degree Requirements and the selection procedure at any point. It is the applicant’s responsibility to stay informed of the specific requirements and selection procedure for which they will be evaluated.

Program planning, advising, and career counseling are provided by the Guidance and Counseling Division. We urge the applicant to utilize their services early on in the career planning process. Appointments for counseling may be scheduled by calling (209) 381‐6478. Allied Health Counselors are located in the Counseling and Guidance Area (Lesher Student Services Building ‐ 2nd Floor). The application, selection, and admission processes for the nursing programs are coordinated by the Allied Health Division.



Introduction

Merced College Merced College, founded in 1962, is a two year community college located in the central San Joaquin Valley. The College’s mission, vision, core values and beliefs, and institutional student learning outcomes acknowledge the importance of meeting the needs of our students and contributing to an educated community.

The mission of the Merced College Registered Nursing Program is to prepare our students for careers as professional Registered Nurses that will best serve the community to a recognized standard of excellence. The Merced College Registered Nursing program focuses on 1)student success; 2)cultural diversity; 3)interrelationships of life experiences and knowledge; by providing a continually improving educational program which is accredited by the California Board of Registered Nursing. The Merced College Registered nurse graduate is educated for full participation in the life of the community, both professionally and as a citizen, and is instilled with a commitment to continued professional growth and lifelong learning.

Program Goals The Program Goals of the Merced Nursing Program reflect the mission of the program and align with those of Merced College and include:

1. Obtain licensure to practice as a Registered Nurse through successful completion of the

NCLEX‐RN® Examination. 2. Function as a beginning nurse in acute, intermediate, long‐term, and ambulatory

healthcare settings. 3. Utilize best practices as the foundation for the delivery of health care to the patient and

family. 4. Contribute to quality improvement of health care to support quality, safe, patient‐

centered nursing care. 5. Incorporate lifelong professional and personal development as part of one’s identity as a

nurse.



Aggregate Program Outcomes

To measure success, the Merced ADN Program expects to attain the following aggregate program outcomes:

1. Eighty percent of students will complete the program within 150% of the length of the program.

2. The pass rate of first‐time NCLEX‐RN® test takers will be at or above the national mean. Eighty percent of graduates will be working in a Registered Nurse position or enrolled in a higher degree nursing program within one year after graduation. Eighty percent of graduates surveyed will report satisfied or very satisfied with the education they received in Merced’s Nursing Program.

3. Eighty percent of employers surveyed will report agree or strongly agree the education

the new graduates received at Merced’s Nursing Program provided them with the knowledge, skills, and attitudes to perform well on initial employment.



Program Student Learning Outcomes

Faculty believe in the importance of incorporating professional standards, guidelines, and competencies as the basis for the nursing curriculum. The program must reflect current nursing practice. To this end, a rigorous and thorough examination of the current literature on nursing practice and nursing education was conducted. These findings guided the development of the program student learning outcomes. Additionally, faculty examined the current healthcare environment in the Merced area. The program student learning outcomes, along with their related competencies, reflect the expected behaviors of the graduates of Merced’s Nursing Program. Therefore, these program student learning outcomes are used to organize the curriculum, and guide all activities related to the teaching/learning process, including delivery of instruction and evaluation of student progress. The six student program learning outcomes are: 1. Provide quality, safe, patient‐centered nursing care through evidence‐based practice. 2. Participate in collaborative relationships with members of the interdisciplinary team to

provide and improve patient care. 3. Engage in critical thinking skills and strategies and clinical reasoning necessary to provide

quality patient care. 4. Provide leadership in a variety of healthcare settings for diverse patient populations. 5. Use information technology to communicate, manage knowledge, mitigate error, and

support decision‐making. 6. Function as a competent nurse assimilating all professional, ethical, and legal principles.

The documentation from which these program student learning outcomes flow is presented in the next section. The Program Student Learning Outcomes align with Institutional Student Learning Outcomes as follows: 1. Provide quality, safe, patient‐centered nursing care through evidence‐based practice.

Sequencing of Courses

The program is designed to culminate in these student learning outcomes. Therefore, each course is leveled based on these outcomes as well as populations of patients. The sequencing of courses reflects higher cognitive levels, a different patient population, and/or more complexity in patient care provided in a variety of healthcare settings. Patricia Benner’s Model of Novice to Expert is used in the development of the professional nurse. Courses build to assist students with their journey to advanced beginner by graduation, prepared to become competent nurses as they experience their first two years of practice (Benner, Tanner, & Chesla, 2009).

Documentation for Development of Program Student Learning Outcomes Current trends in nursing and health care as well as traditional standards and values of nursing practice influenced the choice of these six program student learning outcomes. Following is an explanation of the specific current trends and traditional standards and values used to develop the program student learning outcomes. 1. Provide quality, safe, patient‐centered nursing care through evidence‐based practice. This outcome focuses on the many aspects of the nurse/patient relationship and integrates the work of the QSEN group derived from the IOM studies (Finkelman & Kenner, 2009). Safety is a major concept and is based on QSEN.org documentation as well as the NCSBN’s Nursing Pathways for Patient Safety (2010). The importance of patient‐centered care is addressed by the Institute of Medicine (2010) stating that many systems are designed to meet the needs of providers. “True patient‐ and family‐centered care will focus on the whole patient, putting the patient, family, and care team together as a system” (p. 16). This outcome also relates to two of NLN’s Competencies for ADN graduates (2010): “Advocate for patients and families in ways that promote their self‐determination, integrity, and ongoing growth as human beings” (p. 33), and “Implement one’s role as a nurse in ways that reflect…..a nurse committed to evidence‐based practice, caring, and advocacy, and safe, quality care for diverse patients within a family and community context (p. 35). This outcome also embraces the major categories of content presented in the NCLEX‐RN® test plan. The ANA’s (2010 Nursing: Scope and Standards of Practice are applied when planning quality, patient‐centered nursing care. Major concepts for this learning outcome include:

2. Participate in collaborative relationships with members of the interdisciplinary team to provide and improve patient care. The importance of collaboration is emphasized in the work of the QSEN group derived from the IOM studies (Finkelman & Kenner, 2009) with the competencies of Teamwork and Collaboration. The QSEN and IOM competencies of Quality Improvement and Patient‐Centered Care also relate to this outcome. This outcome incorporates NLN’s 2010 definition of teamwork: “to function effectively within nursing and interprofessional teams, fostering open communication, mutual respect, and shared decision making to achieve quality care” (p. 69). The silo approach to care in which each professional works in parallel is no longer acceptable in the current healthcare environment. Health professionals must “cooperate, collaborate, communicate, and integrate care in teams to ensure that care is continuous and reliable” (IOM, 2003, p. 4). Additionally, the Institute of Medicine (2010) notes that 60 to 70% of adverse events happening to patients in the acute care setting can be traced to problems with communication. Some of those problems stem from disruptive behavior by both nurses and physicians.

This program student learning outcome addresses the nurse’s role in working with other healthcare professionals to plan and implement care and quality improvement measures. The importance of the broader context of a systems approach to care rather than the narrower nurse/patient relationship as the primary focus of the work environment is imperative for meeting the quality improvement competency for this outcome (Day & Smith, 2007). Major concepts for this learning outcome include:

3. Engage in critical thinking skills and strategies and clinical reasoning necessary to provide quality patient care.

Critical thinking is a broad term that encompasses all the thought processes that relate to clinical decision making and clinical reasoning. It is part of the term NLN (2010) defines as



nursing judgment which encompasses three processes: critical thinking, clinical judgment, and integration of best evidence into practice (p. 67). The National Council of State Boards of Nursing (NCSBN) identified critical thinking as the number two attribute for entry level RNs (NCSBN, 2006). Critical thinking is evidenced not only by the student’s use of the nursing process, but also when interfacing with the clinical microsystem and the larger healthcare system to deliver quality, safe, patient‐centered care. This outcome also relates to two of NLN’s Competencies for ADN graduates (2010): “Make judgments in practice, substantiated with evidence, that integrate nursing science in the provision of safe, quality care and promote the health of patients within a family and community context” (p. 34) and “Examine the evidence that underlies clinical nursing practice to challenge the status quo, question underlying assumptions, and offer new insights to improve the quality of care for patients, families, and communities” (p. 36). Major concepts for this learning outcome include:

4. Provide leadership in a variety of healthcare settings for diverse patient populations. This outcome focuses on the core component of Leadership. Leadership is comprehensive and includes managing care, delegating to others, integrating and coordinating care, investigating and sharing best practice guidelines, and serving as a leader in many capacities within the healthcare environment. Another major component under leadership derives from the Quality and Safety Education in Nursing (QSEN) (2007) competency, Quality Improvement. Quality improvement refers to the use of data to monitor the outcomes of care processes and the use of improvement methods to design and test changes to continuously improve the quality and safety of healthcare systems (Smith, Cronenwett, & Sherwood, 2007). This outcome also relates to two of NLN’s Competencies for ADN graduates (2010): “Advocate for patients and families in ways that promote their self‐determination, integrity, and ongoing growth as human beings” (p. 33), and “Implement one’s role as a nurse in ways that reflect integrity, responsibility, ethical practices and evolving identity…..for diverse patients within a family and community context” (p. 35). Major concepts for this learning outcome include:

Human flourishing 5. Use information technology to communicate, manage knowledge, mitigate error, and support decision‐making. Traditionally, communication referred to engaging in verbal and written exchange of information. More recently it also includes using information and communication technologies. Knowledge and use of information systems and nursing informatics in health care mandates that students learn about new technologies. This program student learning outcome is a specific competency recommended by QSEN. Knowledge of informatics is also recommended by the NLN in their 2008 position statement Preparing the Next Generation of Nurses to



Practice in a Technology‐Rich Environment: An Informatics Agenda. In this position paper the NLN called for nursing schools to incorporate informatics into the curriculum. Major concepts for this learning outcome include:

6. Function as a competent nurse assimilating all professional, ethical, and legal principles.

The general term professionalism is used to include all professional, ethical, and legal principles to guide the practice of the registered nurse. The foundation for this program student learning outcome flows from two American Nurses Association documents, Nursing: Scope and Standards of Practice and The ANA Code of Ethics as well as the California Board of Registered Nursing’s Nurse Practice Act. This outcome also relates to one of NLN’s Competencies for ADN graduates (2010): “Implement one’s role as a nurse in ways that reflect integrity, responsibility, ethical practices, and an evolving identity as a nurse committed to evidence‐based practice, caring, advocacy, and safe, quality care for diverse patients within a family and community context” (p. 35). Personal and professional development is part of this competency which includes lifelong learning. Major concepts for this learning outcome include:

Prerequisites Comparison Grid

This grid (refer to page 28) represents catalog lower division pre‐co requisite coursework

preparation for admission to the Registered Nursing Program at Merced College. It is

published for informational purposes only. Although every effort has been made to ensure

its accuracy, errors do occur. This is not considered an irrevocable contract between

Merced College and the student. Final acceptance of lower division pre‐co requisite courses

will be determined by the Nursing Department Admissions Committee. Students are

advised to consult an Allied Health college counselor for supplementary information. If

your college and/or course is/are not listed, students must submit official transcripts

to the Evaluation office one to two months prior to the application deadline (the

official transcripts must be from other schools attended, NOT Merced College). After

your transcripts have been evaluated, students are advised to schedule an appointment

with an Allied Health Counselor 209‐381‐6478 prior to the application deadline. It is

advisable to include a course outline and a syllabus along with official transcripts.

Courses not listed on grid (page 28) could take up to two (2) months or more to be

evaluated. Your application may be returned if evaluation is not completed at time of

application. Students are responsible for ensuring their course work is equivalent to

college requirements.



Program Application Process Applications are available from the Allied Health Division’s website ONLY. It is the applicant’s responsibility to submit all required paperwork within the specified day and times. Applications will be accepted during February. Specific day & times will be noted on the website. Up to 60 students will be accepted for the next academic year (1/2 for Fall and 1/2 for Spring – based on grant funding).

Selection Process The Merced College RN Program class selection process is determined using the multi‐criteria screening process established by the California Community College Chancellor’s office in accordance with AB 1559. A multi‐criteria score is determined for each candidate based on previous degrees/certificates, work/volunteer experience in a healthcare setting, identified coursework GPA, life experience/special circumstances, foreign language proficiency and TEAS V/ATI TEAS assessment score. Ten percent of the available seats in the program will be offered to students with the highest points. The remaining seats will be chosen by random selection lottery from the pool of applicants meeting the minimum qualification. These students will be sequentially numbered from one to the maximum allowed for the in‐coming class, including a predetermined number of alternate students. Depending on the number of qualified applicants and constraints of the Chancellor’s Office multi‐criteria screening model, the selection pool will vary in size from semester to semester.

Minimum Qualifications for application: ‐ All program prerequisites must be passed with a grade of “C” or better and combined GPA of 2.5

or higher

‐ Students must receive 62% or higher on their FIRST ATTEMPT of the ATI TEAS exam. For those students who were unsuccessful passing TEAS V, OR would like to submit a higher “ATI TEAS” score than their score on TEAS V, multiple attempts on the ATI score report will be accepted (with only one failed attempt on TEAS V). You must submit test results for other attempts (possibly for earlier versions). Applications will not be accepted for ATI test results that show the “first attempt” was an “ATI TEAS” score that was not passing. Students not meeting these minimum qualifications are ineligible to apply.

Multiple Applications There is no waiting list maintained for the R.N. Program. Applicants applying to the program a second, third or fourth time must submit pages 29, 30 & 31. The Allied Health Office has your file from the previous years’ selection. However, the applicant will be responsible for updating any other information that may change their point total and must comply with local and/or state requirements.

If an applicant has applied to the program more than once consecutively, the applicant’s name will be added to the application pool an additional time for each such application. An applicant is considered qualified for this purpose if all admission requirements have been met, the applicant meets minimum qualifications on the multi‐criteria selection process, and a complete application packet has been submitted by the application deadline. Upon the applicants’ fourth consecutive application, their name will be assigned to a FALL seat in the next school year. If the applicant declines their seat in the program and decides to apply in any subsequent application period, they will be considered a new applicant. All “consecutive applications” will become null and void.

Requirements for Accepted Applicants Only The following must be completed prior to starting the first course in the RN program and is addressed at the orientation session:

1) Health Clearance (including necessary immunizations or proof of immunity by titer) 2) Criminal Background Check (requires proof of valid social security number) 3) Drug Screening 4) Current CPR Certification, Healthcare Provider (American Heart Association only)

Note: Criminal background check and drug screen is repeated between the third and fourth semesters as a requirement prior to the mental health rotation.

Pre-assessment Testing Requirement Prospective RN program students must pass an assessment test, Test of Essential Academic Skills (TEAS), Version V or ATI TEAS. The “first test” result of the TEAS testing by ATI will need a minimum score of 62, otherwise the student will be ineligible to apply to the program. If a student has already taken TEAS V or ATI TEAS from another college, they need to include verification of this score with the application. As part of the application paperwork, we will be asking for students to sign and confirm that their TEAS result is from a “first test”. Any false information provided on the application will be grounds for immediate disqualification. Limited supplies of review books may be checked out from the Allied Health office and other practice links may be accessed on the Registering Nursing web page. ATI does provide (for a fee) remediation and study guides on their website. We highly encourage students to prepare for the TEAS test due to the need to achieve a 62 score.

LVN to RN Pathway The pathway for California Licensed Vocational Nurses requires one academic year beyond completion of the prerequisite courses, non‐Nursing breadth requirements, and Competencies. LVN to RN students enter into the third semester of the RN curriculum identified above after completion of REGN‐01 and all other prerequisites. Applicants submit an application found on the Allied Health website. Graduates will earn the Associate in Science Degree upon completion of designated courses and competency requirements. Communication, natural and social science, and nursing courses must be completed with a grade of "C" or better to be eligible for licensure requirements of the State Board of Nursing but must meet the admission requirements for the RN Program. Applications for the LVN to RN pathway are accepted continuously and held on a wait list until enough students are seated to begin a class. Enrollment eligibility to the LVN to RN pathway requires the completion of previously identified prerequisite courses. Applicants must meet the same selection requirements as generic RN students entering the program in the first semester. In addition, REGN‐01 is a prerequisite for the LVN to RN applicant who has already met the selection criteria. Randomized selection is used to select a cohort from the pool of qualified applicants according to available seats. Only those students formally accepted into the program may register for courses identified as Registered Nursing Curriculum. Students may, however, enroll in other courses designated as non‐Nursing program requirements while awaiting selection into the program.



LVN to RN “30 Unit Option” In compliance with regulations by the California Board of Registered Nursing, Licensed Vocational Nurses (LVNs) may seek eligibility for the RN licensure exam (NCLEX‐RN) without meeting the requirements for an Associate of Science degree by successfully completing the “30‐unit option”. Students who select this option will be required to sign a statement of understanding regarding the limitations of this option. Students who select this option are eligible for licensure in California, but may not be able to attain endorsement for licensure in other states without the AS degree in registered nursing. Students interested in the “30‐unit option” are encouraged to make an appointment with the RN Director to discuss further. Transfer Credits earned in the Merced College Registered Nursing Program may be transferable to California State Universities. Since prerequisite science and social science courses vary at each institution, students are advised to consult the catalog of their intended transfer school and establish a transfer plan with the Allied Health Counselor. Minimum Requirements for Selection 1. High School Education or the Equivalent

An applicant must meet the general preliminary education requirement of a high school education in the United States or the equivalent. Upon request of the Board, an applicant shall provide evidence of education equivalent to completion of a high school course of study in the United States by submitting one of the following:

A high school diploma received from a U.S. high school.

A General Education Development Certificate.

A degree from any junior college, college or university accredited by a state agency authorized to accredit such institutions

An evaluation by a high school, unified school district, junior college, college, university or board of education which the board determines, after review, establishes that an education equivalent to a high school education in the United States was obtained in this or another state or in any foreign country.

2. Successful completion of all program prerequisites with a grade of “C” or better and combined GPA of 2.5 or higher

3. Acceptance Process

Upon notification of qualification for an RN class, the applicant must reply regarding acceptance of admission. The acceptance letter will outline all details. Failure to notify the Allied Health Office with a “Intention to Enroll” in the prescribed time constitutes grounds to assign the applicant’s position to an alternate and the previous applicant’s name will be removed from the incoming class roster. Accepted applicant’s declining admission must submit the “Program Application” during the next application period if interested in being considered for subsequent classes. Refer to page 12 “Multiple Applications”.

4. Evidence of good physical and emotional health is important and a Health Screening Survey

is required of each Selectee. See “Core Performance Standards” in Appendix which identifies the cognitive, sensory, affective, and psychomotor performance requirements that students are required to perform to successfully complete the program.

5. Students must receive 62% or higher on their FIRST ATTEMPT of the ATI TEAS exam. For those students who were unsuccessful passing TEAS V, OR would like to submit a higher “ATI TEAS” score than their score on TEAS V, multiple attempts on the ATI score report will be accepted (with only one failed attempt on TEAS V). You must submit test results for other attempts (possibly for earlier versions). Applications will not be accepted for ATI test results that show the “first attempt” was an “ATI TEAS” score that was not passing.

Request two official sealed transcripts to be sent to you directly (the official transcripts must be from other schools attended, NOT Merced College). *To be official, transcripts must remain sealed in the original envelope. It is the applicants responsibility to obtain a copy of past college catalogs or course descriptions, to assist in the establishment of equivalencies.

Step 3: When Minimum Requirements have been met, prepare a COMPLETED application packet. The entire application, pages 29‐42 (except Criteria IV‐Foreign Language, pg.41) in this handbook are “fillable” forms. You must complete the forms on a computer by typing information into the form’s displayed fields (page 41 is the only exception, this page will need to be filled out by hand). Once all of the necessary fields have been filled out, you will then print your completed forms, sign and date where required, and submit them to the Allied Health office during the day and times they will be accepted (listed on the front page of this handbook). Review your application carefully before submitting.

Step 4: Submit COMPLETED application packet to the Allied Health Division office IN PERSON

ONLY. FYI: Mailed applications will not be accepted. If it is determined minimum requirements have not been met or an incomplete application packet is submitted, the applicant will not be considered for the selection process. Incomplete applications will be returned in the self‐addressed stamped envelope you provided.

Step 5: Applicant Notification: ALL applicants will receive a letter indicating their selection status, tentatively three months after the application closing date. LVN to RN applicants are notified of eligibility approximately two‐three months prior to the next semester. The LVN‐RN application period remains open. To progress into the RN program students must successfully complete REGN‐01 LVN to RN transition class which is offered when a sufficient number of qualified applicants exist.

Step 6: LVN to RN Students Only:

Space in the RN program is determined based on the number of RN students progressing from the second semester to the third semester. Students must submit a letter of intent to enroll in the third semester RN class 30 days prior to the next semester. Students are accepted based on availability and by lottery if there is not enough space to accommodate all LVN’s that have completed the bridge (REGN‐01) course.



Acceptance Once accepted into the program, the student must be able to:

Attend a MANDATORY pre‐registration orientation meeting at which time course and registration information is provided.

Attend clinical hours involving day, evening, and weekend shifts. Typical hours are either 6:30am‐2:30pm or 6:30am‐6:30pm. Some traveling to facilities in adjacent counties is required.

Visit the hospital the day or evening prior to scheduled clinical experience for patient data collection.

Submit a completed Health Screening Application verifying current immunizations or titers for DT, MMR, Polio, Hepatitis B, TB, and Varicella. Proof of Hepatitis B immunity or release form is required. Applicants must be in good health to cope with the rigorous requirements of the program.

Submit additional evidence of negative tuberculosis testing–PPD or Chest X‐ray prior to progression to the second year of the program.

Submit proof of current American Heart Association CPR certification for HEALTH CARE PROVIDER.

Pass background checks and/or drug screens performed by the clinical facilities. Submission accomplished through the San Joaquin Valley Nursing Education Consortium https://portal.castlebranch.com/ms64 for background check and drug screening. An additional background check and drug screen is required prior to the start of the fourth semester for the mental health rotation. This requires an active social security number or ITIN (Individual Taxpayer Identification Number).

Failure to meet standards in any of these areas will result in non‐acceptance to the RN program.



General Information for Nursing Applicants

1. Policy for Science Recency Requirements

It is recommended that the science prerequisite courses be completed within five years of acceptance into the program. Whenever a course taken by the applicant is not exactly equal to the prescribed prerequisites, the applicant must first submit an official transcript to the Institutional Evaluator to determine equivalency. The equivalency process must be completed, approved and on file prior to submitting an application to the RN Program. It is the applicants’ responsibility to obtain a copy of past college catalogs or course descriptions, to assist in the establishment of equivalencies.

2. Guidelines for Prerequisite Science Course Equivalencies

Microbiology

Must be at least a four‐semester unit college course with a lab, and content must include the fundamental aspects involving the identification, morphology, metabolism, pathogenicity and genetics of microorganisms.

Anatomy and Physiology

Must be at least four semester unit college courses EACH with a lab. Content must include the relationships of structure and function of the cellular, skeletal, muscular, circulatory, respiratory, digestive, excretory, secretory, reproductive, and central and autonomic nervous systems of the human body.

3. Advanced Placement Admissions

Qualified applicants may apply for advanced placement in the RN program. Advanced placement is defined as entering the program at a level beyond the first semester. Applicants will be admitted on a space available basis only. Admission applications and supporting documents must be submitted to the Allied Health Division at least 90 days before the beginning of the semester. Submission of previous course outlines, syllabi, or other materials and/or passing written and performance placement examinations encompassing previously learned nursing content may be required. Following the evaluation of previous course work a determination of placement will be made. Individuals seeking advanced placement must meet the multi‐criteria selection application requirements for consideration.

4. Readmission Policy & Procedure

Readmission to the Registered Nursing Program after successful completion of the first semester is dependent upon space availability. After review of written and performance achievement evaluations and/or interview with the teaching team and the Program Director, a decision will be made using the following procedure:



Readmission Procedure:

a. A student who withdraws from the program for any reason before successful completion of the first semester must reapply for admission to the program through the general admission process. No special consideration will be given. Applicants who are selected and then declined OR are not successful completing the first semester may apply again but WILL NOT be recognized as a “consecutive” applicant. Applicants may only be accepted to the program a maximum of two times.

b. Students who withdraw from the program after satisfactory completion of one or more semesters must submit a letter requesting reentry into the program to the Allied Health Division 30 days prior to the semester for which they are applying.

c. Readmission of students who have performed unsatisfactorily in the program and for whom withdrawal was recommended or failure of a course resulted, will be based upon the following:

i. Students whose previous performance in a program was not satisfactory will be considered for readmission one time only. Applications must be made within one semester of withdrawal or dismissal.

ii. Submission of a letter requesting reentry into the RN Program to the Allied Health Division 30 days prior to the semester for which they are applying.

iii. Submission of evidence and/or demonstration that instructor's written recommendations for readmission have been satisfactorily fulfilled.

iv. A written and/or performance examination encompassing previous content may be required for assessment purposes.

v. For students dismissed for safety considerations, readmission will not be recommended.

5. Credit by Examination

The Merced College Credit by Examination policy is outlined in the College Catalog. Exams may include teacher‐made final examinations, standardized tests, and/or performance criteria. Course outlines, course objectives and textbook/reference reading lists will be available to the student at least four weeks before the exam is scheduled. Students successfully passing exams will be assigned the grade based on their total score.

6. History of Convictions

Applicants will be required to pass a background check prior to clinical rotations. Applicants should be aware of the provisions of the California Nurse Practice Act which requires the Board of Registered Nursing to review the history of previous misdemeanor and felony convictions. In those cases where offenses bear a substantial relationship to the Nursing profession, the Board of Registered Nursing may withhold licensure. See “Policy Statement on Denial of Licensure” in the appendices. Failure to pass drug screening may result in release from the program.

7. Drug Screening

Applicants will be required to pass a drug screen prior to clinical rotations. Failure to pass drug screening may result in release from the program.

11. 30‐unit option Upon completion of this option an Associate Degree in Nursing (ADN) will not be granted. The school is only the facilitator of the minimum requirements needed to apply for the NCLEX‐RN exam. A degree in General Education or other majors may be obtained at any time.

Employment may be limited. For example, Veterans Administration Hospitals and the military require that an RN have an ADN and/or BSN for employment/commissions respectively.

It is uncertain whether BSN programs will accept a "30‐unit Option" RN from California for admission.

"Conviction" includes a plea of no contest and any conviction that has been set aside or deferred pursuant to Section 1000 or 1203.4 of the Penal Code, including infractions, misdemeanor, and felonies. It is not necessary to report a conviction for an infraction with a fine of less than $1,000 unless the infraction involved alcohol or controlled substances. However, any convictions in which a plea of no contest was entered and any convictions that were subsequently set aside pursuant or deferred pursuant to Section 1000 or 1203.4 of the Penal Code must be disclosed. "License" includes permits, registrations, and certificates. "Discipline" includes, but is not limited to, suspension, revocation, voluntary surrender, probation, or any other restriction.

The following information must be provided for each license discipline or conviction sustained:

A detailed written explanation describing the circumstances and events that led to your arrest(s) and conviction(s), including: date and place of arrest; arresting agency; court where case was heard; and sentencing information, including fines, courses, counseling, restitution, probation, parole, community service, and jail or prison time.

Documents relating to the arrest, such as: police report, arrest report, booking report, complaint, citation or ticket.

Do you have to report an arrest if you were not convicted?

No. You only need to report an arrest if you were convicted.

Do you have to report traffic violations and/or automobile accidents?

Yes, if a traffic ticket was issued and you were fined over $1,000.

What happens if you can't obtain certified court documents and police reports on convictions?

You must submit written proof from the court or police department specifying inability to locate the required documents. The Board verifies this and bases its decision on other substantial information (e.g., letters of recommendation, evidence of rehabilitation, etc.).

Do you have to report non-payment of child support?

No. However, if your case is in the District Attorney's (DA) Office for non-payment, the DA notifies the Department of Consumer Affairs Family Support Unit. If you are deemed eligible for licensure, your license is issued on a temporary basis for 150 days. If at the end of that time period, you have not established a payment agreement with the DA's Office, your license is suspended.

Do you have to report a conviction that was expunged (i.e., conviction was stricken or deleted from official records)?

Yes. Pursuant to Penal Code section 1203.4, you are required to report a conviction that was expunged. The law specifies that it does not relieve you from the obligation to disclose the conviction in response to any direct questions contained in any questionnaire or application for public office, for licensure by any state or local agency.

What happens if you fail to disclose all or part of your convictions?

Failure to disclose all or part of your convictions may be grounds for disciplinary action as you falsified information required on your renewal of licensure.



Do you have to report misdemeanors, felonies and/or convictions which you pled nolo contendere?

Yes. Pursuant to Business and Professions Code, section 2765, a plea or verdict of guilty or a conviction following a plea of nolo contendere is considered a conviction. Any conviction substantially related to the qualifications, functions and duties of a licensee can be grounds for discipline of a license.

Do you have to report convictions if you were under 18 years old and tried as an adult?

Yes. If you were under 18 years of age, but were tried and convicted as an adult, you are required to disclose the conviction.

Do you have to report military convictions or dishonorable discharge?

Yes. Any offense or violation during your military career must be reported to the Board.

How does the Board determine which violations are substantially related to the applicant's license?

Pursuant to Title 16, California Code of Regulations, Section 1444, a conviction or act shall be considered to be substantially related to the qualifications, functions or duties of a registered nurse if to a substantial degree it evidences present or potential unfitness of a registered nurse to practice in a manner consistent with the public health, safety or welfare. Such convictions or acts shall include but not be limited to those involving the following issues:

o Assaultive or abusive conduct including, but not limited to, those violations listed in subdivision (d) of Penal Code Section 11160.

o Failure to comply with any mandatory reporting requirements.

o Theft, dishonesty, fraud, or deceit.

o Any conviction or act subject to an order of registration pursuant to Section 290 of the Penal Code.

How does the Board determine satisfactory rehabilitation of an "applicant for licensure" or licensee?

Pursuant to Business and Professions Code sections 480 and 490, and Title 16 California Code of Regulations, Section 1444, the Board utilizes the following general criteria to determine satisfactory



rehabilitation:

o The nature and severity of the act(s) or crime(s) under consideration.

o Evidence of any act(s) committed subsequent to the act(s) or crime(s) under consideration.

o The time that has elapsed since commission of the act(s) or crime(s) under consideration.

o The extent to which the applicant has complied with any terms of parole, probation, restitution, or any other sanctions lawfully imposed.

o Evidence, if any, of rehabilitation.

What does the Board look for on the Department of Justice (DOJ) and/or Federal Bureau of Investigation (FBI) reports?

The Board reviews the RAP sheets (i.e., criminal record reports) to ensure that the conviction history matches what you provided on your Report of Conviction form and to determine if the violation is related to the qualifications of a licensee.

How can you get a copy of your RAP sheet ( i.e., criminal record report)?

You may request a copy of your RAP sheet from the Department of Justice (DOJ). DOJ requires you to complete a "Record Review Process" form, complete another fingerprint card and pay a fee for processing. To obtain more information on this process, you must contact DOJ directly.

How long does a conviction stay on your RAP sheet?

The information contained on your RAP sheet is retained indefinitely. Therefore, you should always report all convictions. Failure to disclose all past convictions may result in disciplinary action.

The entire application, pages 29‐42 (except Criteria IV‐Foreign Language, pg.41) in this handbook are “fillable” forms. You must complete the forms on a computer by typing information into the form’s displayed fields (page 41 is the only exception, this page will need to be filled out by hand). Once all of the necessary fields have been filled out, you will then print your completed forms, sign and date where required, and submit them to the Allied Health office during the day and times they will be accepted (listed on the front page of this handbook). Review your application carefully before submitting.

All applications submitted on the application day will be reviewed at a later date. If it is determined minimum requirements have not been met or an incomplete application packet is submitted, the applicant will not be considered for the selection process. Incomplete applications will be returned in the self‐addressed stamped envelope provided by the applicant.

Applicants applying to the program a second, third or fourth time must submit pages 29, 30 & 31. The Allied Health Office has your file from the previous years’ selection. However, the applicant will be responsible for updating any other information that may change their point total and must comply with local and/or state requirements. Refer to page 12. Applicants who have completed courses at another educational institution will need to request two official sealed transcripts to be sent to you directly (the official transcripts must be from other schools attended, NOT Merced College). *To be official, transcripts must remain sealed in the original envelope. It is the applicants responsibility to obtain a copy of past college catalogs or course descriptions, to assist in the establishment of equivalencies.

All Students applying for the Merced College Registered Nursing program must meet the minimum passing score of 62% on Version V or ATI TEAS test. As a reminder – students must receive 62% or higher on their FIRST ATTEMPT of the ATI TEAS exam to apply for the Registered Nursing Program.

For those students who were unsuccessful passing TEAS V, OR would like to submit a higher “ATI TEAS” score than their score on TEAS V, multiple attempts on the ATI score report will be accepted (with only one failed attempt on TEAS V). You must submit test results for other attempts (possibly for earlier versions). Applications will not be accepted for ATI test results that show the “first attempt” was an “ATI TEAS” score that was not passing.

You must attach your TEAS V or ATI TEAS test results (Individual Performance Profile printout). FYI – If your test results for TEAS V or ATI TEAS states that there has been more than one attempt, you must submit test results for “other attempts” (possibly for past versions). If you are selected to begin the RN program you may be required to submit an Official TEAS result score through the testing agency.
